Summary

Main use
Treatment of bacterial infections (bacteremia, pneumonia, skin infections, etc.)
Who receives it
Adults · Children
Adult administration
This drug should be administered with the dose adjusted according to the type and severity of infection, the patient's clinical response, sex, renal function, and other factors as follows. Adults and elderly patients with normal renal function Skin and soft tissue infections Pneumonia Complicated urinary tract infections Initial dose: 6 mg/kg intravenously or intramuscularly every 12 hours for 3 doses Maintenance dose: 6 mg/kg intravenously or intramuscularly once daily Bone and joint infections Initial dose: 12 mg/kg intravenously every 12 hours for 3 ∼ 5 doses Maintenance dose: 12 mg/kg intravenously or intramuscularly once daily Infective endocarditis Initial dose: 12 mg/kg intravenously every 12 hours for 3 ∼ 5 doses Maintenance dose: 12 mg/kg intravenously or intramuscularly once daily The dose should be calculated and administered according to the patient's body weight (kg). The duration of administration should be determined based on the clinical response. For infective endocarditis, administration for at least 21 days is appropriate. The treatment duration should not exceed 4 months. Elderly patients No dose adjustment is necessary unless renal impairment is present. Adults and elderly patients with renal impairment When administering to patients with renal impairment, dose reduction is not necessary through Day 4; however, dosage adjustment is necessary to maintain a minimum serum trough concentration of 10 mg/L. After Day 4 of administration Mild to moderate renal impairment (creatinine clearance 30 ∼ 80 mL/min): Halve the dose by administering the initial unit dose every 2 days or administering 1/2 of the initial dose daily. Severe renal impairment (patients with creatinine clearance less than 30 mL/min) and patients on hemodialysis: Administer the initial unit dose every 3 days or administer 1/3 of the initial dose once daily. This drug is not removed by dialysis. Patients undergoing continuous ambulatory peritoneal dialysis If the patient has a fever, administer a single intravenous loading dose of 400 mg (6 mg/kg), followed by 20 mg per L of dialysate: in every dialysis bag during the first week, in alternate bags during the second week, and only in the overnight bag during the third week. The frequency of administration is generally once daily, but in severe infections, administer twice on the first day of treatment to rapidly achieve the target blood concentration. In severe infections, the serum trough concentration should not be less than 10 mg/L. The peak concentration measured 1 hour after intravenous administration of 400 mg is generally in the range of 20 ∼ 50 mg/L; a peak concentration of 250 mg/L has been reported after intravenous administration of 25 mg per kg of body weight.
Child administration
Children (2 months ∼ 12 years of age): Administer an initial dose of 10 mg/kg intravenously every 12 hours for 3 doses, followed by a maintenance dose of 6 ∼ 10 mg/kg intravenously once daily. Neonates (up to 2 months of age): Administer an initial dose of 16 mg/kg by intravenous infusion once on the first day, followed from the next day by a maintenance dose of 8 mg/kg once daily by intravenous infusion over 30 minutes.

This drug is a glycopeptide antibiotic produced by Actinoplanes teichomyceticus, primarily effective against aerobic and anaerobic gram-positive bacteria. It is particularly effective against methicillin-resistant Staphylococcus aureus (MRSA), Enterococcus, various streptococci, and Clostridium among infecting bacteria. This drug is used to treat skin and soft tissue infections, bone and joint infections, pneumonia acquired in hospitals and communities, complicated urinary tract infections, infectious endocarditis, and peritonitis caused by continuous ambulatory peritoneal dialysis. It is also effective against bacteremia due to infections and has low cross-resistance to various antibiotics, making it reliable for treatment. Therefore, this drug is safely and effectively used for the treatment of various serious gram-positive bacterial infections.

As serious skin reactions may occur, if skin rashes or mucosal damage appear, use should be stopped immediately. This drug may cause ototoxicity, hematotoxicity, hepatotoxicity, nephrotoxicity, and other side effects, hence patients with poor kidney function or those on long-term treatment should have their hearing and blood, liver, and kidney functions regularly checked. This drug should not be administered intraventricularly as there is a risk of convulsions. Patients with hypersensitivity should not be administered this drug. Caution should be taken when administering to patients with impaired kidney or liver function, the elderly, or those who have had hypersensitivity to certain antibiotics. This drug can cause various side effects including nausea, vomiting, diarrhea, changes in blood pressure, decreased blood cells, hearing loss, liver dysfunction, kidney dysfunction, skin rashes, and allergic reactions, so immediate consultation with medical personnel is advisable upon occurrence of symptoms. It is recommended to use this drug only for the minimum necessary duration to prevent the development of resistant bacteria, and it's important to check the susceptibility of the infecting bacteria. Caution should be exercised when used in combination with other nephrotoxic or ototoxic drugs as side effects may increase. Pregnant or nursing women should administer this drug based on the physician’s judgment, and it is not known whether this drug is secreted in breast milk. This drug can cause dizziness or headaches, so caution should be exercised when driving or operating machinery.
