Dosage

Acute diarrhea: The initial dose is 4 mg of loperamide hydrochloride administered orally, with a maintenance dose of 2 mg for each occurrence of loose stools. The usual daily dose is 6-8 mg, and the maximum daily dose is 16 mg. Chronic diarrhea: The initial dose is 4 mg of loperamide hydrochloride administered orally, with a maintenance dose of 2 mg for each occurrence of loose stools until diarrhea resolves. After that, the dose should be adjusted individually according to patient needs.

Precautions

This medication should not be used in infants under 24 months old or children with chronic diarrhea. It should also not be administered to patients with bacterial diarrhea accompanied by bloody stools or high fever. Patients with acute ulcerative colitis or pseudomembranous colitis associated with extensive antibiotic use should not use this medication. Patients with serious gastrointestinal diseases such as bowel obstruction, megacolon, or toxic megacolon should not receive this medication, and if symptoms of constipation, abdominal distension, or intestinal obstruction occur, use should be stopped immediately. Patients who have previously shown allergic reactions to this medication should also avoid it. In patients with hemorrhagic colitis or those infected with certain bacterial diarrhea, symptoms may worsen with this medication, so caution is advised. Furthermore, patients with lactose intolerance or specific genetic metabolic disorders should not take this medication. Patients with compromised liver function should use this medication cautiously as it may affect the metabolism of the drug, and symptoms such as dizziness and drowsiness may occur during treatment, necessitating caution when driving or operating machinery. If diarrhea persists for more than 48 hours (acute) or 10 days (chronic), medication use should be stopped and a doctor consulted. Women who are breastfeeding are not recommended to use this medication as it may be excreted in small amounts in breast milk. Elderly patients may have reduced body functions, so dose adjustment and careful observation are necessary. In case of overdose, serious side effects such as central nervous system depression, respiratory depression, and cardiac arrhythmia may occur, so the prescribed dosage should not be exceeded, and immediate medical attention should be sought for treatment in the event of an overdose. Finally, it is advisable to avoid administering this medication in tablet or capsule form to children under 7 years of age.

Side effects

Severe hypersensitivity reactions including urticaria, angioedema, toxic epidermal necrolysis, erythema multiforme, vesicular rash including Stevens-Johnson syndrome, anaphylactic shock, and anaphylactic reactions, intestinal obstruction, abdominal distension, toxic megacolon, acute pancreatitis, severe allergic reactions, severe skin adverse reactions, severe edema, or urinary abnormalities.

Interactions

This medication should be discussed with a doctor or pharmacist when taken in conjunction with quinidine or ritonavir.
