Summary

Main use
Treatment of influenza A and B infections; prevention of influenza A and B infections
Who takes it
Adults · Children
Adult dose
Influenza A and influenza B virus infections: Initiate treatment on the first or second day after the onset of influenza symptoms. This medicine may be administered with or without food; in some patients, taking it with food may improve tolerability. Adults and adolescents aged 13 years and older: Administer 75 mg of oseltamivir orally twice daily for 5 days. Adults and adolescents aged 13 years and older who have difficulty swallowing capsules should use this medicine as powder for suspension. Patients with renal impairment: No dose adjustment is necessary in patients with creatinine clearance ≥ 60 mL/min. For patients with creatinine clearance of 30 ~ 60 mL/min, a reduced dose of 30 mg twice daily for 5 days is recommended. For patients with creatinine clearance of 10 ~ 30 mL/min, a reduced dose of 30 mg once daily for 5 days is recommended. Patients receiving regular hemodialysis should receive a 30mg dose of this medicine after each dialysis session to maintain plasma concentrations within the therapeutic range; treatment must not exceed 5 days. If influenza symptoms develop during the 48-hour interval between dialysis sessions, the dose may also be administered before dialysis begins. For peritoneal dialysis, a single 30mg dose of this medicine after dialysis is recommended. The pharmacokinetics of oseltamivir have not been studied in patients with end-stage renal disease who are not undergoing dialysis (e.g., creatinine clearance less than 10mL/min); therefore, no recommended dose is available for these patients. Patients with hepatic impairment: No dose adjustment is necessary for treatment in adults with mild to moderate hepatic impairment (Child-Pugh score ≤ 9). Prevention of influenza A and influenza B virus infections Adults and adolescents aged 13 years and older: Following close contact with an infected person, administer 75 mg of oseltamivir orally once daily for 10 days. Start administration within 2 days of contact with the infected person. The recommended dose for prevention during an influenza outbreak is 75 mg once daily. The safety and efficacy of this medicine have been demonstrated for up to 6 weeks. The preventive effect continues while this medicine is being taken. Patients with renal impairment: No dose adjustment is necessary in patients with creatinine clearance ≥ 60 mL/min. For patients with creatinine clearance of 30 ~ 60 mL/min, a reduced dose of 30 mg once daily is recommended. For patients with creatinine clearance of 10 ~ 30 mL/min, a reduced dose of 30 mg once daily every other day is recommended. Patients receiving regular hemodialysis should receive a 30mg dose of this medicine after every 2 dialysis sessions to maintain plasma concentrations within the therapeutic range. The first dose may also be administered before dialysis begins. For peritoneal dialysis, a 30mg dose of this medicine after dialysis once weekly is recommended. The pharmacokinetics of oseltamivir have not been studied in patients with end-stage renal disease who are not undergoing dialysis (e.g., creatinine clearance less than 10mL/min); therefore, no recommended dose is available for these patients. Patients with hepatic impairment: No dose adjustment is necessary for prevention in adults with mild to moderate hepatic impairment (Child-Pugh score ≤ 9).
Child dose

This medication is used for the treatment of influenza A and B virus infections in neonates aged 2 weeks and older through adults. It should be initiated within 48 hours of the onset of infection for it to be effective, and prompt treatment after the onset of symptoms is crucial. Additionally, it is used for the prevention of influenza in children aged 1 year and older and adults, but vaccination is prioritized for prevention, and this medication does not replace vaccination. It can only be used for prevention in cases where there is a risk of infection from circulating strains not covered by the vaccine or when vaccination is not feasible. Thus, this medication is effectively used for the treatment of influenza infections and limited prevention.

Precautions

This medication is used to treat or prevent influenza virus infections. However, it cannot replace influenza vaccination, so it is essential to receive the influenza vaccine every year. This medication has only been proven effective against influenza A and B viruses, and its safety and efficacy have not been confirmed for other types of infections. Particularly in children and adolescents, neurological and psychiatric adverse reactions such as seizures, delirium, and abnormal behavior have been reported after taking this medication, requiring caution. For patients in this age group, it is generally recommended to avoid using this medication except in special cases, and caregivers should ensure that the patient is not left alone for at least 2 days. Patients with allergies to this medication or hypersensitivity to its components should not use it. Patients with impaired renal function or those receiving dialysis may require dosage adjustments, and it is not recommended for patients with end-stage renal failure. Furthermore, it should be administered with caution to high-risk patients such as those with asthma, chronic bronchitis, or immunosuppressive conditions. The most common side effects during use include nausea, vomiting, and headache, and most of these symptoms are temporary and mild. Rarely, skin rashes, liver dysfunction, neurological symptoms, or acute renal failure may occur, and if any unusual symptoms arise, medical consultation should be sought immediately. This medication has a low risk of drug interactions, but caution may be needed when used in combination with certain other drugs, so it is important to inform your healthcare provider of any concurrent medications. Pregnant or nursing women should only use this medication when the benefits of treatment outweigh the risks, as data on its effects on fetuses or neonates are limited. For children under 1 year of age, safety and effectiveness have not been established, so use is not recommended. Elderly patients should be carefully monitored for any physiological decline, though dosage adjustments are generally not necessary. When using, adhere to the prescribed dosage and schedule, and if a dose is missed, take it as soon as possible, but skip it if the next dose is too close. This medication should be stored out of reach of children and transferring it to another container should be avoided as it may jeopardize quality. When using this medication, starting early in the onset of influenza symptoms is effective, and if symptoms are severe or worsening, immediate medical consultation should be sought.
