Treatment and prevention of recurrence of gastric and duodenal ulcers; treatment of esophagitis and heartburn; improvement of gastric lesions
This medication is used to treat active gastric and duodenal ulcers, and to prevent the recurrence of duodenal ulcers. It is also effective in alleviating heartburn symptoms associated with endoscopically diagnosed erosive and ulcerative esophagitis and gastroesophageal reflux disease. It also helps improve gastric mucosal lesions such as erosion, bleeding, redness, and edema that may occur during exacerbations of acute or chronic gastritis. Thus, this medication is used to treat and improve symptoms of various gastric and esophageal-related diseases.
1. Treatment of active gastric and duodenal ulcers Adults: Administer nizatidine 300mg once daily at bedtime, or 150mg twice daily, in the morning and evening, orally. Most ulcers heal within 4 weeks, but treatment may be continued for up to 8 weeks if needed. In cases of active gastric ulcer, malignancy must be ruled out before treatment. 2. Prevention of recurrence of duodenal ulcers After healing of a duodenal ulcer, adults should take 150mg orally once daily at bedtime. Treatment may be continued for up to 1 year; outcomes with longer-term administration are not known. 3. Gastroesophageal reflux disease Adults: Administer 150mg orally twice daily, in the morning and evening. Treatment may be continued for up to 12 weeks. 4. Improvement of gastric mucosal lesions Adults: Administer 150mg orally twice daily, in the morning and evening, for 2 weeks. 5. Dosage adjustment in patients with severe renal impairment Reduce the dose for patients with renal impairment as follows. Active duodenal ulcer, gastroesophageal reflux disease (GERD), benign gastric ulcer: Creatinine clearance 20~50 ml/min, 150mg daily Active duodenal ulcer, gastroesophageal reflux disease (GERD), benign gastric ulcer: Creatinine clearance < 20 ml/min, 150mg every other day Maintenance therapy: Creatinine clearance 20~50 ml/min, 150mg every other day Maintenance therapy: Creatinine clearance < 20 ml/min, 150mg every 3 days Some elderly patients may have creatinine clearance below 50mL/min. Based on pharmacokinetic data in patients with renal impairment, the dose should also be reduced in such patients according to the degree of renal impairment. The clinical effects of this dose reduction in patients with renal impairment have not been evaluated.
This medication should not be administered to certain patients. In particular, it should not be used in patients with allergies to this medication or other H2-receptor antagonists. When using this medication, rare instances of hypersensitivity reactions such as skin rash, itching, and difficulty breathing may occur, and use should be discontinued immediately if such symptoms arise. Nursing mothers should be cautious as the drug may be secreted in breast milk. Safety in children has not been adequately verified, so caution is required in its use. This medication may mask symptoms of gastric cancer; therefore, it should be administered only after confirming that it is not a malignant condition, and if no therapeutic effect is observed, it is advisable to change to another treatment. In cases of overdose, symptoms such as vomiting, diarrhea, and excessive salivation may occur, and gastric lavage or supportive therapy should be performed as necessary. Hemodialysis is not effective in removing this medication. Lastly, this medication generally has few drug interactions; however, caution should be exercised when used concomitantly with high doses of aspirin, as blood salicylate levels may increase.
