This medication is used for patients with Chronic Obstructive Pulmonary Disease (COPD). It is particularly used for moderate and severe patients whose symptoms are not adequately controlled with combination therapy of long-acting beta2-agonists and inhaled corticosteroids or long-acting muscarinic antagonists to relieve symptoms and reduce exacerbations. This medicine works by reducing inflammation in the airways and helping to dilate the bronchi to improve breathing. It helps to maintain lung function and improve the quality of life through continuous maintenance therapy. Therefore, it is effective for symptom management and prevention of exacerbations in patients with moderate to severe COPD.

Dosage and Administration

Adult dosage

This medication is for inhalation, 2 puffs per dose, up to 2 times a day (morning and evening). If a dose is missed, it should be taken as soon as possible, and the next dose should be at the originally scheduled time. The dose should not be doubled.

Children's dosage

This medication is not administered to children and adolescents (under 18 years of age) for chronic obstructive pulmonary disease.

Precautions

This medication should not be used in acute situations where bronchospasm suddenly worsens. It is not to be administered to asthma patients, as safety and efficacy in asthma have not been established. Individuals who are allergic to any components or excipients of this medication should not use it. Patients must not abruptly stop using this medication; immediate medical attention is required if symptoms do not improve even when the recommended dose is used. Particularly, if chronic obstructive pulmonary disease suddenly worsens, rapid medical assessment is essential as it can be life-threatening. Caution is also needed when switching from oral steroids because there may be a risk of adrenal insufficiency. Additional steroid treatment may be necessary in cases of significant stress or upcoming surgery. This medication can cause paradoxical bronchospasm, similar to other inhalation therapies, so if breathing difficulties or wheezing suddenly worsen, use should be discontinued immediately and medical assistance sought. It is not suitable for the treatment of acute bronchospasm or exacerbations of COPD. Individuals with cardiovascular diseases such as heart disease, hyperthyroidism, myocardial infarction, and heart failure should use caution. The inhaled corticosteroid component included in this medication may lead to systemic side effects with long-term high-dose use, increasing the risk of diseases like osteoporosis, cataracts, and glaucoma. It is advisable for those at high risk of osteoporosis to have regular bone density checks. Patients with glaucoma or cataracts or those with closed-angle glaucoma should carefully monitor symptoms like increased intraocular pressure or eye pain while using this medication. Blood potassium levels may temporarily decrease, and diabetic patients need to be attentive to blood sugar control. The presence of anticholinergic components in this medication means that caution is necessary for those with benign prostatic hyperplasia, urinary retention, or closed-angle glaucoma. In patients with severe renal impairment or those requiring dialysis, the benefits and risks of using this medication should be carefully considered. Patients with severe liver dysfunction should also closely monitor liver function status when using this medication. The risk of pneumonia may increase with inhaled steroid use in COPD patients, so patients should consult a doctor immediately if pneumonia symptoms appear. This medication consists of multiple components, and side effects related to each component may occur, so medical consultation is advised if any unusual symptoms develop.
