Summary

Main use
Monotherapy for epilepsy (partial-onset seizures) and adjunctive therapy (such as for myoclonic seizures)
Who takes it
Adults · Children
Adult dose
Adults and adolescents weighing 50 kg or more (12–17 years) should start treatment at 500mg twice daily. Depending on clinical response and tolerability, the dose may be increased or decreased by 1000mg per day (500mg per dose, twice daily) every 2 to 4 weeks. The maximum recommended dose is 3000mg per day. For adjunctive therapy for primary generalized tonic-clonic seizures, treatment is started at 1,000mg per day (500mg per dose, twice daily), and the dose is increased by 1,000mg per day every 2 weeks, up to a maximum of 3,000mg. Dosage should be adjusted according to renal function in patients with renal impairment and hepatic impairment. Dose adjustment is also recommended for older adults (65 years and older), depending on reduced renal function.
Child dose
Children aged 4 to 11 years and adolescents aged 12 to 17 years weighing less than 50kg should start with an initial dose of 10mg/kg twice daily. Depending on clinical response, the dose may be increased up to 60mg/kg per day (30mg/kg per dose, twice daily). Dose adjustments should not exceed 20mg/kg per day (10mg/kg per dose, twice daily) every 2 weeks. The maximum recommended dose is 3000mg per day. For children weighing 20kg or less, it is recommended to start treatment with the 100mg/ml oral solution. Children weighing 20~40kg should start with 500mg per day (250mg per dose, twice daily), with increases of 500mg per day every 2 weeks, up to a maximum of 1500mg per day. Children weighing more than 40kg should start with 1000mg per day (500mg per dose, twice daily), with increases of 1000mg per day every 2 weeks, up to a maximum of 3000mg per day. For adjunctive treatment of myoclonic seizures in children (aged 12 years and older), start with 500mg twice daily and increase the dose by 1000mg per day every 2 weeks, up to a maximum of 3000mg. For adjunctive treatment of primary generalized tonic-clonic seizures in children (aged 12 years and older, weighing less than 50kg), start with 20mg/kg per day (10mg/kg per dose, twice daily), and increase the dose by 20mg/kg per day every 2 weeks, up to a maximum of 60mg/kg. Use in infants younger than 1 month is not recommended because of insufficient safety data for the oral formulation. Tablets are not suitable for children 4 years of age or younger; the oral solution should be used instead. The injectable formulation is not recommended for children younger than 16 years. For children and adolescents weighing less than 50kg with renal impairment, adjust the dose according to creatinine clearance: normal, 10-30mg/kg twice daily; mild, 10-20mg/kg twice daily; moderate, 5-15mg/kg twice daily; severe, 5-10mg/kg twice daily. For patients with end-stage renal disease undergoing dialysis, administer 10-20mg/kg once daily; a loading dose of 15mg/kg on the first day is recommended.

This medication is used alone for the treatment of secondary generalized seizures or partial seizures in patients aged 4 years and older who have just been diagnosed with epilepsy. It is also used as an adjunctive treatment for seizures that are not controlled by existing first-line epilepsy medications. In children aged 12 years and older, it is applicable for the treatment of specific seizures caused by myoclonic epilepsy and idiopathic generalized epilepsy. For patients who have difficulty taking medication orally, it can be administered as an injectable form. This medication is used to effectively improve various seizures in epilepsy patients.

Precautions

This medication is used for the treatment of epilepsy, and there is an increased risk of suicidal thoughts or behavior and mental health problems like depression during use. Therefore, if any mood changes or unusual behavior occur during treatment, it is essential to consult with a healthcare professional immediately. CNS side effects such as drowsiness, fatigue, coordination problems, and behavioral abnormalities may occur, particularly within the first 4 weeks of administration. In children, drowsiness, fatigue, and behavioral abnormalities have also been reported. Abrupt discontinuation of epilepsy treatment can worsen seizures, so the dosage should be gradually reduced under medical supervision when discontinuing this medication. Patients with allergies to this medication or its components should not take it. Patients with impaired kidney function may require dosage adjustments, and severe liver impairment patients are recommended to undergo kidney function testing before administration. Blood-related adverse reactions such as decreased blood cell counts may occur, so those with symptoms like weakness, fever, or frequent infections should undergo blood tests. Psychiatric symptoms, such as aggression, irritability, anxiety, depression, and increased sensitivity, may manifest, and in cases of severe behavioral changes, dosage adjustment or discontinuation should be considered. Seizures may worsen during initial treatment or dose increases, so if the frequency of seizures increases, the patient should consult a doctor immediately. Patients with heart disease or prolonged QT interval on EKG should use this medication with caution. The effects of this medication on the fetus during pregnancy are not fully understood, necessitating careful assessment, and for breastfeeding mothers, it is recommended to stop breastfeeding or discontinue the medication, as it can be excreted in breast milk and affect the baby. There is a risk of drowsiness or decreased concentration while driving or operating machinery, so caution is required. In cases of overdose, symptoms such as drowsiness, agitation, and consciousness impairment may occur; since there is no specific antidote, emergency care with blood dialysis may be beneficial. This medication generally has few interactions with other drugs, but if used in combination with methotrexate, careful monitoring of blood concentration changes is required. It can also be used in children and the elderly, but monitoring for dosage adjustment and adverse effects is crucial. This medication should be stored out of reach of children, and transferring it to another container should be avoided.
