Efficacy & effects

This injectable medication helps reduce the toxicity of folate metabolism antagonists. Additionally, it is used in conjunction with 5-Fluorouracil (5-FU) to enhance treatment effects during the treatment of metastatic colorectal cancer. Through this action, it alleviates potential side effects that may arise during treatment and improves cancer treatment efficacy.

Dosage

Precautions

This medication is not suitable for treating malignant anemia due to vitamin B12 deficiency or other megaloblastic anemias. Although the hematological condition may improve while neurological symptoms continue to progress, neurological symptoms may worsen. When using the injectable ampoule, there is a risk of glass shards contaminating the solution when cutting the glass vials, so extra caution should be taken when administering to children and the elderly. Women who are pregnant or breastfeeding should not use this medication when administered together with 5-FU, and care should be taken when deciding to use it for alleviating the toxicity of folate antagonists. Allergic reactions such as hives or anaphylaxis may occur when using this medication. Pain at the injection site or temporary discomfort may arise, and, rarely, fever or central nervous system abnormal symptoms may occur. When used in combination with 5-FU, various side effects such as diarrhea, skin reactions, and mucositis may occur; therefore, if adverse reactions are severe, dose adjustments for 5-FU may be necessary. This medication should be used according to the instructions of a physician experienced in chemotherapy and timing intervals should be well-maintained during administration to ensure the toxicity mitigation effect is retained. Serum methotrexate concentrations should be regularly monitored to adjust treatment duration and dosage, and dose adjustments may be necessary if there are issues with renal function or fluid status in the body. This medication utilizes the same transport system as methotrexate, thus patients who are resistant to methotrexate may also be resistant to this drug. In case of vomiting or difficulty with absorption, non-oral administration is recommended. For patients with central nervous system tumors, repeated administration may lead to accumulation of the drug, reducing the anticancer effect, so overdose should be avoided. This medication is not used for the treatment of macrocytosis caused by specific DNA synthesis inhibitors, nor is it effective against non-hematological toxicities such as nephrotoxicity from methotrexate. When used with other medications, it can enhance liver metabolism and may lower the blood concentration of certain anticonvulsants, requiring careful dosage adjustments and monitoring of blood concentrations. Caution should be exercised when used in conjunction with 5-FU, as it can increase the effect and side effects of 5-FU. When administered with methotrexate and other antifolate chemotherapeutic agents, the anticancer effect may decrease or be completely eliminated. In pregnant women, administration should be for essential cases only, considering potential risks to the fetus, and for breastfeeding mothers, caution is required due to unclear excretion into breast milk. In elderly patients, the risk of gastrointestinal toxicity may increase, so particular caution is necessary, and consideration should be given to decreased physiological functions when administering. This medication should not be administered intrathecally, and due to calcium content, intravenous administration rates should be limited per minute to ensure safety.
