Summary

Main use
Treatment of respiratory and ear infections, urinary tract infections, and skin infections
Who uses it
Adults · Children
Adult dosage
Adults Skin and soft tissue infections, respiratory tract infections: Depending on the severity, administer 250mg~500mg (potency) of cephradine orally 4 times daily, or 500mg~1g (potency) 2 times daily. Urinary tract infections: For uncomplicated urinary tract infections, administer 500mg (potency) per dose 2 times daily; for severe urinary tract infections, administer 500mg (potency) per dose 4 times daily or 1g (potency) per dose 2 times daily. Severe infections may also require a longer treatment period and intensive therapy. Patients with renal impairment For patients not receiving hemodialysis Creatinine clearance (mL/min) >20: dose per administration 500mg, dosing interval 6~12 hours Creatinine clearance (mL/min) 15~20: dose per administration 250mg, dosing interval 12~24 hours Creatinine clearance (mL/min) 10~14: dose per administration 250mg, dosing interval 24~40 hours Creatinine clearance (mL/min) 5~9: dose per administration 250mg, dosing interval 40~50 hours Creatinine clearance (mL/min) <5: dose per administration 250mg, dosing interval 50~70 hours For patients receiving chronic, intermittent hemodialysis, administer an initial dose of 250mg (potency) when hemodialysis is started, followed by 250mg (potency) at 6~12 hours and again at 36~48 hours. Then administer 250mg at the next hemodialysis session (only if at least 30 hours have passed since the most recent dose). Adults, injectable formulation: Administer 2~4g (potency) of cephradine daily in 4 divided doses by intravenous or intramuscular injection. For bone infections, administer 1g (potency) per dose 4 times daily by intravenous injection. For most infections, administer 500mg (potency) per dose 4 times daily by intravenous or intramuscular injection. For severe infections (e.g., sepsis), the dose may be increased to 8g (potency) daily. Patients with renal impairment, injectable formulation: For patients not receiving hemodialysis, follow the table for capsules and dry syrup. Patients with renal impairment, injectable formulation: For patients receiving chronic, intermittent hemodialysis, administer an initial dose of 250mg (potency) when hemodialysis is started, followed by 250mg (potency) at 6~12 hours and again at 36~48 hours. Before the start of the next hemodialysis session, administer 250mg (potency) if at least 30 hours have passed since the previous dose. Adjust the dose as appropriate according to age and symptoms. As with other antibiotics, treatment should generally continue for at least 48~72 hours after symptoms have resolved or evidence indicates that the bacteria have been eliminated. For group A beta-hemolytic streptococcal infections, treatment for at least 10 days is advisable to prevent the risk of rheumatic fever or glomerulonephritis. For intravenous injection, dissolve each 500mg (potency) of this medicine in 5mL of water for injection, 5% dextrose injection, or normal saline injection, and administer intravenously over 3~5 minutes. For intravenous infusion, dissolve in an infusion solution such as 5% or 10% dextrose injection or normal saline injection, and administer by intravenous infusion. For intramuscular injection, dissolve each 500mg (potency) in 2.0mL of water for injection or normal saline injection, and inject intramuscularly.
Children's dosage
Children: Administer 25~50mg (potency) per kg of body weight daily in 2~4 divided doses orally. Do not exceed 4g (potency) daily or the adult dose. For otitis media: Administer 75~100mg (potency) per kg of body weight daily in 2~4 divided doses. Do not exceed 4g (potency) daily or the adult dose. It is preferable not to administer to infants 9 months of age or younger. Children, injectable formulation: 50~100mg (potency) per kg of body weight daily may be administered in 4 divided doses by intravenous or intramuscular injection, but the adult dose must not be exceeded.

Precautions

This medicine should not be used in patients who have experienced severe allergic reactions or shock from it in the past. Patients who have shown allergic reactions to this medicine or antibiotics of the same class should be given it with extreme caution. Patients allergic to penicillin antibiotics should also be careful when using this medicine. Those with a tendency to manifest allergic symptoms such as bronchial asthma, skin rashes, and urticaria should be cautious as well. Patients with severely impaired kidney function need special attention when using this medicine. Patients who have difficulty swallowing pills, those who receive nutrition other than orally, elderly patients, and those in poor overall condition should also be observed for signs of vitamin K deficiency. This medicine can rarely cause severe shock, so patients' conditions should be closely monitored during administration. If allergic reactions such as rashes, urticaria, itching, or fever occur, the medication should be discontinued immediately, and treatment should be sought. Gastrointestinal side effects such as abdominal pain, diarrhea, or bloody stools may occur; if symptoms are severe, the medication should be stopped, and medical advice should be sought. Respiratory issues such as fever, asthma, cough, and breathing difficulties can rarely occur; if these symptoms arise, the medication should be stopped, and treatment should be sought. Blood-related anomalies such as transient leukopenia or anemia may occur, necessitating regular blood tests. Severe rashes or rare skin reactions like Stevens-Johnson syndrome have been reported, so skin condition must be monitored carefully. Liver function abnormalities may rarely occur, which may require liver function tests. Kidney function may deteriorate suddenly, so regular kidney function tests should be conducted, and if any abnormalities are found, the drug must be discontinued. Oral inflammation or Candida infections may occur, thus oral conditions should be checked. Signs of bleeding tendency due to vitamin K deficiency or vitamin B complex deficiency symptoms may rarely develop; hence, attention should be paid to bleeding symptoms. Other side effects such as headaches, fatigue, and discomfort in the upper abdomen may also occur. While using this medication, it is advisable to perform sensitivity testing and use it for the minimum necessary period to prevent the emergence of resistant bacteria. To prevent serious reactions such as shock, it is essential to investigate the patient’s medical history in advance and conduct skin reaction tests if appropriate. Be prepared for emergencies and ensure patients are sufficiently observed in a stable condition post-administration. When used in conjunction with diuretics or aminoglycoside antibiotics, renal burden may increase, so caution is required. Due to insufficient safety established during pregnancy, it should only be used when the therapeutic benefits outweigh the risks; nursing women should also use cautiously. It is preferable not to administer to infants, especially those under 9 months of age. Elderly patients may exhibit side effects more readily due to decreased body function, so dosage and administration intervals should be adjusted, with close observation of their condition. It may affect the results of certain clinical tests, so caution is needed when interpreting test results. When administered by injection, pain or thrombophlebitis at the injection site may occur, requiring caution; if used as a suspension, it should be refrigerated and used within 2 weeks.
