Adult dosage
For adults: Administer 500mg (potency) of vancomycin intravenously every 6 hours, or 1g (potency) every 12 hours. Oral administration: 500-2000mg per day divided into 3-4 doses for 7-10 days. The total daily dose should not exceed 2g. Intermittent infusion: Infuse the prepared injection at least over 60 minutes. Continuous infusion: Only if intermittent infusion is not possible.
Children's dosage
For children: 10mg (potency) per Kg body weight intravenously every 6 hours. For infants: 10-15mg (potency) per Kg body weight intravenously every 12 hours. For neonates: 1 dose per body weight.

This medication is effective against various harmful bacteria such as Streptococcus, Staphylococcus, Clostridium difficile, and diphtheroids. It is used to treat endocarditis, osteomyelitis, arthritis, peritonitis, meningitis, as well as superficial secondary infections from burns or surgical sites. It is also effective for serious systemic infections such as pneumonia, sepsis, pulmonary abscess, and empyema. It can be administered for severe infections that are not treatable with penicillin or cephalosporin antibiotics. Notably, it is used orally for treating antibiotic-associated colitis caused by Clostridium difficile. This drug effectively treats a range of serious infections.

Dosage and Administration

Adult dosage

For adults: Administer 500mg (potency) of vancomycin intravenously every 6 hours, or 1g (potency) every 12 hours. Oral administration: 500-2000mg per day divided into 3-4 doses for 7-10 days. Precautions

This drug should not be used in patients who have previously experienced severe allergic reactions or shock from this drug. It is also safe to avoid administering this drug to patients with allergies to this or similar types of antibiotics. Caution should be given to patients with hearing impairment or past experiences of hearing loss when administering this drug. In patients with poor renal function, the drug can accumulate in the body, so blood levels should be frequently monitored and adjusted. Patients with reduced liver function should also be cautious as this drug may further impair liver function. Elderly patients, premature infants, and neonates should be administered carefully, as well as patients with damage to the vestibular or auditory systems. There may be many side effects when using this drug, such as neurological disturbances, digestive issues, blood disorders, hearing loss, abnormalities in liver and kidney function, and skin reactions, so symptoms should be closely observed. Especially, if there are severe skin rashes, blisters, or respiratory distress appearing as hypersensitivity reactions, administration should be stopped immediately, and appropriate treatment should be received. Rapid administration of this drug can cause symptoms like hypotension, flushing, and muscle spasms, so it is essential to administer it slowly. The use of antibiotics can disrupt the intestinal bacterial balance, leading to diarrhea or antibiotic-associated colitis, so if diarrhea persists, patients should consult with a physician. The risk of side effects is increased when this drug is used with other medications affecting the kidneys or nerves, so careful monitoring is required. Pregnant or breastfeeding individuals should use this drug only when necessary, as it may affect the fetus or newborn; when breastfeeding, it is recommended to discontinue nursing. Dosing for children or elderly patients should be adjusted considering their renal function, and they should be observed closely. In case of overdose, kidney damage and hearing loss may occur, so the prescribed dosages must be strictly followed. This drug must be administered intravenously; administering it via intramuscular injection or other routes can cause tissue damage or pain. Additionally, care must be taken as this drug can cause physical or chemical instability when mixed with other drugs. Long-term use may lead to resistant bacteria developing; thus, it is preferable to check susceptibility and administer only for the necessary duration. Regular blood tests and liver and kidney function tests should be done during administration, and hearing tests should also be conducted periodically.

Hives, severe swelling (edema), chest tightness, difficulty breathing, cold sweats, pallor, severe rash with fever, blisters, skin peeling, pain in mucous membranes of the mouth/eyes, shortness of breath or difficulty breathing, asthma attacks, respiratory abnormalities accompanied by cough+fever, yellowing of the skin or eyes (jaundice), extreme lethargy, sudden and severe headaches, thunderclap headache, seizures, confusion, visual disturbances, paralysis, urination difficulties.
