Treatment of candidiasis and dermatomycosis, treatment of cryptococcosis and prevention of recurrence, prevention of fungal infections
This medication is used to treat acute or recurrent vaginal candidiasis. It is also effective against candidemia and systemic candidiasis, which includes invasive candidal infections in various areas such as the peritoneum, endocardium, lungs, and urinary tract. It helps treat cryptococcal meningitis and cryptococcosis in various parts of the body in patients with compromised immune function. It is also used as maintenance therapy for the prevention of recurrence of cryptococcosis in AIDS patients. Additionally, it is used to prevent fungal infections in patients with weakened immunity due to chemotherapy or bone marrow transplants. It is effective for treating dermatophyte infections such as onychomycosis, athlete's foot, tinea corporis, tinea cruris, and cutaneous candidiasis. Overall, it is used for the treatment and prevention of various fungal infections.
The route of administration is determined by the patient's clinical condition. Because oral formulations are rapidly and almost completely absorbed, the doses of oral formulations (tablets, capsules, dry syrup) and injectable formulations are the same. ○ Adults 1. Vaginal candidiasis: Administer a single oral dose of 150 mg fluconazole. 2. Systemic candidiasis, including candidemia, disseminated candidiasis, and other invasive Candida infections (peritoneal, endocardial, pulmonary, urinary tract, etc.): The usual dose of this drug is 400 mg once on the first day, followed by 200 mg once daily from the next day; the dose may be increased to a maximum of 400 mg once daily according to patient response. The duration of treatment is determined according to clinical response and medical judgment. 3. Cryptococcal meningitis and cryptococcosis at other sites: The usual dose of this drug is 400 mg once on the first day, followed by 200-400 mg once daily from the next day. The duration of treatment is determined by clinical and mycological response, but treatment should generally continue for at least 6 ~ 8 weeks. 4. To prevent recurrence of cryptococcal meningitis in patients with AIDS, administer 200 mg of this drug once daily indefinitely after completion of the patient's initial treatment. 5. Prevention of fungal infections in immunocompromised patients at risk of fungal infections due to neutropenia caused by cytotoxic chemotherapy, radiation therapy, or bone marrow transplantation: Administer 50-400 mg of this drug once daily according to the patient's risk of fungal infection. A dose of 400 mg once daily is recommended for patients at very high risk of systemic fungal infection, such as bone marrow transplant recipients with severe neutropenia or in whom prolonged neutropenia is expected. In these patients, administer 400 mg starting 5-6 days before the expected onset of neutropenia and continue 400 mg for 1 week after the neutrophil count increases to 1,000 cells/mm3 or more. 6. For onychomycosis, administer 150 mg once weekly until the fingernails or toenails have regrown (until new, uninfected nails have grown). New fingernails usually take 3-6 months to grow and toenails take 6-12 months; growth rates may vary by individual and age. In some cases, the nails may remain damaged after successful long-term treatment. For cutaneous fungal infections, including athlete's foot (tinea pedis), tinea corporis, jock itch (tinea cruris), and cutaneous candidiasis: The usual dose of this drug is 50 mg once daily or 150 mg once weekly for 2-4 weeks. However, athlete's foot may require treatment for 6 weeks, depending on the course of the infection. For tinea versicolor, administer 50 mg once daily for 2-4 weeks. ○ Elderly If renal function is normal, the usual adult dose is administered. In patients with renal impairment (creatinine clearance 50 mL/min or less), reduce the dose or extend the dosing interval. ○ Patients with renal impairment This drug is excreted primarily in the urine unchanged. No dose adjustment is necessary for a single dose; however, when multiple doses are required, administer the usual dose only on the first day and adjust the dose from the next day as follows: Creatinine clearance (mL/min) > 50: 100 % of the usual dose Creatinine clearance (mL/min) ≤50 (not on dialysis): 50% of the usual dose Patients on hemodialysis: 100% of the usual dose after each hemodialysis session Patients on hemodialysis should receive 100% of the usual dose after each hemodialysis session. On days when dialysis is not performed, administer a reduced dose according to creatinine clearance.
This medication should not be administered to certain patients. Firstly, patients who are allergic to this medication or similar drugs should not use it. Pregnant women or those who may become pregnant, as well as breastfeeding women, should not take this medication. This medication inhibits enzymes involved in drug metabolism in the liver, necessitating dosage adjustments and patient state monitoring when used with other drugs metabolized by these enzymes. This medication is generally safe, but in some patients, there may be changes in blood, liver, or kidney function, so regular testing is recommended. Rarely, side effects may include reduction in blood cell count, allergic reactions, liver function impairment, cardiac rhythm disturbances, gastrointestinal disturbances, skin rashes, etc. Particularly, severe skin reactions may occur, so if any abnormal symptoms arise, immediate consultation with medical personnel is necessary.
