Children's dosage
For children: Administer 10 mg (units) per kg of body weight by intravenous injection every 6 hours. For infants, administer 10-15 mg (units) per kg of body weight by intravenous injection every 12 hours. For infants and neonates, the initial dose is recommended at 15 mg per kg of body weight.

This medication is effective against various bacteria such as Streptococcus, Staphylococcus, Clostridium difficile, and diphtheroid bacteria. It is used for conditions such as endocarditis, osteomyelitis, arthritis, peritonitis, meningitis, superficial secondary infections from burns and surgeries. It also helps treat pneumonia, sepsis, lung abscesses, and empyema. It is effective for infections that are difficult to treat with penicillin and cephalosporin antibiotics. It is used for the treatment of small intestine and colon infections and antibiotic-associated pseudomembranous colitis caused by Clostridium difficile. This medication is useful for safely treating various severe infections.

Dosage and Administration

Adult dosage

For adults: Administer 500 mg (units) of vancomycin every 6 hours or 1 g (units) every 12 hours by intravenous drip. When taken orally, generally 500-2000 mg should be divided into 3-4 doses per day for 7-10 days, with a total daily dose not exceeding 2 g.

Children's dosage

Precautions

This medication should not be used in patients who have previously experienced severe allergic reactions or shock from this drug. Patients who have shown allergic reactions to this drug or peptide-type and aminoglycoside-type antibiotics should also not be administered this drug. Special caution should be taken when administering to patients with hearing loss or those who have previously used antibiotics that caused hearing loss. Patients with poor kidney function are at risk of drug accumulation and side effects, so blood concentrations should be regularly monitored and administered with caution. Patients with impaired liver function may also experience worsening liver function from this drug, so caution is necessary. Elderly patients, premature infants, and neonates have weakened kidney function, so they should be treated with greater care. This drug may affect the central nervous system, potentially causing dizziness, tinnitus, and hearing impairment, so regular hearing tests are advisable. Regular liver and kidney function tests should be performed to detect adverse reactions early and discontinue use or adjust the dose if necessary. If severe skin rashes, blisters, itching, or redness occur, medical personnel should be notified immediately, and treatment should be discontinued if severe. Rapid infusion may cause redness or pain on the face, neck, and body trunk, as well as muscle cramps, so slow infusion is safer. Long-term use of this drug can lead to resistance or excessive growth of other bacteria, so it should be used for the minimum necessary duration. If diarrhea, abdominal pain, or fever persists during treatment, serious infection such as pseudomembranous colitis may occur, and prompt medical attention is necessary. The risk of side effects may increase when used with other neurotoxic or nephrotoxic drugs, so careful monitoring is required. During pregnancy, safety for the fetus has not been fully established, and should only be administered with caution when absolutely necessary; during lactation, use should be avoided if possible or breastfeeding should be ceased. Overdosage can cause kidney damage or hearing loss, so it is crucial to adhere to the prescribed dosage, and immediate medical help should be sought in case of overdose. This drug must be administered through intravenous injection, and intramuscular injection or other inappropriate methods should be avoided to prevent tissue damage. The injection solution must be diluted and infused slowly, and if mixed with other medicines, chemical stability must be confirmed.

Shock, anaphylactic symptoms, urticaria, severe swelling, chest tightness, shortness of breath, cold sweat, pallor, severe allergic reactions, severe skin reactions (exfoliative dermatitis, bullous dermatitis, Stevens-Johnson syndrome, toxic epidermal necrolysis, drug hypersensitivity syndrome, acute generalized exanthematous pustulosis), hypotension, wheezing, respiratory distress, urticaria, itching, drug hypersensitivity syndrome (DRESS), acute generalized exanthematous pustulosis.
