Treatment of respiratory and ear infections, urinary tract and prostate infections, and skin and soft tissue infections
This medication is available in capsule form and dry syrup, effective against various bacteria including Staphylococcus, Streptococcus pneumoniae, and Influenza bacillus. It is primarily indicated for respiratory infections such as tonsillitis, pharyngitis, lobar pneumonia, and bronchitis. It is also effective against urinary tract infections such as cystitis, pyelonephritis, urethritis, and prostatitis. Additionally, it is used to treat skin and soft tissue infections like abscesses, soft tissue infections, boils, and impetigo. It can also be used to treat otitis media, making it widely applicable for various bacterial infections. This medication helps in treating multiple bacterial infections.
○ Adults 1) Skin and soft tissue infections, respiratory tract infections: Depending on the severity, administer cefradine 250mg~500mg (potency) 4 times daily or 500mg~1g (potency) twice daily by mouth. 2) Urinary tract infections: For uncomplicated urinary tract infections, administer 500mg (potency) twice daily; for severe urinary tract infections, administer 500mg (potency) 4 times daily or 1g (potency) twice daily. Severe infections may also require an extended treatment period and intensive therapy. ○ Patients with renal impairment 1) For patients not receiving hemodialysis, administer as follows. Creatinine clearance (mL/min) / Dose per administration (mg) / Dosing interval (hours) >20 / 500 / 6~12 15~20 / 250 / 12~24 10~14 / 250 / 24~40 5~9 / 250 / 40~50 <5 / 250 / 50~70 2) For patients receiving chronic, intermittent hemodialysis, administer an initial dose of 250mg (potency) when hemodialysis is started, followed by 250mg (potency) 6~12 hours later and 36~48 hours later. Then administer 250mg at the next hemodialysis session (only if at least 30 hours have elapsed since the most recent dose). (Injection) ○ Adults: Administer cefradine 2~4g (potency) daily in 4 divided doses by intravenous or intramuscular injection. For bone infections, administer 1g (potency) intravenously 4 times daily. For most infections, administer 500mg (potency) by intravenous or intramuscular injection 4 times daily. For severe infections (e.g., septicemia), the dose may be increased to 8g (potency) daily. ○ Patients with renal impairment 1) For patients not receiving hemodialysis, follow the dosage table for capsules and dry syrup. 2) For patients receiving chronic, intermittent hemodialysis, administer an initial dose of 250mg (potency) when hemodialysis is started, followed by 250mg (potency) 6~12 hours later and 36~48 hours later. Administer 250mg (potency) before the next hemodialysis session if at least 30 hours have elapsed since the previous dose. Adjust the dose as appropriate according to age and symptoms. As with other antibiotics, treatment should generally be continued for at least 48~72 hours after symptoms have resolved or evidence of bacterial eradication has appeared. For group A beta-hemolytic streptococcal infections, treatment for at least 10 days is recommended to prevent the risk of rheumatic fever or glomerulonephritis.
○ Children: Administer 25~50mg (potency) per Kg of body weight daily in 2~4 divided oral doses. Do not exceed 4g (potency) daily or the adult dose. For otitis media, administer 75~100mg (potency) per Kg of body weight daily in 2~4 divided doses. Do not exceed 4g (potency) daily or the adult dose. Administration to infants 9 months of age or younger is not recommended. ○ Children: 50~100mg (potency) per Kg of body weight daily may be administered in 4 divided doses by intravenous or intramuscular injection, but the adult dose must not be exceeded.
This medicine should not be used in patients who have experienced a severe allergic reaction or shock due to this medication in the past. Patients with a history of allergy to this medication or cephalosporin antibiotics should avoid its administration; if absolutely necessary, it must be given with extreme caution. Patients with a penicillin allergy should also take care when using this medication. If the patient or their family have a predisposition to allergic reactions like asthma, rashes, or hives, administration should be done cautiously. Patients with severely impaired kidney function require special caution when using this medication, and their condition should be monitored through regular exams. Patients who are unable to eat well or are receiving parenteral nutrition, the elderly, and those in poor general condition are at risk of bleeding due to vitamin K deficiency and should be closely monitored. This medication can cause severe allergic reactions leading to shock, thus the patient's condition should be observed closely during administration. If symptoms like discomfort, oral abnormal sensations, wheezing, dizziness, constipation, tinnitus, or sweating occur, administration should be stopped immediately, and appropriate treatment should be sought. If hypersensitivity reactions occur such as skin rashes, hives, itching, fever, lymphadenopathy, edema, or arthralgia, treatment should be stopped and necessary medical assistance should be provided. Gastrointestinal side effects may rarely include severe colitis; if abdominal pain or diarrhea worsens, administration should be stopped immediately. Nausea, vomiting, heartburn, diarrhea, abdominal pain, and loss of appetite may occur, thus symptoms should be observed carefully. Respiratory side effects may rarely include fever, asthma, cough, and difficulty breathing; if such symptoms occur, administration should be halted, and appropriate treatment must be provided. Hematological side effects including granulocytopenia, anemia, and thrombocytopenia may occur rarely, hence regular blood tests are recommended. Severe skin reactions like erythema multiforme, Stevens-Johnson syndrome, and toxic epidermal necrolysis have been rarely reported, so the skin should be monitored closely. Liver function abnormalities may rarely occur; therefore, liver function tests may be necessary. Kidney function may be impaired, requiring regular kidney function tests, and if abnormalities are found, administration should be discontinued. Oral candidiasis or inflammation may occur. There may be a tendency to bleed due to vitamin K deficiency, and symptoms of vitamin B complex deficiencies have also been rarely reported, requiring caution. Headaches, generalized malaise, upper abdominal discomfort, glossitis, and sensory abnormalities may rarely occur. To prevent the occurrence of resistant bacteria, susceptibility should be confirmed, and this medication should be used for the minimum necessary duration. To prevent severe allergic reactions, thorough medical history taking and skin reaction tests should be conducted prior to administration. In case of shock reactions, emergency treatment preparations should be in place, and the patient should be kept under observation in a stable condition following administration. Caution must be taken when using this medication along with diuretics or aminoglycoside antibiotics, as this may increase kidney dysfunction. Safety during pregnancy has not been established; hence it should be used cautiously only when necessary, and caution is also required for nursing mothers as the drug may pass into breast milk. It is advisable not to administer this medication to infants, particularly those under 9 months old. Elderly patients may exhibit higher rates of adverse reactions due to decreased physical functions, hence dosages and intervals should be adjusted, and their condition should be closely monitored. This medication can affect results of certain clinical tests, so caution is advised when interpreting test results. When administered via injection, there may be temporary pain at the injection site or thrombotic phlebitis, requiring caution during use. Also, if the injection is improperly administered subcutaneously, an aseptic abscess may form, thus deep injections should be performed for intramuscular injection.
