Treatment of herpes zoster; treatment of genital herpes and suppression of recurrence
This medication is used for the treatment of herpes zoster virus infections and genital herpes infections. It is also effective in suppressing the recurrence of recurrent genital herpes. It helps in alleviating the symptoms of these viral infections and preventing disease progression. Therefore, it is effectively used for herpes zoster and genital herpes.
1. Treatment of herpes zoster Adults: Administer famciclovir 250 mg orally 3 times daily for 7 days. It is preferable to start treatment as soon as symptoms appear after infection. 2. Treatment of an initial episode of genital herpes Adults: Administer 250 mg of this drug orally 3 times daily for 5 days. It is preferable to start treatment as soon as symptoms appear after infection. 3. Treatment of acute recurrent genital herpes Adults: Administer 125 mg of this drug orally twice daily for 5 days. It is preferable to start treatment during the prodromal period or as soon as symptoms appear after infection. 4. Suppression of recurrent genital herpes Adults: Administer 250 mg of this drug orally twice daily. Discontinue treatment periodically every 6~12 months to observe whether the natural course of the disease has changed. 5. Patients with renal impairment: Since the clearance of this drug is reduced in patients with renal impairment, dose adjustment should be undertaken with care. The dosage and administration for these patients are as follows. 1) Treatment of herpes zoster and an initial episode of genital herpes: Creatinine clearance (mL/min/1.73㎡) 30~59: 250 mg per dose twice daily 10~29: 250 mg per dose once daily 2) Treatment of acute recurrent genital herpes: Creatinine clearance (mL/min/1.73㎡) 30~59: 125 mg per dose twice daily 10~29: 125 mg per dose once daily 3) Suppression of recurrent genital herpes: Creatinine clearance (mL/min/1.73㎡) 30~59: 250 mg per dose twice daily 10~29: 125 mg per dose twice daily 4) If only serum creatinine is known, creatinine clearance can be calculated using the following equations. Men: {body weight (kg) × (140-age)}/ {72 × serum creatinine (mg/dL)} Women: 0.85 × the value calculated for men 5) Patients with renal impairment undergoing hemodialysis It is preferable to administer the drug every 48 hours, taking the dialysis interval into account. Since 4 hours of hemodialysis reduces the plasma concentration of this drug by approximately 75%, administer this drug immediately after dialysis. The recommended dose is 250mg for patients with herpes zoster and 125mg for patients with genital herpes. 6. Patients with hepatic impairment: No dose adjustment is necessary in patients with chronic compensated liver disease. There is insufficient treatment experience in patients with overt chronic decompensated liver disease; therefore, no clear recommended dose has been established.
This medication should not be used in certain patients. Patients who have had an allergy or hypersensitivity reaction to this medication or its components should not be administered this drug. Additionally, this medication contains lactose, so patients with lactose intolerance or genetic lactose metabolism disorders should also refrain from taking it. Patients with renal impairment need to be cautious when using this medication. If renal function is compromised, elimination of the drug may be difficult and increase the risk of side effects, therefore dosage adjustment may be necessary in consultation with healthcare professionals. Symptoms such as headache, nausea, vomiting, dizziness, and drowsiness may occur during treatment with this medication. In particular, older adults may more frequently experience central nervous system symptoms such as drowsiness or confusion, thus caution is advised. Rarely, thrombocytopenia, leukopenia, palpitations, liver function abnormalities, skin rashes, itching, and severe skin reactions have been reported. If these symptoms occur, immediate communication with healthcare professionals is necessary. This medication does not cure genital herpes, and while undergoing treatment, it is important to avoid sexual contact and maintain safe sexual practices if symptoms are present. Patients should be trained to begin treatment immediately if recurrence symptoms are observed. Although no specific effects have been reported for driving or operating machinery, caution is advised if symptoms such as dizziness or drowsiness occur. Patients with mild to moderate liver impairment usually do not require dosage adjustment, but in patients with severe liver impairment, caution is necessary since the drug's effectiveness may be reduced. This medication has a low potential for interactions with other drugs; however, when used in combination with drugs excreted by the kidneys, such as probenecid, concentrations of the drug may increase, and caution should be exercised. Additionally, when used in combination with lamotrigine, therapeutic efficacy may be reduced, thus consultation with healthcare professionals is necessary. During pregnancy, safety has not been established, so it should be administered only when necessary, and nursing mothers should take it only when the benefits outweigh the risks. The safety and efficacy of this medication in children under 18 years of age have not been confirmed; therefore, its use is not recommended. Older adults typically do not require dosage adjustments, but adjustment may be needed if renal function is impaired. In case of overdose, appropriate supportive care is necessary, and drug concentration can be reduced by hemodialysis. This medication should be stored in a tightly closed container to prevent contact with air or moisture, and it is advisable to keep it in a dry place at a temperature of 30 degrees Celsius or lower.
