Treatment of epilepsy (partial-onset seizures) and neuropathic pain
This medication is used for the treatment of epilepsy, particularly as monotherapy for partial seizures in adolescents aged 13 years and older and adults. It can also be used as adjunctive therapy for partial seizures in children aged 3 years and older and adults. Additionally, it is effective in alleviating neuropathic pain. In other words, this drug helps improve symptoms associated with epilepsy and neuropathic pain.
Epilepsy Monotherapy and adjunctive therapy in patients aged 13 years and older: To achieve a maintenance dose of 900 mg/day of gabapentin, titrate gradually over the first 3 days of treatment as follows: Day 1: Administer one 300 mg capsule of this drug once daily or one 100 mg capsule of this drug 3 times daily (gabapentin 300 mg/day). Day 2: Administer one 300 mg capsule of this drug twice daily or two 100 mg capsules of this drug 3 times daily (gabapentin 600 mg/day). Starting on Day 3: Administer one 300 mg capsule of this drug 3 times daily or three 100 mg capsules of this drug 3 times daily (gabapentin 900 mg/day). Alternatively, as a starting dose, one 300 mg capsule of this drug may be administered 3 times daily (gabapentin 900 mg/day). The dose may be increased to 1,200 mg/day of gabapentin, and the total daily dose must not exceed 2,400 mg of gabapentin. (There are insufficient data on the safety and efficacy of doses exceeding 2,400 mg/day of gabapentin.) The duration of treatment should depend on clinical need. Long-term treatment is generally required for epilepsy. Although there is no evidence of a rebound phenomenon (increased frequency of epileptic seizures resulting from sudden discontinuation of treatment), sudden discontinuation of antiepileptic drugs in patients with epilepsy may cause status epilepticus. If treatment with this drug is to be discontinued, or if it is to be used concomitantly with another antiepileptic drug, the change should be made gradually over at least 1 week. Neuropathic pain Adults (18 years and older): To achieve a maintenance dose of 900 mg/day of gabapentin, titrate gradually over the first 3 days of treatment as follows: Day 1: Administer one 300 mg capsule of this drug once daily or one 100 mg capsule of this drug 3 times daily (gabapentin 300 mg/day). If necessary, the dose may be increased to 1,800 mg/day of gabapentin within 1 week, and the total daily dose must not exceed 3,600 mg of gabapentin. The total daily dose of this drug should be divided into 3 doses. The safety and efficacy of treatment for neuropathic pain for longer than 5 months have not been evaluated. Debilitated patients In debilitated patients with low body weight or patients after transplantation, the dose of this drug should be increased in 100 mg increments. Elderly patients Dosage adjustment may be necessary due to age-related decline in renal function (see table). Patients with renal impairment (creatinine clearance ≤ 80 mL/min) or patients undergoing hemodialysis Dosage adjustment is required based on the following table. Renal function, creatinine clearance (mL/min) ≥80: Total daily dosea 900 ~ 2,400 (mg/day) Renal function, creatinine clearance (mL/min) 50 - 79: Total daily dosea 600 ~ 1,800 (mg/day) Renal function, creatinine clearance (mL/min) 30 - 49: Total daily dosea 300 ~ 900 (mg/day) Renal function, creatinine clearance (mL/min) 15 - 29: Total daily dosea 150b~600 (mg/day) Renal function, creatinine clearance (mL/min) < 15: Total daily dosea 150b~300 (mg/day) a: The total daily dose should be divided into 3 doses. b: Administer 3 × 100 mg every 2 days. In patients with creatinine clearance below 15 mL/min, the dose should be reduced in proportion to creatinine clearance. Dosage in patients undergoing hemodialysis Initial dose: 300~400 mg of gabapentin is recommended. Maintenance dose: Administer 200~300 mg of gabapentin after 4 hours of hemodialysis; do not administer this drug on days when hemodialysis is not performed. Method and duration of administration Swallow this drug whole with plenty of fluid. It may be taken with or without food. When administered 3 times daily, ensure that the interval between doses does not exceed 12 hours to prevent recurrence of seizures. If a dose of this drug is missed (when 12 hours or more have elapsed since the most recent dose), the physician should determine whether an additional dose should be taken to make up for it. When used concomitantly with antacids containing magnesium or aluminum, administer this drug at least 2 hours after the antacid. This can significantly reduce the decrease in the bioavailability of this drug. The duration of treatment with this drug should depend on clinical need. If the dose of this drug is reduced, treatment is discontinued, or the drug is switched to another alternative, this should be done gradually over at least 1 week (a longer period may be required at the prescriber's discretion). To reduce the main adverse reactions, such as drowsiness, dizziness, fatigue, and ataxia, the first dose on Day 1 should be taken at bedtime.
Adjunctive therapy in children aged 3 to 12 years: Gradually titrate over the first 3 days of treatment to a maintenance dose of 30 mg/kg/day of gabapentin as follows. Day 1: administer 10 mg/kg of gabapentin; Day 2: administer 20 mg/kg of gabapentin; From Day 3 onward: administer 30 mg/kg of gabapentin. The dose may be increased, if needed, to a maximum of 40 mg/kg/day. The total daily dose should be divided into 3 doses.
This medication may increase the risk of suicidal thoughts or behavior, so if any abnormal changes in mood or behavior occur during treatment, consult a doctor immediately. Discontinuing this medication abruptly may lead to more frequent seizures or severe status epilepticus, so the dosage must be gradually reduced according to a doctor's instructions. Some cancer occurrences have been reported in animal tests, but the effects in humans are not yet clear. During treatment with this medication, sudden and unexpected deaths have been reported, but this falls within the typical risk range that can generally occur in epilepsy patients. If symptoms of pancreatitis such as persistent abdominal pain, nausea, or repeated vomiting occur, discontinue use immediately and inform your doctor. In children aged 3 to 12 years, behavioral changes, emotional instability, hyperactivity, and decreased attention may occur, so careful observation is necessary. Taking this medication along with narcotics or central nervous system depressants can cause severe drowsiness, respiratory depression, and coma; thus, it should only be used at the lowest effective dose for a short period under careful monitoring by a physician when necessary. Patients with allergies to this medication or its components, those with acute pancreatitis, those with generalized tonic-clonic seizures, and those with specific genetic lactose intolerance should not take this medication. Patients with impaired kidney function, elderly patients over 65 years, individuals with a history of mental illness, children with attention deficit disorders, pregnant women or those who may become pregnant, and breastfeeding women need to take extra precautions during treatment. Drowsiness, dizziness, and decreased motor skills may occur during treatment, so avoiding driving or operating dangerous machinery is advised when starting or changing the dosage. Side effects such as weight gain, vision disturbances, or skin rashes may occur, so regular monitoring of the patient's condition is necessary. Withdrawal symptoms may occur, so if discontinuing the medication, the dosage should be gradually reduced following a doctor's instructions. During pregnancy, there may be risks to the fetus; therefore, it is essential to consult a doctor if planning a pregnancy or if pregnant, and caution should be exercised during breastfeeding.
