Treatment of bacterial infections (skin and soft tissue infections, pneumonia, bacteremia, etc.)
This drug is a bactericidal glycopeptide antibiotic produced by the fermentation of Actinoplanes teichomyceticus, primarily effective against gram-positive bacteria. It particularly acts on methicillin-resistant Staphylococcus aureus, various streptococci, enterococci, and gram-positive anaerobic bacteria such as Clostridium. It is used to treat skin and soft tissue infections, bone and joint infections, hospital and community-acquired pneumonia, complicated urinary tract infections, infectious endocarditis, and peritonitis caused by continuous ambulatory peritoneal dialysis. Additionally, this drug does not show cross-resistance to various antibiotics, making it effective against a range of resistant bacteria. It also assists in the treatment of bacteremia associated with infections. Therefore, this drug is safely and effectively used for treating various severe infections.
Adults and elderly patients with normal renal function Skin and soft tissue infections, pneumonia, complicated urinary tract infections: Initial dose of 6 mg/kg intravenously or intramuscularly every 12 hours for 3 doses, followed by a maintenance dose of 6 mg/kg intravenously or intramuscularly once daily Bone and joint infections: Initial dose of 12 mg/kg intravenously every 12 hours for 3 ∼ 5 doses, followed by a maintenance dose of 12 mg/kg intravenously or intramuscularly once daily Infective endocarditis: Initial dose of 12 mg/kg intravenously every 12 hours for 3 ∼ 5 doses, followed by a maintenance dose of 12 mg/kg intravenously or intramuscularly once daily Calculate and administer the dose according to the patient's body weight (kg). For infective endocarditis, treatment for at least 21 days is appropriate. The duration of treatment should not exceed 4 months. Elderly patients: Dose adjustment is not necessary unless renal impairment is present. Adults and elderly patients with renal impairment: In patients with renal impairment, dose reduction is not necessary through Day 4; however, dosage adjustment is necessary to maintain a minimum serum trough concentration of 10 mg/L. After Day 4 of administration Mild to moderate renal impairment (creatinine clearance 30 ∼ 80 mL/min): Halve the dose by administering the initial unit dose every 2 days or administering half the initial dose daily. Severe renal impairment (patients with creatinine clearance below 30 mL/min) and patients on hemodialysis: Administer the initial unit dose every 3 days or administer one-third of the initial dose once daily. Patients on continuous ambulatory peritoneal dialysis: If the patient has a fever, administer a single intravenous loading dose of 400 mg (6 mg/kg), followed by 20 mg per L of dialysate in every dialysis bag during the first week, in alternate bags during the second week, and only in the overnight bag during the third week. The frequency of administration is generally once daily, but in severe infections, two doses should be administered on the first day of treatment to rapidly achieve the target blood concentration. Most infections caused by susceptible strains respond to treatment within 48 ∼ 72 hours. The total duration of administration is determined by the type and severity of infection and the patient's clinical response. For severe infections, the serum trough concentration should not be less than 10 mg/L. The peak concentration measured 1 hour after intravenous administration of 400 mg is usually in the range of 20 ∼ 50 mg/L, and concentrations of up to 250 mg/L have been reported after intravenous administration of 25 mg per kg of body weight.
Children (2 months ∼ 12 years of age): Administer an initial dose of 10 mg/kg intravenously every 12 hours for 3 doses, followed by a maintenance dose of 6 ∼ 10 mg/kg intravenously once daily. Neonates (up to 2 months after birth): Administer an initial dose of 16 mg/kg by intravenous infusion once on the first day, followed from the next day by a maintenance dose of 8 mg/kg once daily by intravenous infusion over 30 minutes.
This drug should be cautiously cut to avoid glass fragments when using a glass ampoule injection. Extra caution is especially required when used in children and the elderly. This drug can cause severe allergic reactions; if allergy symptoms appear, administration must be stopped immediately and emergency assistance sought. Patients allergic to similar class drugs should use it with caution. During infusion, infusion-related side effects such as shock, skin rash, and itching may occur, so slow administration over more than 30 minutes is recommended for safety. Rapid administration should be avoided. Rarely, serious skin reactions like Stevens-Johnson syndrome or toxic epidermal necrolysis may occur, so if abnormal skin symptoms appear, use must be stopped immediately. This drug can cause ototoxicity and nephrotoxicity; therefore, auditory, blood, liver, and kidney functions should be regularly checked in patients with weak kidney function or those using it for an extended period. Initial dosing should be carefully adjusted, and serum creatinine levels must be confirmed. Intraventricular administration is prohibited due to the risk of seizures. Individuals allergic to this drug or its components should not use it. Patients with allergies to similar class drugs, hearing impairment, weak kidney or liver function, or elderly patients should use it with caution. Generally, this drug is well tolerated, but various side effects such as nausea, vomiting, diarrhea, blood pressure changes, dizziness, and hearing loss may occur, and if abnormal symptoms arise, consultation with a physician is advised. When using this drug, it is important to use it only for an appropriate duration after susceptibility testing to prevent the occurrence of resistant bacteria. Caution should be exercised when using this drug in conjunction with other nephrotoxic or ototoxic medications, as the risk of side effects may increase. Women who are pregnant or breastfeeding should only use this drug after carefully weighing its benefits and risks. This drug may cause dizziness and headaches, so caution should be taken when driving or operating machinery. In cases of overdose, thrombocytopenia or anxiety symptoms may arise, and since it is not well removed by hemodialysis, symptomatic treatment should be administered. This drug is prohibited for intraventricular injection, and the dissolution solution should be used immediately or discarded within 24 hours when stored at 4 degrees Celsius. When dissolving the injectable solution, aseptic conditions should be maintained, and the solution should be dissolved exactly as specified.
