Dosage
Administer 20 to 500 mL intravenously as a single dose. The infusion rate of the intravenous injection should be kept below 0.5 g/kg per hour as glucose. An appropriate amount should be used for dilution of the injection solution. Adjust according to age and symptoms.
Precautions
Caution is required when opening the ampule as glass fragments may be mixed in, especially when used for children or the elderly to ensure no glass fragments enter. The glucose contained in this medication can increase the consumption of vitamin B1 (thiamine) in the body during intravenous infusion, potentially leading to severe deficiency, thus caution is needed. This medication should not be administered to patients with specific conditions such as storage dehydration, hyperhydration, hyperglycemia, acidosis, hypokalemia, hyperosmolar coma, glucose intolerance, anuria, and hepatic coma. In patients with spinal hemorrhage, cerebral hemorrhage, or tremor delirium, the hypertonic solution form of this medication should also not be used. Patients with a tendency for potassium, phosphorus, magnesium deficiency or conditions such as hyponatremia, diabetes insipidus, renal failure, heart failure, severe malnutrition, thiamine deficiency, sepsis, critically ill or trauma patients should use this medication cautiously. Rapid or large-volume administration of this medication may cause electrolyte imbalances or fluid overload, so it should be administered slowly, and exceeding the recommended dose may increase serum bilirubin and lactate levels. Post-administration, side effects such as hypokalemia, hyperosmolar coma, acidosis, dehydration, hyperosmolarity, polyuria, phlebitis, thrombosis, and injection site pain may occur, so caution is required. Long-term use may affect insulin secretion, necessitating insulin administration if needed, and patients after surgery or with glucose intolerance should strictly monitor blood glucose levels. During pregnancy, safety for the fetus has not been established, so it should only be used when the benefits of treatment outweigh the risks, and during labor, there is a risk of hyperglycemia and metabolic acidosis for the mother and newborn, thus careful consideration is needed. Infants, especially premature and low birth weight infants, are at high risk for hypoglycemia or hyperglycemia, requiring close monitoring of blood glucose levels, and older adults require more cautious administration due to decreased bodily function. Excessive dosing may cause hyperglycemia or diabetes, necessitating insulin therapy in such cases. Subcutaneous injections can cause electrolyte imbalances and local irritation and should be avoided, and the solution remaining after opening should be discarded. To reduce the risk of infection during administration, cleanliness should be maintained during the injection process, and the medication should be warmed to body temperature before use. Particular caution is required during hypertonic solution administration due to the risk of thrombophlebitis, and sudden discontinuation after high concentration solution administration may induce hypoglycemia. Do not administer simultaneously with blood, and if mixing with other medications, ensure careful verification of chemical and physical incompatibilities before administration.
