This medication may increase the risk of suicidal thoughts and behaviors in pediatric, adolescent, and young adult patients with major depressive disorder or other psychiatric disorders; therefore, the physician should carefully assess the need for treatment and risks. During the initial months of treatment or when adjusting the dosage, the patient's condition must be closely monitored for any signs of suicidal tendencies or worsening depression. Family members or caregivers should also carefully observe changes in the patient's behavior and notify healthcare professionals immediately if any atypical signs are noticed. This medication is not approved for the treatment of pediatric depression, and caution should be exercised in patients suspected of having bipolar disorder. Serious side effects can occur, such as neuroleptic malignant syndrome, which can be fatal, hence immediate treatment is required if symptoms such as high fever, muscle rigidity, or changes in mental state arise. Irreversible tardive dyskinesia may develop, so the lowest dose and shortest duration of treatment should be used for long-term administration; consideration should be given to discontinuing the medication if symptoms arise. In dementia patients, the risk of cerebrovascular accidents, such as stroke, may increase, so usage is not recommended. Regular monitoring of blood glucose is necessary in diabetic patients or those with risk factors, as this medication can affect blood sugar control. There is a risk of venous thromboembolism, so caution is warranted in patients with thrombotic risk factors. Patients with hypersensitivity to this medication or lactose intolerance should not use it. It should be administered cautiously in patients with orthostatic hypotension, history of seizures, or impaired cognitive and motor functions, and caution should be exercised while driving or operating machinery due to symptoms such as drowsiness. The ability to regulate body temperature may be diminished, so care should be taken in hot environments or during strenuous exercise. In elderly patients or those with dementia who have dysphagia, there is a risk of aspiration pneumonia, so the medication should be used carefully. Various adverse reactions may occur during treatment; therefore, symptoms such as nausea, vomiting, headache, dizziness, insomnia, anxiety, drowsiness, weight gain, nasopharyngitis, restlessness, tremors, sedation, blurred vision, hypersalivation, drooling, malaise, decreased appetite, stuttering, hyperactivity, sleep disturbances, palpitations, abnormal movements, numbness, hallucinations, gastrointestinal disturbances, urinary disorders, frequent urination, dry mouth, muscle pain, musculoskeletal stiffness, muscle spasms, and limb pain should be closely monitored. In particular, motor disorders such as akathisia, extrapyramidal symptoms, and tremors may occur; if such symptoms appear, consultation with a healthcare professional is advised. Long-term use may lead to weight gain and changes in the electrocardiogram, so regular testing is necessary. The appropriate dosage and duration of treatment should be determined based on the patient's condition, and the treatment plan should be adjusted by consulting a physician immediately in case of adverse reactions or worsening symptoms.
