Infections of internal organs (lungs, urinary tract, etc.), tissue infections (skin, soft tissue, osteoarticular), and bacteremia
This drug is a bactericidal glycopeptide antibiotic produced by the microorganism Actinoplanes teichomyceticus, primarily effective against gram-positive bacteria. It acts on various gram-positive bacteria, including Staphylococcus aureus (including methicillin-resistant), Streptococcus, and Enterococcus, as well as anaerobic bacteria. This drug is used to treat various infections including skin and soft tissue infections, bone and joint infections, hospital and community-acquired pneumonia, complicated urinary tract infections, infective endocarditis, and peritonitis due to continuous ambulatory peritoneal dialysis. It is also effective in treating bacteremia associated with infections. This drug can be used safely for various infections as it has no cross-resistance with other antibiotics. In summary, this drug is used to treat major gram-positive bacterial infections.
Adults and elderly patients with normal renal function Skin and soft tissue infections, pneumonia, complicated urinary tract infections: 6 mg/kg intravenously or intramuscularly every 12 hours for 3 doses; maintenance dose 6 mg/kg intravenously or intramuscularly once daily Bone and joint infections: 12 mg/kg intravenously every 12 hours for 3 ∼ 5 doses; maintenance dose 12 mg/kg intravenously or intramuscularly once daily Infective endocarditis: 12 mg/kg intravenously every 12 hours for 3 ∼ 5 doses; maintenance dose 12 mg/kg intravenously or intramuscularly once daily Calculate and administer the dose according to the patient's body weight (kg). The duration of treatment should be determined based on the clinical response. For infective endocarditis, treatment for at least 21 days is appropriate. The duration of treatment must not exceed 4 months. Elderly patients: No dose adjustment is necessary unless renal impairment is present. Adults and elderly patients with renal impairment: In patients with impaired renal function, dose reduction is not necessary until Day 4; however, dose adjustment is required to maintain a minimum serum trough concentration of 10 mg/L. After Day 4 of treatment Mild and moderate renal impairment (creatinine clearance 30 ∼ 80 mL/min): Halve the dose by administering the initial unit dose every 2 days or by administering 1/2 of the initial dose daily. Severe renal impairment (patients with creatinine clearance below 30 mL/min) and patients on hemodialysis: Administer the initial unit dose every 3 days or administer 1/3 of the initial dose once daily. This medicine is not removed by dialysis. Patients receiving continuous ambulatory peritoneal dialysis: If the patient has a fever, administer a loading dose of 400 mg (6 mg/kg) by intravenous injection once, followed by 20 mg per liter of dialysis solution in every dialysis bag during the first week, in alternate bags during the second week, and in the overnight bag only during the third week. The usual frequency of administration is once daily; however, in severe infections, administer two doses on the first day of treatment to rapidly achieve the target blood concentration. Most infections caused by susceptible strains respond to treatment within 48 ∼ 72 hours. The total duration of treatment is determined by the type and severity of infection and the patient's clinical response. In severe infections, the minimum serum concentration must not be below 10 mg/L. The peak concentration measured 1 hour after intravenous administration of 400 mg is usually in the range of 20 ∼ 50 mg/L; concentrations of up to 250 mg/L have been reported after intravenous administration of 25 mg per kg of body weight.
Pediatrics (2 months to 12 years of age): As an initial dose, administer 10 mg/kg intravenously 3 times at 12-hour intervals, followed by a maintenance dose of 6 ∼ 10 mg/kg intravenously once daily. Neonates (up to 2 months of age): As an initial dose, administer 16 mg/kg by intravenous infusion once on the first day, followed from the next day by a maintenance dose of 8 mg/kg once daily by intravenous infusion over 30 minutes.
This drug may cause adverse reactions due to glass particles entering the glass ampoule injection, so care must be taken to cut it carefully to avoid mixing in glass particles during use. Extra caution should be exercised particularly in children and the elderly. This drug can induce severe allergic reactions, including anaphylactic shock, so administration should be stopped immediately and emergency measures should be taken if allergic symptoms appear. Patients allergic to vancomycin should use this drug with caution. Infusion-related side effects such as shock or red man syndrome may occur during infusion, so it should be administered slowly by intravenous drip for more than 30 minutes, and rapid infusion should be avoided. This drug can rarely cause life-threatening skin reactions such as Stevens-Johnson syndrome or toxic epidermal necrolysis, so administration should be stopped immediately if skin abnormalities appear. Patients with renal failure or those receiving long-term therapy, and those using drugs that cause ototoxicity or nephrotoxicity should have regular monitoring of hearing, blood, liver, and kidney functions. If the initial dose is high, there is an increased possibility of adverse reactions, so blood tests, including serum creatinine levels, should be performed. This drug may cause seizures when administered intraventricularly, so it must not be administered intraventricularly. It should not be administered to patients with hypersensitivity to this drug or its components. Caution is advised when administering to patients with hypersensitivity to glycopeptides, aminoglycosides, or vancomycin, as well as those with hearing loss, renal or liver function impairment, and the elderly. Generally, this drug is well tolerated, but adverse reactions such as nausea, vomiting, diarrhea, changes in blood pressure, blood cell reduction, dizziness, hearing loss, liver dysfunction, kidney dysfunction, and skin rashes may occur, so patients should consult a physician if any abnormal symptoms appear. This drug is only effective against susceptible bacteria, so unnecessary use should be avoided, and the treatment period should be minimized. This drug may increase side effects when used with other nephrotoxic or ototoxic drugs, so caution is needed. Pregnant or breastfeeding women should consider the potential risks and benefits when using this drug, and it is not known whether this drug is excreted in breast milk. This drug may cause dizziness and headaches, so caution is needed when driving or operating machinery. In cases of overdose, symptoms such as thrombocytopenia and anxiety may appear, and since it is not easily removed by hemodialysis, peritoneal dialysis or symptomatic treatment may be necessary. The solution should be used immediately, and any remaining solution should be discarded within 24 hours at 4 degrees Celsius. This drug should not be given intraventricularly and should only be dissolved using safe diluents.
Not everyone experiences these, and they can vary from person to person.
