When people think of double eyelid surgery, most imagine a 'cosmetic surgery' that makes the eyes look bigger and more defined. Of course, in reality, many people choose double eyelid surgery for cosmetic purposes.

 

However, when seeing patients in the clinic, there are quite a few cases where surgery is necessary not simply to change the shape of the eyes, but to alleviate discomfort caused by structural problems with the eyelids.

 

In particular, if you experience problems such as eyelids obstructing your vision, straining your forehead to open your eyes, or eyelashes poking your eyes, you should approach this from a functional perspective rather than a mere cosmetic one.

 

Today, from a plastic surgeon's perspective, I will explain 'cases where double eyelid surgery can be considered even if it is not for cosmetic purposes.'

1️⃣ Is double eyelid surgery simply a 'surgery to create a line'?

A double eyelid is formed by a 'connection' between the eyelid skin and the structure that opens the eye.

Therefore, it is best to understand that double eyelid surgery is not simply about creating a line on the skin, but rather a procedure that adjusts the relationship between the eyelid skin and internal tissues to allow the skin to fold naturally when the eyes are open.

 

If this is accompanied by eyelid drooping, sagging skin, or decreased function of the eye-opening muscles, it is difficult to resolve the problem by simply forming a double eyelid line.

 

In this case, depending on the patient's condition, a surgical plan such as adjusting the skin or correcting ptosis may be necessary.

2️⃣ Is sagging eyelid skin blocking your vision?

If skin sagging becomes severe, it can go beyond merely making the eyes look smaller and obstruct your vision.

 

As we age, the elasticity of the eyelid skin decreases, causing the upper skin to sag downward. This condition is commonly referred to as dermatochalasis.

 

If skin sagging becomes severe, it can go beyond simply making the eyes appear smaller and obstruct outer or upward vision. Patients typically describe it as follows:

🔹 “I keep tensing my forehead when I try to open my eyes.”
🔹 “My eyes get more tired in the evening.”
🔹 “I feel like I can't see well because the upper part feels stuffy.”

 

In cases where sagging skin affects vision, upper eyelid surgery to remove or reposition the necessary amount of skin may be performed for the purpose of functional improvement.

 

In fact, an evidence review by the American Academy of Ophthalmology reports that properly correcting ptosis and upper eyelid laxity can improve visual field, peripheral vision, and functions related to daily life.

3️⃣ Do you constantly tense your forehead when you open your eyes?

If it is difficult to open your eyes using only your eyelids, your body uses your forehead muscles instead.

 

There are people whose eyelids do not lift sufficiently even when they keep their eyes open comfortably. In this case, it is necessary to check for what is commonly referred to as blepharoptosis.

 

Ptosis is a condition in which the upper eyelid is positioned lower than normal, covering part of the pupil. It can occur congenitally or acquired as the structure responsible for lifting the eyelid weakens with age.

 

If you cannot fully open your eyes using only your eyelids, your body uses the frontal muscles to compensate. As a result, you may find yourself constantly raising your eyebrows, developing deep wrinkles on your forehead, or feeling fatigued just by opening your eyes.

 

In such cases, it is necessary to evaluate the eye-opening function itself rather than simply creating double eyelids. If necessary, ptosis correction can be considered along with double eyelid surgery.

4️⃣ Have you developed a habit of tilting your head back to look?

The posture of lifting the chin is one of the indicators used to determine the need for functional surgery.

 

Some patients with ptosis or severe eyelid sagging unconsciously lift their chin to look at objects. This is a compensatory behavior to clear their field of vision by tilting their head back, as their eyelids obstruct their upper vision.

 

A report by the American Academy of Ophthalmology also suggests the occurrence of a chin-up position due to eyelids obstructing vision as one of the indicators for determining the need for functional surgery.

5️⃣ Do your eyes feel uncomfortable due to sagging skin or eyelashes?

If eyelashes irritate the eyes, discomfort such as a foreign body sensation, redness, and tearing may recur.

 

If the eyelid skin sags severely, the weight of the skin can affect the direction of the eyelashes.

 

If eyelashes droop downward or repeatedly irritate the eye, discomfort such as a foreign body sensation, redness, tearing, and corneal irritation may occur.

 

Even in this case, the primary goal of the surgery is not simply to create beautiful double eyelids, but to correct the eyelid structure toward a normal direction to reduce discomfort.

 

Therefore, before surgery, it is necessary to check not only skin sagging but also the direction of the eyelashes, the position of the eyelids, and the function of closing the eyes.

 

6️⃣ Has only one eyelid drooped, causing a significant difference between the left and right sides?

For sudden unilateral sagging, identifying the cause comes before surgery.

 

Human faces are not naturally perfectly symmetrical. However, if one eyelid has noticeably lowered or if the difference in size between the left and right eyes has widened recently, it should not be judged merely as cosmetic asymmetry.

 

Ptosis can occur in relation to aging, trauma, previous ophthalmic surgery, or changes in eyelid structure, and rarely, it may be caused by nerve or muscle-related diseases.

 

Especially if one eyelid suddenly droops, it is important to identify the cause first rather than planning cosmetic surgery immediately.

 

📌 Since the surgical method for ptosis varies depending on the cause, an accurate diagnosis must be prioritized before surgery.

 

7️⃣ In that case, do you need to have double eyelid surgery in all such instances?

That is not the case. 'Why it sagged' comes before 'what kind of surgery to have.'

 

That is not the case. This is the most important point. Just because your eyelids are heavy or your eyes look small does not mean that everyone needs the same surgery.

 

First, you must determine whether the cause is skin sagging, ptosis, drooping eyebrows themselves, or a problem with eyelid fat or other tissues.

 

For example, if the skin is significantly sagging but the function of the eye-opening muscles is normal, properly adjusting the loose skin may be sufficient. Conversely, if there is ptosis and only the skin is removed, the eye-opening function itself may not improve.

While addressing functional issues, you must also consider left-right balance, the height of the double eyelid, and the natural shape of the eyelids.

 

Conversely, even when planning cosmetic double eyelid surgery, the strength of the eye-opening muscles and eyelid function should not be overlooked. If you focus solely on aesthetic results and excessively remove tissue or neglect functional issues of the eyelids, unexpected problems such as dryness or difficulty closing the eyes may occur.

 

Literature on upper eyelid surgery published in 2023 also emphasizes that it is important to evaluate the functional condition of the eye before surgery, as eyelid surgery can affect not only the field of vision but also the cornea, tear film, and the surface of the eyeball.

 

9️⃣ What does a plastic surgeon check before surgery?

Before designing the lines, we first examine the functions of the eyelids one by one.

 

When treating such patients, I do not simply design a double eyelid line. It is important to first examine how much the eyelid skin is sagging, check the position of the upper eyelid when the eyes are opened comfortably, see if the eyes can be opened sufficiently without using the eyebrows, check for any difference in the strength of opening the left and right eyes, see if the eyelashes are irritating the eyes, and check for any pre-existing discomfort such as dry eye syndrome.

 

If necessary, a visual field test or ophthalmological evaluation may be helpful.

 

Only by going through this process can you determine whether simple double eyelid surgery is necessary, whether skin sagging needs correction, or whether ptosis surgery must be performed in conjunction.

Q1. Does everyone with drooping eyelids need double eyelid surgery?

 

A1. No. Since the causes of drooping eyelids vary from person to person, an accurate diagnosis is required first. It could simply be dermatochalasis, where the skin of the upper eyelid is stretched, or it could be ptosis, where the function of lifting the eyelid is weakened. In some cases, the eyelids may appear droopy because the eyebrows themselves have dropped. Therefore, rather than deciding on a surgical method based solely on the shape of the eyelids, it is important to determine the necessary surgery after comprehensively evaluating factors such as skin sagging, eyelid position, eye-opening strength, and eyebrow position.

 

Q2. I have to strain my forehead a lot when opening my eyes. Do I need double eyelid surgery?

 

A2. If you raise your eyebrows or wrinkle your forehead every time you open your eyes, you need to check the condition of your eyelids. If the eyelids do not lift sufficiently, you may use the frontal muscles in your forehead to compensate. This phenomenon can occur particularly if you have ptosis or if sagging skin covers your eyes. However, you cannot conclude that you have ptosis based solely on the symptom of straining your forehead. You must select a treatment method appropriate to the cause after accurately evaluating the position of the eyelids and the function of opening your eyes.

A3. If upward or peripheral vision is actually restricted due to sagging eyelid skin or ptosis, functional improvement can be expected through appropriate surgery. However, the decision to undergo surgery is not made based solely on the feeling that one's eyes feel "tight." Therefore, while correcting skin sagging or ptosis, the height, shape, and left-right balance of the double eyelid can also be taken into account to suit the patient's eye structure. However, for patients requiring functional improvement, securing the normal function of the eyelids must take precedence over cosmetic design. If sufficient tissue adjustment is required, the incisional method may be more suitable.

A5. In this case, it is important to identify the cause first rather than deciding on cosmetic surgery immediately. In particular, if drooping of one eyelid occurs suddenly and was not present before, there may be causes related to the eyelid muscles or nerves, in addition to simple aging or cosmetic asymmetry. If diplopia (seeing double), changes in pupil size, severe headaches, or other neurological symptoms occur together, a prompt medical evaluation may be necessary. Therefore, regarding sudden eyelid drooping, it is safer to determine the necessary treatment or surgery after first diagnosing the exact cause, rather than thinking that "creating a double eyelid will solve the problem."
