Treatment of essential hypertension and chronic stable angina; adjunctive treatment of congestive heart failure
This medication is used for cardiovascular conditions such as essential hypertension, chronic stable angina, and congestive heart failure. It helps alleviate symptoms and improve heart function when used as an adjunct treatment with diuretics, digitalis preparations, ACE inhibitors, or other vasodilators.
Do not increase the dose or stop taking this medicine without a doctor's instructions. 1. Essential hypertension 1) Adults (1) For the first 2 days, administer 12.5 mg of carvedilol orally once daily, then administer 25 mg orally once daily. (2) The usual dose is 25 mg orally once daily for 2 weeks. If a dose increase is needed, increase to 25 mg twice daily. The maximum dose per administration is 25 mg, and the maximum daily dose is 50 mg. 2) Elderly patients For some elderly patients, 12.5 mg once daily may provide sufficient effect. However, if the effect is insufficient after 2 weeks of treatment, the dose may be increased. 2. Chronic stable angina 1) For the first 2 days, administer 12.5 mg of this medicine twice daily, then administer 25 mg twice daily. 2) The usual dose is 25 mg twice daily for 2 weeks. If a dose increase is needed (excluding elderly patients), the dose may be increased to 50 mg twice daily. 3. Congestive heart failure Before starting treatment with this medicine, the standard treatment regimen already being used (conventional treatment for chronic heart failure with an ACE inhibitor, a diuretic, and usually a digitalis preparation) must be stable. Fluid retention should also be minimized before starting treatment with this medicine. 1) The usual initial dose is 3.125 mg of this medicine twice daily for 2 weeks. If the patient tolerates this treatment well, increase the dose to 6.25 mg twice daily for 2 weeks, then to 12.5 mg twice daily for 2 weeks, and then to 25 mg twice daily. 2) Increase the dose gradually only after taking each dose for at least 2 weeks, up to the dose the patient can tolerate. 3) The minimum effective dose is 6.25 mg twice daily. The maximum dose is 25 mg twice daily for patients weighing 85 kg or less, and 50 mg twice daily for patients weighing more than 85 kg. 4) To control the rate of absorption, it is recommended that the medicine be taken with meals in the morning and evening. Taking it with meals slows absorption and may reduce the frequency of orthostatic hypotension.
It is very important that this medication is not used in certain patients. For example, it should not be administered to patients who are allergic to this drug or its components, patients with rapidly declining heart function due to cardiogenic shock, patients with very slow heart rates or conduction problems in the heart. Furthermore, this drug should not be used in patients with pulmonary hypertension leading to right heart failure, patients with diabetic ketoacidosis, patients with severely weakened heart function, or patients with respiratory diseases such as asthma or chronic obstructive pulmonary disease, as it may worsen their symptoms. Additionally, pregnant women or those who may become pregnant, nursing mothers, patients with severely impaired liver function, and patients with peripheral blood circulation disorders should also avoid using this medication. Caution is advised for patients with diabetes, history of hypersensitivity reactions, mildly impaired heart function, reduced kidney function, and the elderly. In particular, patients with low blood pressure or slow heart rates, patients with specific heart conditions, and those taking other medications should carefully follow their doctor's instructions. During treatment with this drug, symptoms such as dizziness, headache, and fatigue may occur, and rare central nervous system symptoms such as hallucinations, depression, and psychosis may also develop. If heart rates slow down too much or heart failure symptoms worsen, regular cardiac function tests are necessary. Also, there is a risk of sudden drops in blood pressure leading to dizziness or fainting, so caution is required. Patients with reduced kidney function may experience further deterioration of kidney function, and patients with respiratory conditions may have worsening dyspnea. Gastrointestinal symptoms may include nausea, diarrhea, abdominal pain, vomiting, and constipation, and allergic reactions such as skin rashes or itching have been rarely reported. This drug can also affect liver function, so if any liver function abnormalities occur, immediate consultation with a physician is necessary. Additionally, blood sugar control may become difficult, so diabetic patients should regularly check their blood sugar levels. During prolonged use, regular checks of heart rate, blood pressure, liver, and kidney function should be conducted, and if symptoms worsen, the dosage may need to be adjusted or discontinued. Sudden discontinuation of this medication can worsen cardiac symptoms, so it should be gradually reduced under a physician's guidance. When taken with other medications, interactions may occur, so it is essential to inform your doctor if you are taking any other medications, especially blood pressure medications, diabetes medications, sedatives, antidepressants, and antiarrhythmic agents. Women who are pregnant or breastfeeding should not take this medication, and safety in children and adolescents has not been established. In the elderly, the drug concentration may increase due to decreased liver function, so treatment should start with a low dose and be administered cautiously. Overdose may result in low blood pressure, decreased heart rate, impaired cardiac function, shock, respiratory distress, and altered consciousness, so immediate medical treatment is necessary.
Not everyone experiences these, and they can vary from person to person.
