Precautions

This drug should not be administered to certain patients. For example, individuals allergic to this drug or ergot alkaloids, patients with acute myocardial infarction, acute bleeding, those taking sympathomimetic agents, patients with very slow heart rates (bradycardia), patients who have not completed hemostasis after intracranial hemorrhage, and those with orthostatic hypotension should not use this drug. Moreover, patients with mild bradycardia or renal impairment should use this drug with caution. This drug may cause gastrointestinal symptoms such as loss of appetite, diarrhea, constipation, nausea, vomiting, abdominal pain, and dry mouth. There is also a rare possibility of elevated liver function tests such as ALT and AST. Cardiovascular symptoms may include dizziness, orthostatic dysregulation, hypertension, hypotension, and elevated serum uric acid levels. Central nervous system side effects may include drowsiness, lethargy, headaches, tinnitus, fatigue, and insomnia, though these occur rarely. There may also be hypersensitivity reactions such as rash, urticaria, pruritus, facial flushing, and warmth. Careful monitoring of blood pressure is advised when used with antihypertensives, as blood pressure may decrease further. Attention is needed in patients with a history of hyperuricemia or gout due to its effect on uric acid metabolism. Caution is advised when driving or operating machinery after taking this drug. This drug is metabolized by a specific liver enzyme (CYP450 2D6), which may interact with other drugs sharing this metabolic pathway. Pregnant women or those who may become pregnant should use this drug only when the therapeutic benefits outweigh the potential risks to the fetus. If breastfeeding, it is recommended to discontinue nursing as this drug may be secreted in breast milk. Elderly patients may have reduced physiological functions, so dose adjustments may be necessary when using this drug. Safety in children has not been adequately confirmed, so caution is advised for use in this population. In case of overdose, blood pressure may temporarily decrease, so blood pressure changes should be closely monitored and appropriate measures taken. This drug must be kept out of the reach of children and should be stored in its original container tightly closed.
