This medication is used in various forms and is effective for anesthesia and pain relief in multiple areas. The 2% solution is used for local anesthesia during the irritation or inflammation of the oral and pharyngeal mucosa, as well as for suppressing gag reflex during dental X-rays or impressions. The 2% gel provides surface anesthesia and lubrication during cystoscopy, gastroscopy, bronchoscopy, rectoscopy, and intubation, and is also effective for pain relief in cystitis and urethritis. The injectable form is used for reducing pain during intramuscular injections, as well as for epidural anesthesia, nerve block, infiltration anesthesia, and surface anesthesia; it is also utilized in the treatment of ventricular arrhythmias. Overall, this medication is widely used for anesthesia and pain relief.
Dosage
(Solution: 2% viscosity) The maximum single dose should not exceed 4.5 mg of lidocaine hydrochloride per kilogram of body weight, and the total administered amount should not exceed 300 mg (15 mL of this medication). For symptomatic treatment of irritation or inflammation of the oral and pharyngeal mucosa, administer 300 mg (15 mL) of lidocaine hydrochloride. When used orally, the medication should be held in the mouth and then spat out, while for pharyngeal use, it should be held and then swallowed. Re-administration is prohibited within 3 hours, and should not exceed 8 times within 24 hours.
Precautions
This medication is a glass ampoule injection, and care must be taken when breaking the container to avoid the inclusion of glass fragments. Caution is especially necessary when using in children or the elderly. This medication should not be used in certain patients. For example, it should not be administered to patients with severe bleeding or in shock, those with inflammation at the injection site or with sepsis, or to patients who are allergic to this medication or similar local anesthetics. Care should be taken when administering to patients with central nervous system disorders, pregnant women, elderly individuals, patients with severe hypertension, and those with severe spinal deformities. This medication can rarely cause serious side effects such as shock, hypotension, and respiratory depression, so patient condition should be carefully monitored. Additionally, malignant hyperthermia symptoms such as rapid temperature elevation, muscle rigidity, and arrhythmias may occur rarely; if such symptoms appear, administration must be halted immediately, and appropriate treatment should be initiated. Since it can affect the central nervous system, symptoms like tremors, convulsions, drowsiness, dizziness, and nausea may occur; suitable measures should be taken immediately if these symptoms are observed. When using this medication, allergic reactions such as urticaria or edema may occur, so caution is needed; in severe cases, anaphylaxis may occur. Before use, it is important to check the patient's general condition and to use the lowest concentration and minimal dose possible for safety. Particularly when injecting in highly vascular areas, only a small amount should be used, and care must be taken to avoid the needle entering a blood vessel. When used in conjunction with vasoconstrictors, the risk of side effects may be higher in certain patients, so the decision to use should be made with caution. This medication should only be administered to pregnant women or those potentially pregnant when the benefits outweigh the risks. Elderly patients may have slower metabolism of this medication, leading to a higher risk of side effects; therefore, dosage adjustments and condition monitoring are required. This medication is not for ophthalmic use and caution should be exercised as prolonged use at high concentrations may present a carcinogenic risk. Moreover, it should be administered with care to patients with severely impaired liver or kidney function or those with cardiac conduction disturbances, avoiding overdose and regularly checking blood pressure and electrocardiogram.

Urticaria, severe swelling (edema), pallor, respiratory depression, shock, malignant hyperthermia (unexplained tachycardia, arrhythmias, blood pressure fluctuations, rapid temperature rise, muscle rigidity, crimson staining of blood, cyanosis, hyperventilation, sweating, acidosis, hyperkalemia, myoglobinuria), central nervous system toxicity symptoms (tremors, convulsions), anaphylaxis, hypotension, cardiac arrest
