Treatment of HIV-1 infection in combination with other antiretroviral agents
This medication is used for the treatment of human immunodeficiency virus type 1 (HIV-1) infection in adults and adolescents aged 12 years and older with a weight of at least 35 kg, and is used in combination with other antiretroviral agents to enhance treatment efficacy.
Treatment with this medicine should be initiated by a physician experienced in the management of HIV infection. Adults and adolescents (12 years of age or older and weighing 35 kg or more) The dosing regimen for this medicine is shown in Table 1. Table 1: Dose of this medicine according to the third agent in the HIV treatment regimen Dose of this medicine Third agent in the HIV treatment regimen (see “Precautions for Use '5. Renal impairment No dose adjustment of this medicine is necessary in adults or adolescents (12 years of age or older and weighing 35 kg or more) with an estimated creatinine clearance (estimated CrCl) of 30 mL/min or greater. If the estimated creatinine clearance decreases to less than 30mL/min during treatment with this medicine, discontinue this medicine. No dose adjustment is necessary in adults with end-stage renal disease receiving chronic hemodialysis (estimated creatinine clearance less than 15mL/min). However, this medicine should not generally be used, and may be used in a patient only when the potential benefit outweighs the potential risk (see “Precautions for Use 4. General Precautions, 9) Onset or Worsening of Renal Impairment”). On dialysis days, take this medicine after dialysis is completed. The safety of this medicine has not been established in patients with an estimated creatinine clearance of 15 mL/min or greater and less than 30 mL/min, or in patients with an estimated creatinine clearance less than 15 mL/min who are not receiving hemodialysis; therefore, it should not be used in these patients. Hepatic impairment No dose adjustment of this medicine is necessary in patients with hepatic impairment. This medicine may be taken orally with or without food. Do not chew, crush, or split the film-coated tablets. If a dose is missed, take this medicine as soon as possible if it is within 18 hours of the usual dosing time, and take the next dose at the usual time. If more than 18 hours have passed, skip the missed dose and resume the usual dosing schedule with the next dose. If vomiting occurs within one hour after taking this medicine, take 1 additional tablet.
There are no data on dose adjustment in patients under 18 years of age with end-stage renal disease. The safety and efficacy of this medicine have not been established in children under 12 years of age or weighing less than 35kg.
This medication may increase the risk of abnormal liver function reactions when administered to HIV patients simultaneously infected with hepatitis B or C viruses, so it is essential to confirm the presence of hepatitis B virus infection prior to administration. Abruptly discontinuing this medication may result in acute worsening of hepatitis, so liver status should be carefully monitored for several months after discontinuation. In patients beginning antiretroviral therapy with severely weakened immunity, an inflammatory response known as immune reconstitution syndrome may occur; if symptoms arise, medical personnel should be notified immediately. It is recommended not to coadminister this medication with certain anticonvulsants, antitubercular agents, certain antivirals, and some herbal products (e.g., St. John's Wort). The therapeutic effect may be reduced or side effects may increase when taken with these drugs. This medication should not be used if there is a known allergy to its active or other ingredients. Side effects such as diarrhea, nausea, headache, rash, and fatigue may occur during treatment, so if symptoms are severe or persistent, consultation with medical personnel is necessary. This medication should be used cautiously in patients with reduced kidney function, and administration will be determined based on kidney function test results. Women who are pregnant or breastfeeding must consult a physician before using this medication, and breastfeeding should be avoided while using this medication. This medication can cause weight gain and changes in blood lipid and glucose levels, necessitating regular health check-ups and blood tests. There is a potential risk of bone health issues such as decreased bone density or osteonecrosis, so if there are any pain or abnormalities in the bones, immediate consultation with medical personnel is necessary. In case of overdose, prompt hospital visit is required to monitor vital signs and receive necessary treatment, with some components being removable via hemodialysis. This medication should be stored out of the reach of children and kept sealed in the original container to protect it from moisture and air.
