Blood sugar control for type 2 diabetes and prevention of cardiovascular events
This medication is used as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes. In particular, this drug helps in glycemic control with the combination of empagliflozin and metformin. Additionally, in patients with insufficient blood sugar control and cardiovascular disease, caution should be exercised regarding the impact of this medication on cardiovascular events. Therefore, this medication is prescribed for adult type 2 diabetes patients considering both blood sugar management and cardiovascular health.
The recommended dosage for this medication is 1 tablet, taken twice daily. The dosage should be determined based on each patient's treatment regimen, efficacy, and tolerance, without exceeding the maximum recommended daily dose of 25 mg of empagliflozin and 2000 mg of metformin. For patients using metformin alone or in combination with other medications, it is recommended to take 5 mg of empagliflozin twice daily (total 10 mg) alongside their current metformin dosage. If tolerance is good and additional glycemic control is needed, the empagliflozin dose can be increased to 25 mg (total dose per day). For patients transitioning to this medication from the combination of empagliflozin (10 mg or 25 mg total daily dose) and metformin, continue with their existing doses of empagliflozin and metformin. When combining this medication with sulfonylureas or insulin, consider reducing the dose of sulfonylureas or insulin to reduce the risk of hypoglycemia (see warnings on adverse reactions and interactions). This medication should be taken with meals to reduce gastrointestinal side effects associated with metformin. If a dose is missed, take it as soon as you remember. However, do not take a double dose at once; skip the missed dose in such cases. Special populations: Patients with renal impairment: Dose adjustments are not necessary for patients with mild renal impairment. Since this medication contains metformin, it can be used with dose adjustments in patients with an estimated glomerular filtration rate (eGFR) of 45-59 ml/min/1.73m2 or CrCl of 45-59 ml/min who do not have other symptoms that may increase the risk of lactic acidosis. In such patients, the starting dose of metformin is 500 mg or 850 mg once daily; therefore, starting doses with individual monotherapy of empagliflozin and metformin should be administered. The maximum daily dose of metformin hydrochloride that can be administered with this medication is 1000 mg, given as 500 mg twice daily. Renal function should be carefully monitored every 3-6 months (see warnings regarding caution in specific patients). If CrCl < 45 ml/min or eGFR < 45 ml/min/1.73m2 indicates decreased renal function, this medication should be discontinued immediately (see contraindications regarding specific patients). Elderly patients: Patients over the age of 75 should consider the risks of decreased renal function and fluid volume depletion.
This medication helps to control blood sugar. However, several important precautions must be followed while taking this medication. First, this medication can cause a rare but very serious side effect known as lactic acidosis. Lactic acidosis is a condition that occurs when there is an excessive buildup of lactic acid in the body, which can present symptoms such as difficulty breathing, muscle pain, fatigue, and abdominal pain. If these symptoms occur, you should immediately consult a physician. Second, when used with other diabetes medications or insulin, hypoglycemia may occur, so blood glucose levels should be monitored frequently, and medication dosages should be adjusted according to medical advice. Third, patients with impaired kidney function, acute kidney issues, severe dehydration, severe infection, or heart problems should not take this medication, and those about to undergo surgery should temporarily suspend its usage. Fourth, this medication may increase the risk of urinary tract infections and genital infections, so symptoms of infection should be reported to a healthcare professional immediately. Fifth, there may be a reduction in vitamin B12 levels, so regular blood tests are advised for long-term users. Sixth, it is safest for pregnant women, women who may become pregnant, and nursing mothers not to take this medication. Seventh, elderly patients or those at risk for fluid loss should be vigilant for dehydration symptoms and monitor fluid status and adjust medication use if necessary. Finally, it is advisable to avoid alcohol intake while taking this medication, and if any sudden unusual symptoms or changes in health occur, consult a physician immediately.
