Treatment of respiratory and ear infections, urinary tract infections, and skin infections
This medication is used in the treatment of various infectious diseases such as tonsillitis, pharyngitis, pneumonia, bronchitis, cystitis, pyelonephritis, urethritis, prostatitis, abscesses, and soft tissue infections, in both capsule and dry syrup forms. It is also effective for skin and soft tissue infections such as otitis media and impetigo. There is an injectable form available, and its main effective bacterial strains include Staphylococcus, Streptococcus pneumoniae, Haemophilus influenzae, Escherichia coli, Proteus mirabilis, Klebsiella, Streptococcus, etc. This drug has a broad antibacterial activity against various strains, helping alleviate symptoms and treat related infectious diseases.
○ Adults 1) Skin and soft tissue infections, respiratory tract infections: Depending on severity, administer 250mg~500mg (potency) of cephradine per dose 4 times daily, or 500mg~1g (potency) twice daily by mouth. 2) Urinary tract infections: For uncomplicated urinary tract infections, administer 500mg (potency) per dose twice daily. For severe urinary tract infections, administer 500mg (potency) per dose 4 times daily or 1g (potency) per dose twice daily. Severe infections may also require an extended treatment period and intensive therapy. ○ Patients with renal impairment 1) For patients not receiving hemodialysis, administer as follows. Creatinine clearance (mL/min) Dose per administration (mg) Dosing interval (hours) >20 500 6~12 15~20 250 12~24 10~14 250 24~40 5~9 250 40~50 <5 250 50~70 2) For patients receiving chronic, intermittent hemodialysis, administer an initial dose of 250mg (potency) when hemodialysis is started, followed by 250mg (potency) 6~12 hours later and 36~48 hours later. Administer 250mg at the next hemodialysis session (only if at least 30 hours have elapsed since the most recent dose). (Injection) ○ Adults: Administer 2~4g (potency) of cephradine per day by intravenous or intramuscular injection in 4 divided doses. For bone infections, administer 1g (potency) per dose intravenously 4 times daily. For most infections, administer 500mg (potency) per dose by intravenous or intramuscular injection 4 times daily. For severe infections (such as sepsis), the dose may be increased to 8g (potency) per day. ○ Patients with renal impairment 1) For patients not receiving hemodialysis, follow the dosage table for capsules and dry syrup. 2) For patients receiving chronic, intermittent hemodialysis, administer an initial dose of 250mg (potency) when hemodialysis is started, followed by 250mg (potency) 6~12 hours later and 36~48 hours later. Administer 250mg (potency) before the next hemodialysis session if at least 30 hours have elapsed since the previous dose. Adjust the dose appropriately according to age and symptoms. As with other antibiotics, treatment should generally continue until at least 48~72 hours after symptoms have resolved or evidence of bacterial eradication has appeared. For group A beta-hemolytic streptococcal infections, treatment for at least 10 days is recommended to prevent the risk of rheumatic fever or glomerulonephritis. <Preparation of this drug> For intravenous injection, dissolve 500mg (potency) of this drug per 5mL of water for injection, 5% glucose injection, or saline injection, and inject intravenously over 3~5 minutes. For intravenous infusion, dissolve in an infusion solution such as 5% or 10% glucose injection or saline injection, and administer by intravenous infusion. For intramuscular injection, dissolve 500mg (potency) in 2.0mL of water for injection or saline injection, and inject intramuscularly.
○ Children: Administer orally in 2~4 divided doses at a daily dose of 25~50mg (potency) per Kg of body weight. However, do not administer more than 4g (potency) per day or exceed the adult dose. ○ Children, otitis media: Administer in 2~4 divided doses at a daily dose of 75~100mg (potency) per Kg of body weight. However, do not administer more than 4g (potency) per day or exceed the adult dose. It is preferable not to administer to infants 9 months of age or younger. (Injection) ○ Children: 50~100mg (potency) per Kg of body weight per day may be administered by intravenous or intramuscular injection in 4 divided doses, but the adult dose must not be exceeded.
This drug should not be used by individuals who have had serious allergic reactions, particularly shock, after using this medication in the past. It contains lactose, so it should not be taken by patients with galactose intolerance, Lapp lactase deficiency, or glucose-galactose malabsorption. Individuals with a history of allergies or hypersensitivity reactions to this drug or cephalosporin antibiotics should generally avoid it but may be prescribed with caution in necessary cases. Those allergic to penicillin antibiotics should also exercise caution. Caution is advised for individuals with a familial history of bronchial asthma, rashes, or urticaria, as allergic symptoms may occur more frequently. Patients with severely impaired kidney function, those unable to take orally, and elderly individuals with poor nutritional status may be at risk of vitamin K deficiency and should be closely monitored. This medication contains Yellow No. 5 dye, and those with a history of allergy or hypersensitivity to this ingredient should use it cautiously. During treatment, symptoms such as shock, discomfort, oral anomalies, wheezing, dizziness, tinnitus, sweating, and others may occur. If such symptoms appear, treatment should be immediately discontinued, and medical advice should be sought. If skin rashes, urticaria, erythema, itching, fever, lymphadenopathy, or arthralgia occur, medical treatment should be sought. Gastrointestinal disorders such as abdominal pain, diarrhea, nausea, vomiting, or heartburn may occur infrequently. If severe colitis symptoms appear, treatment should be promptly discontinued. Respiratory symptoms like fever, asthma, cough, and shortness of breath may be rare and should be reported to medical personnel if present. Blood tests may reveal rare occurrences of granulocytopenia, anemia, or thrombocytopenia, necessitating periodic laboratory checks. Serious skin reactions, including severe rashes or Stevens-Johnson syndrome, may occur infrequently, so skin condition should be closely monitored. Liver function abnormalities can occur infrequently, requiring liver function tests to be performed. Kidney function may deteriorate rapidly, and kidney function should be regularly monitored. If any anomalies are observed, treatment should cease immediately. Dysbiosis, such as oral candidiasis or stomatitis, can occur infrequently, warranting attention to oral health. Vitamin K deficiency can lead to bleeding tendencies or vitamin B complex deficiency symptoms, which may occur infrequently; consult healthcare personnel if such symptoms arise. Symptoms such as headaches, malaise, upper abdominal discomfort, and glossitis may also occur, so health status should be closely monitored. When using this drug, it is advisable to check the susceptibility of the infectious organisms to prevent the development of resistant microorganisms and to use it for the minimum duration necessary. To prevent severe reactions like shock, thorough medical history taking and skin response tests before administration are recommended. During treatment, emergency preparations should be made, and patients should be closely observed for stability. When used in conjunction with diuretics or aminoglycoside antibiotics, there's an increased risk of kidney function deterioration. Safety during pregnancy has not been established; therefore, treatment should only be cautiously given when the benefits outweigh the risks for pregnant women or those who may become pregnant. This drug can be excreted in breast milk, so nursing mothers should use it cautiously. Safety for premature infants and newborns has not been established, thus it is better to avoid use in infants under 9 months old. Elderly individuals may experience increased adverse reactions due to decreased physiological function and a potential risk of bleeding from vitamin K deficiency; thus, dose and administration intervals should be carefully adjusted, and health status closely monitored. In some clinical tests, false-positive results may arise from this drug; caution in interpreting test results is needed. When used in injection form, injection site pain or thrombophlebitis may occur, and sterile abscesses may form during subcutaneous administration, hence correct administration techniques must be followed.
