Summary

Main use
Treatment and prevention of influenza (type A·type B) infection
Who takes it
Adults · Children
Adult dose
Treatment of influenza A and influenza B virus infections: Initiate treatment on the first or second day after the onset of influenza symptoms. This drug may be administered without regard to food; in some patients, tolerability may be improved when taken with food. Adults and adolescents aged 13 years and older: Administer 75 mg of oseltamivir orally twice daily for 5 days. Adults and adolescents aged 13 years and older who have difficulty swallowing capsules should use this drug powder for suspension. Patients with renal impairment (treatment): No dosage adjustment is required in patients with creatinine clearance of 60 mL/min or greater. For patients with creatinine clearance of 30 ~ 60 mL/min, a reduced dose of 30 mg twice daily for 5 days is recommended. For patients with creatinine clearance of 10 ~ 30 mL/min, a reduced dose of 30 mg once daily for 5 days is recommended. Patients receiving regular hemodialysis should receive a 30mg dose of this drug after each dialysis session to maintain blood concentrations within the therapeutic range; the duration of administration should not exceed 5 days. If influenza symptoms occur during the 48 hours between dialysis sessions, the dose may be administered before the start of dialysis. For peritoneal dialysis, a single 30mg dose of this drug after dialysis is recommended. The pharmacokinetics of oseltamivir have not been studied in patients with end-stage renal disease who are not receiving dialysis (e.g., creatinine clearance less than 10mL/min); therefore, no recommended dose is available for these patients. Patients with hepatic impairment (treatment): No dosage adjustment is required for treatment in adult patients with mild to moderate hepatic impairment (Child-Pugh score ≤ 9). Prevention of influenza A and influenza B virus infections—Adults and adolescents aged 13 years and older: For close contacts of an infected person, administer 75 mg of oseltamivir orally once daily for 10 days. Start administration within 2 days of contact with the infected person. The recommended dose for prevention during an influenza outbreak is 75 mg once daily. The safety and efficacy of this drug have been demonstrated for up to 6 weeks. The preventive effect continues while taking this drug. Patients with renal impairment (prevention): No dosage adjustment is required in patients with creatinine clearance of 60 mL/min or greater. For patients with creatinine clearance of 30 ~ 60 mL/min, a reduced dose of 30 mg once daily is recommended. For patients with creatinine clearance of 10 ~ 30 mL/min, a reduced dose of 30 mg once daily every other day is recommended. Patients receiving regular hemodialysis should receive a 30mg dose of this drug after every 2 dialysis sessions to maintain blood concentrations within the therapeutic range. The initial dose may be administered before the start of dialysis. For peritoneal dialysis, administration of 30mg of this drug after dialysis once weekly is recommended. The pharmacokinetics of oseltamivir have not been studied in patients with end-stage renal disease who are not receiving dialysis (e.g., creatinine clearance less than 10mL/min); therefore, no recommended dose is available for these patients. Patients with hepatic impairment (prevention): No dosage adjustment is required for prevention in adult patients with mild to moderate hepatic impairment (Child-Pugh score ≤ 9).

This medication is used for patients infected with influenza A and B viruses ranging from newborns over 2 weeks old to adults. It is especially important to start treatment within 48 hours after the first onset of influenza symptoms. It can also be used for the prevention of influenza in children over 1 year of age, but this is limited to cases where vaccination is not feasible or during outbreaks not covered by the vaccine. This medication cannot replace vaccination, so if vaccination is possible, it should be prioritized. It is effectively used for the treatment and prevention of influenza infections.

Precautions

This medication is used for the treatment and prevention of influenza virus infections, but it cannot replace influenza vaccinations and it is important to use it in conjunction with the vaccine. This medication has been proven effective only against influenza A and B viruses and should not be used for other types of infections. Particularly in children and adolescents, neuropsychiatric adverse reactions such as abnormal behavior, seizures, and delirium have been reported, so careful observation by guardians is necessary for patients in this age group. Patients with known allergies to this medication or its ingredients should not use it. Dose adjustments are necessary for patients with impaired renal function or those undergoing dialysis, and it is not recommended for patients with end-stage renal failure. Common side effects that may occur during treatment include nausea, vomiting, and headaches, which are usually temporary and mild symptoms. Serious side effects such as anaphylaxis, liver dysfunction, skin rashes, acute renal failure, and neuropsychiatric symptoms may occur infrequently, so if such symptoms arise, immediate consultation with a healthcare provider is necessary. Women who are pregnant or breastfeeding should use this medication only when the benefits of treatment are deemed greater than the risks. Safety and efficacy have not been established for children under 1 year old, so usage is not recommended. For the elderly, dose adjustment is not necessary; however, careful monitoring is needed if there is physical decline or pre-existing medical conditions. In the case of overdose, there is no specific antidote, so immediate medical attention should be sought for appropriate measures. This medication has a low potential for interactions with other medications; however, caution may be needed when used concurrently with specific drugs, so any concurrent medication should be disclosed. Store in a place out of reach of children, and do not transfer to another container for safety purposes.
