Treatment of infections (including pneumonia, skin and soft tissue infections, and endocarditis)
This drug is a bactericidal glycopeptide antibiotic effective against Gram-positive aerobic and anaerobic strains, particularly effective against various strains including methicillin-resistant Staphylococcus aureus. It is used to treat skin and soft tissue infections, osteoarticular infections, hospital-acquired and community-acquired pneumonia, complicated urinary tract infections, infectious endocarditis, and peritonitis due to continuous ambulatory peritoneal dialysis. This medication also helps improve bacteremia associated with infections. It does not show cross-resistance with various antibiotics, and excellent therapeutic efficacy is expected for strains with a certain level of susceptibility. This drug can effectively improve the treatment of infections in patients.
Adults and older adults with normal renal function Skin and soft tissue infections, pneumonia, complicated urinary tract infections: 6 mg/kg intravenously or intramuscularly every 12 hours for 3 doses; then 6 mg/kg intravenously or intramuscularly once daily Bone and joint infections: 12 mg/kg intravenously every 12 hours for 3 ∼ 5 doses; then 12 mg/kg intravenously or intramuscularly once daily Infective endocarditis: 12 mg/kg intravenously every 12 hours for 3 ∼ 5 doses; then 12 mg/kg intravenously or intramuscularly once daily Calculate and administer the dose according to the patient's body weight (kg). The duration of administration should be determined based on clinical response. For infective endocarditis, treatment for at least 21 days is appropriate. Treatment must not exceed 4 months. Older adults: Dose adjustment is not necessary unless renal impairment is present. Adults and older adults with renal impairment: In patients with renal impairment, dose reduction is not necessary until Day 4; however, dose adjustment is necessary to maintain a minimum serum trough concentration of 10 mg/L. After Day 4 of administration Mild to moderate renal impairment (creatinine clearance 30 ∼ 80 mL/min): Halve the dose by administering the initial unit dose every 2 days or one-half of the initial dose daily. Severe renal impairment (patients with creatinine clearance below 30 mL/min) and patients on hemodialysis: Administer the initial unit dose every 3 days or one-third of the initial dose once daily. This drug is not removed by dialysis. Patients on continuous ambulatory peritoneal dialysis: If the patient has a fever, administer a loading dose of 400 mg (6 mg/kg) by intravenous injection once, followed by 20 mg per liter of dialysis solution in every dialysis bag during the first week, every other bag during the second week, and only the overnight bag during the third week. Administration is generally once daily; however, in severe infections, administer twice on the first day of treatment to rapidly achieve the desired blood concentration. Most infections caused by susceptible strains respond to treatment within 48 ∼ 72 hours. The total duration of treatment is determined by the type and severity of the infection and the patient's clinical response. Measurement of serum concentrations of this drug can help optimize treatment. In severe infections, the serum trough concentration must not be below 10 mg/L. The peak concentration measured 1 hour after intravenous administration of 400 mg is generally in the range of 20 ∼ 50 mg/L, and concentrations up to 250 mg/L have been reported after intravenous administration of 25 mg/kg. The relationship between serum concentration and toxicity has not been established.
Children (2 months ∼ 12 years of age): Administer an initial dose of 10 mg/kg intravenously every 12 hours for 3 doses, followed by a maintenance dose of 6 ∼ 10 mg/kg intravenously once daily. Neonates (up to 2 months after birth): Administer an initial dose of 16 mg/kg by intravenous infusion once on the first day, followed from the next day by a maintenance dose of 8 mg/kg once daily by intravenous infusion over 30 minutes.
This drug is a glass ampule injection, so care must be taken to avoid glass fragments mixing during use. Extra caution is needed, especially when used in children and the elderly. This drug can cause anaphylactic shock, a severe allergic reaction; therefore, if any allergic symptoms appear, administration must be stopped immediately, and emergency treatment should be received. Caution is warranted for patients with allergies to vancomycin-class drugs. During injection, infusion-related reactions such as shock or skin rash, itching may occur, so the infusion speed should be slow, and it should be administered via slow intravenous infusion over more than 30 minutes. Rapid intravenous injection should be avoided. This drug may cause serious skin conditions such as Stevens-Johnson syndrome or toxic epidermal necrolysis; if any abnormal skin symptoms occur, administration must be stopped immediately. Adverse effects such as hearing loss, blood disorders, and liver or kidney dysfunction may occur; therefore, especially for patients with impaired kidney function or those receiving long-term treatment, regular hearing tests, blood tests, and liver and kidney function evaluations should be conducted. Intraventricular administration must absolutely be avoided due to the risk of seizures. This drug should not be administered to patients with hypersensitivity to it, and it should be used cautiously in patients with hypersensitivity to vancomycin-class or aminoglycoside-class drugs. For patients with impaired kidney or liver function or in the elderly, the concentration of this drug may accumulate in the body, increasing the risk of adverse effects, thus it should be used cautiously. During administration, dizziness or headache may occur; it is advisable to avoid dangerous activities such as driving or operating machinery. In cases of overdose, symptoms such as thrombocytopenia or anxiety may appear, and this drug is not removed by hemodialysis but is slowly removed through peritoneal dialysis. Symptomatic treatment is required in case of overdose. This drug is used for infection treatment, but to prevent the development of resistant bacteria, susceptibility testing should be conducted, and it should be used only for the minimum necessary duration. Also, during long-term use, the risk of superinfection should be monitored. This drug can be diluted with certain glucose solutions or saline injection solutions; the solution should be used as soon as possible after dissolution, and any remaining solution should be discarded within 24 hours. Women who are pregnant or breastfeeding should carefully consider the potential risks and benefits when using this drug.
