Treatment and prevention of influenza A and B infections
This medication is used to treat influenza A and B virus infections when administered within 48 hours of the onset of early symptoms in children aged 2 weeks and older and adults. It can also be used for the prevention of influenza virus infections in individuals aged 1 year and older; however, vaccination is the primary preventive measure, and this medication should only be used as an adjunct when vaccine efficacy is not anticipated or vaccination is not possible. This medication cannot replace vaccination, so it should only be considered for use in conjunction with or as a complement to vaccines. Therefore, this medication is used as a agent to assist in the treatment and prevention of influenza infections.
Influenza A and influenza B virus infections: Initiate treatment on the first or second day after the onset of influenza symptoms. This medicine may be administered with or without food; in some patients, taking it with food may improve tolerability. Adults and adolescents 13 years of age and older: Administer oseltamivir 75 mg orally twice daily for 5 days. Adults and adolescents 13 years of age and older who have difficulty swallowing capsules should use this medicine as powder for suspension. Patients with renal impairment: No dosage adjustment is necessary for patients with a creatinine clearance of 60 mL/min or greater. For patients with a creatinine clearance of 30 ~ 60 mL/min, a reduced dose of 30 mg twice daily for 5 days is recommended. For patients with a creatinine clearance of 10 ~ 30 mL/min, a reduced dose of 30 mg once daily for 5 days is recommended. Patients receiving periodic hemodialysis should receive a 30mg dose of this medicine after each dialysis session to maintain plasma concentrations within the therapeutic range; the duration of administration should not exceed 5 days. If influenza symptoms develop during the 48 hours between dialysis sessions, the dose may also be administered before dialysis begins. For peritoneal dialysis, a single 30mg dose of this medicine after dialysis is recommended. The pharmacokinetics of oseltamivir have not been studied in patients with end-stage renal disease who are not receiving dialysis (e.g., creatinine clearance less than 10mL/min); therefore, no recommended dose is available for these patients. Patients with hepatic impairment: No dosage adjustment is necessary for treatment in adult patients with mild to moderate hepatic impairment (Child-Pugh score ≤ 9). Prevention of influenza A and influenza B virus infections Adults and adolescents 13 years of age and older: For close contacts of an infected person, administer oseltamivir 75 mg orally once daily for 10 days. Start administration within 2 days of contact with the infected person. The recommended dose for prevention during an influenza epidemic is 75 mg once daily. The safety and efficacy of this medicine have been demonstrated for up to 6 weeks. The preventive effect continues while this medicine is being taken. Patients with renal impairment: No dosage adjustment is necessary for patients with a creatinine clearance of 60 mL/min or greater. For patients with a creatinine clearance of 30 ~ 60 mL/min, a reduced dose of 30 mg once daily is recommended. For patients with a creatinine clearance of 10 ~ 30 mL/min, a reduced dose of 30 mg every other day is recommended. Patients receiving periodic hemodialysis should receive a 30mg dose of this medicine after every 2 dialysis sessions to maintain plasma concentrations within the therapeutic range. The initial dose may be administered before dialysis begins. For peritoneal dialysis, administration of 30mg of this medicine once weekly after dialysis is recommended. The pharmacokinetics of oseltamivir have not been studied in patients with end-stage renal disease who are not receiving dialysis (e.g., creatinine clearance less than 10mL/min); therefore, no recommended dose is available for these patients. Patients with hepatic impairment: No dosage adjustment is necessary for prevention in adult patients with mild to moderate hepatic impairment (Child-Pugh score ≤ 9).

This medication cannot replace influenza vaccination and is only to be used for treatment and prevention during periods when influenza virus is circulating. The safety and efficacy of this medication have not been established for the treatment of infections other than influenza A and B. Particularly, neuropsychiatric adverse reactions such as seizures or delirium have been rarely reported, especially in pediatric and adolescent patients; therefore, close monitoring by guardians is necessary in the event of abnormal behavior. It is generally recommended not to use this medication in children over 10 years of age, except in special cases, due to the risk of abnormal behavior-related accidents. During treatment with this medication, there is a risk of abnormal behavior; therefore, guardians should carefully supervise children and adolescents for at least 2 days. Patients with known allergies to this medication or its components should not use it. Patients with renal impairment or those undergoing dialysis may require dosage adjustments, and it is not recommended for patients with end-stage renal failure. Caution is necessary in high-risk patients with asthma, chronic bronchitis, or immunosuppression. Common side effects may include nausea, vomiting, headache, and pain, but most are temporary. Rarely, symptoms such as shock, anaphylaxis, pneumonia, liver function abnormalities, skin phenomena, acute renal failure, leukopenia, and neuropsychiatric symptoms may occur; if any abnormal symptoms appear, immediate consultation with a healthcare professional is recommended. This medication should only be used in pregnant or nursing women when the therapeutic benefits are judged to outweigh the risks, as safety data is limited. Safety and efficacy in children under 1 year of age have not been established; therefore, its use is not recommended. For elderly patients, there is generally no need for specific dose adjustments; however, careful observation is necessary considering decreased physical function and existing medical conditions. Severe adverse reactions related to overdose have been rarely reported, but adherence to prescribed usage and dosage is essential. This medication has a low possibility of interactions with other drugs, though caution may be warranted when combined with certain medications. When used alongside the influenza vaccine, it should be administered with a certain interval from live attenuated influenza vaccines, but can be given at any time with inactivated vaccines. Patients should be aware of the potential for impaired ability to drive vehicles or operate machinery due to influenza symptoms when taking this medication.
