Summary

Main use
Treatment of influenza A and B infections; prevention of influenza
Who takes it
Adults · Children
Adult dose
Treatment of influenza A and influenza B virus infections Initiate treatment on the first or second day after the onset of influenza symptoms. This medicine may be taken with or without food; in some patients, taking it with food may improve tolerability. Adults and adolescents 13 years of age and older: Administer oseltamivir 75 mg orally twice daily for 5 days. Adults and adolescents 13 years of age and older who have difficulty swallowing capsules should use this medicine. Patients with renal impairment: No dosage adjustment is required for patients with creatinine clearance ≥ 60 mL/min. For patients with creatinine clearance of 30 ~ 60 mL/min, reduction to 30 mg twice daily for 5 days is recommended. For patients with creatinine clearance of 10 ~ 30 mL/min, reduction to 30 mg once daily for 5 days is recommended. Patients receiving regular hemodialysis should receive a 30mg dose of this medicine after each dialysis session to maintain therapeutic plasma concentrations; treatment must not exceed 5 days. If influenza symptoms develop during the 48 hours between dialysis sessions, the dose may also be administered before the start of dialysis. For peritoneal dialysis, a single 30mg dose of this medicine after dialysis is recommended. The pharmacokinetics of oseltamivir have not been studied in patients with end-stage renal disease who are not receiving dialysis (e.g., creatinine clearance < 10mL/min); therefore, no recommended dose is available for these patients. Patients with hepatic impairment: No dosage adjustment is required for treatment in adults with mild to moderate hepatic impairment (Child-Pugh score ≤ 9). Prevention of influenza A and influenza B virus infections Adults and adolescents 13 years of age and older: For close contacts of an infected person, administer oseltamivir 75 mg orally once daily for 10 days. Begin administration within 2 days of contact with the infected person. The recommended dose for prevention during an influenza outbreak is 75 mg once daily. The safety and efficacy of this medicine have been demonstrated for up to 6 weeks. The preventive effect continues while taking this medicine. Patients with renal impairment: No dosage adjustment is required for patients with creatinine clearance ≥ 60 mL/min. For patients with creatinine clearance of 30 ~ 60 mL/min, reduction to 30 mg once daily is recommended. For patients with creatinine clearance of 10 ~ 30 mL/min, reduction to 30 mg every other day is recommended. Patients receiving regular hemodialysis should receive a 30mg dose of this medicine after every 2 dialysis sessions to maintain therapeutic plasma concentrations. The initial dose may be administered before the start of dialysis. For peritoneal dialysis, administration of 30mg of this medicine once weekly after dialysis is recommended. The pharmacokinetics of oseltamivir have not been studied in patients with end-stage renal disease who are not receiving dialysis (e.g., creatinine clearance < 10mL/min); therefore, no recommended dose is available for these patients. Patients with hepatic impairment: No dosage adjustment is required for prevention in adults with mild to moderate hepatic impairment (Child-Pugh score ≤ 9).

Precautions

This drug is used for the treatment and prevention of influenza virus infections and cannot replace influenza vaccines. This drug is effective only against influenza A and B viruses, and safety and efficacy have not been confirmed for other infections. Especially, neurological or psychiatric adverse reactions such as seizures or abnormal behaviors have been reported after taking this drug in children and adolescents, so if abnormal behavior occurs, immediate careful observation by a guardian is required. The use of this drug in children over 10 years old is not recommended except in special cases. This drug should not be administered to patients with specific genetic disorders, such as lactose intolerance or glucose-galactose malabsorption. Patients with decreased kidney function or those on dialysis require dose adjustment, and it is not recommended for patients with end-stage renal disease. During administration of this drug, side effects such as nausea, vomiting, headache, and dizziness may commonly occur, but most are temporary. Rarely, serious adverse reactions such as shock, liver dysfunction, skin rash, and psychiatric symptoms may occur, so if symptoms appear, immediate consultation with medical staff is necessary. Pregnant or breastfeeding women should carefully consider the benefits and risks of administering this drug and consult a doctor if necessary. In children, it is advisable not to use in those under 1 year of age as safety and efficacy have not been established. The elderly can use this drug without special dosage adjustments but should be monitored closely due to decreased bodily function. This drug has a low likelihood of drug interactions, but caution may be needed when co-administered with certain medications. If abnormal behavior or changes in mental state occur during administration of this drug, stop taking it immediately and inform medical personnel. When taking this medication, follow the prescribed dosage and if a dose is missed, take it as soon as possible, ensuring it is not too close to the next dosing time. This drug should be stored out of reach of children and used within the specified period after preparation.
