Summary

Main use
Treatment of herpes zoster virus infection
Who takes it
Adults
Adult dose
1. Treatment of herpes zoster Adults: Administer famciclovir 750 mg orally once daily for 7 days. Take it at the same time each day. It is preferable to start treatment as soon as symptoms appear after infection. 2. Patients with renal impairment: Because the clearance of this medicine is reduced in patients with renal impairment, dose adjustment should be made with caution. The dosage for these patients is as follows. 1) Treatment of herpes zoster Creatinine clearance (mL/min/1.73㎡) 30~59 : 250 mg per dose, twice daily 10~29 : 250 mg per dose, once daily 2) If only serum creatinine is known, creatinine clearance may be calculated using the following formulas. Men: {body weight (kg) × (140-age)}/ {72 × serum creatinine (mg/dL)} Women: 0.85 × the value calculated for men 3) Patients with renal impairment undergoing hemodialysis: For patients on hemodialysis, administration every 48 hours is preferable, taking the dialysis interval into account. Since 4 hours of hemodialysis reduces the plasma concentration of this medicine by approximately 75%, administer the full dose immediately after dialysis. 3. Patients with hepatic impairment: No dose adjustment is required in patients with chronic compensated liver disease. There is insufficient treatment experience in patients with overt chronic decompensated liver disease; therefore, no clear recommended dose has been established.

Precautions

This medication should not be used by patients who have had allergic reactions to this medication or its components, including the active substance, penciclovir. Patients with renal impairment should use this medication with caution. In clinical trials, symptoms such as headache and nausea have been reported, but they were mostly mild, and similar symptoms were seen in patients who did not take the medication. Rarely, blood-related abnormalities such as thrombocytopenia may occur, and there is a possibility of decreased white blood cell and neutrophil counts. Common central nervous system symptoms include headache and dizziness, while confusion, drowsiness, and hallucinations may occur rarely, with drowsiness being more frequent in the elderly. Rarely, heart-related symptoms such as palpitations may occur. Gastrointestinal symptoms such as nausea, vomiting, abdominal pain, and diarrhea may occur frequently. Liver function abnormalities may occur frequently, and jaundice may occur rarely. Skin symptoms may include rash and itching frequently, and angioedema such as hives or swelling around the face, eyes, and pharynx may occur rarely. Severe skin reactions have also been reported. In patients with impaired renal function, acute renal failure may rarely occur, necessitating dose adjustments. This medication does not completely cure genital herpes, and if symptoms occur during treatment, sexual contact should be avoided and safe sexual practices maintained. There is no evidence that this medication affects driving or machinery operation; however, it is advisable to avoid these activities if dizziness, drowsiness, or confusion occurs. Patients with mild or moderate liver function impairment generally do not require dose adjustments, but patients with severe liver dysfunction may experience reduced efficacy. This medication has a low potential for interactions with other drugs; however, caution is advised when taken with drugs such as probenecid, as it may increase blood concentration and toxicity risk. Pregnant women or women who may become pregnant should only use this medication when the benefits of treatment outweigh the risks, and nursing mothers should use it cautiously as well. It is advisable to avoid use in children under 18 years of age as safety and efficacy have not been established. Generally, elderly patients do not require dose adjustments but should have their doses adjusted if renal function is impaired. In cases of overdose, appropriate treatment should be received, and blood dialysis can reduce the drug concentration. This medication should be stored in a closed container to protect it from air and moisture. Additionally, it is important to store the medication in a cool, dry place below 30 degrees Celsius, out of reach of children, and not to transfer it to another container.
