Treatment of candidiasis, cutaneous fungal infections, and cryptococcosis; prevention of fungal infections and recurrence
This medication is used to treat various Candida infections, including acute or recurrent vulvovaginal candidiasis. It is also effective in treating systemic candidiasis, including candidemia and disseminated candidiasis. It is used to treat cryptococcal meningitis and other cryptococcal infections in immunocompromised patients, such as AIDS patients and organ transplant recipients. It is also used as maintenance therapy to prevent recurrence of cryptococcosis in AIDS patients. Additionally, it helps prevent fungal infections due to neutropenia caused by chemotherapy, radiation therapy, or bone marrow transplantation. It is effective in treating dermatophyte infections like onychomycosis, tinea pedis, and tinea corporis. This drug is used to treat and prevent various fungal infections.
The route of administration should be determined according to the patient's clinical condition. Since absorption of the oral formulation is rapid and almost complete, the doses of the oral formulations (tablets, capsules, dry syrup) and the injectable formulation are the same. ○ Adults 1. Vaginal candidiasis: Administer 150 mg of fluconazole as a single oral dose. 2. Systemic candidiasis, including candidemia, disseminated candidiasis, and other invasive Candida infections (peritoneal, endocardial, pulmonary, urinary tract, etc.): The usual dose of this medicine is 400 mg as a single dose on the first day, followed by 200 mg once daily from the next day. The dose may be increased to 400 mg once daily according to the patient's response. The duration of treatment should be determined according to clinical response and medical judgment. 3. Cryptococcal meningitis and cryptococcosis at other sites: The usual dose of this medicine is 400 mg as a single dose on the first day, followed by 200-400 mg once daily from the next day. The duration of treatment is determined by clinical and mycological response, but treatment should usually continue for at least 6 ~ 8 weeks. 4. To prevent recurrence of cryptococcal meningitis in patients with AIDS, administer 200 mg of this medicine once daily indefinitely after the patient has completed all initial treatment. 5. Prevention of fungal infections in immunocompromised patients at risk of fungal infection due to neutropenia caused by cytotoxic chemotherapy, radiation therapy, or bone marrow transplantation: Administer 50-400 mg of this medicine once daily, depending on the patient's risk of fungal infection. For patients at very high risk of systemic fungal infection, such as those undergoing bone marrow transplantation who have severe neutropenia or are expected to have prolonged neutropenia, 400 mg once daily is recommended. In these patients, administer 400 mg starting 5-6 days before the expected onset of neutropenia, and continue administering 400 mg for 1 week after the neutrophil count has increased to 1,000 cells/mm3 or higher. 6. For onychomycosis, administer 150 mg once weekly until the fingernails or toenails have regrown (until new, uninfected nails have grown). Fingernails usually take 3-6 months to regrow, and toenails take about 6-12 months; the growth rate may vary between individuals according to age. Even after successful treatment with long-term administration, the fingernails or toenails may sometimes remain damaged. Cutaneous fungal infections, including athlete's foot (tinea pedis), tinea corporis, jock itch (tinea cruris), and cutaneous candidiasis: The usual dose of this medicine is 50 mg once daily or 150 mg once weekly for 2-4 weeks. However, athlete's foot may require treatment for 6 weeks depending on the course of the condition. For tinea versicolor, administer 50 mg once daily for 2-4 weeks. ○ Elderly patients If renal function is normal, administer the usual adult dose. If renal impairment is present (creatinine clearance 50 mL/min or less), the dose should be reduced or the dosing interval extended. ○ Patients with renal impairment This medicine is excreted mainly in the urine unchanged. Dose adjustment is not necessary for a single dose. However, when multiple doses are required, administer the usual dose on the first day of treatment only, and adjust the dose from the next day as follows: Creatinine clearance (mL/min) > 50: 100 % of the usual dose Creatinine clearance (mL/min) ≤50 (not on dialysis): 50% of the usual dose Patients on hemodialysis: 100% of the usual dose after each hemodialysis session Patients on hemodialysis should receive 100% of the usual dose after each hemodialysis session. On days when dialysis is not performed, the dose should be reduced according to creatinine clearance.
This medication should not be administered to certain patients. For instance, patients who have had allergic reactions to this drug or other azole medications, pregnant women or those who may become pregnant, and women who are breastfeeding should not use it. Additionally, caution is required as serious side effects may occur when taken with certain medications. For example, concurrent use with terfenadine, cisapride, astemizole, or erythromycin can lead to dangerous heart issues such as arrhythmias or QT interval prolongation. This drug contains lactose and should not be taken by individuals with lactose intolerance or specific genetic metabolic disorders. Patients with impaired renal or liver function should use this medication carefully, as there is a risk of arrhythmia in those with heart disease or electrolyte imbalances. This drug inhibits CYP enzymes, which can increase the blood levels of other medications, so the types and dosages of concomitant medicines should be adjusted and the patient should be closely monitored. Regular tests, such as liver function, kidney function, and blood tests, are recommended during treatment, especially if liver enzyme levels rise, in which case a doctor should be consulted immediately. Use of this drug during pregnancy may harm the fetus, so women who are pregnant or planning to become pregnant must consult their physician and effective contraception is required during and for some time after treatment. Breastfeeding mothers should avoid this medication as it is excreted in breast milk. Safety in children or infants has not been fully established, so it should only be administered cautiously when necessary. Caution is needed during driving or operating machinery as this medication can cause dizziness or seizures as side effects. In the event of an overdose, hallucinations or unusual behaviors may occur, and immediate medical attention is required; supportive care and gastric lavage may be necessary. This drug is primarily excreted in urine, so in cases of overdose, hemodialysis may help to eliminate it.
