Treatment of mood disorders (depression, premenstrual dysphoric disorder), obsessive-compulsive disorder, and bulimia nervosa
This medication is effectively used for several mental health issues such as depression, bulimia nervosa, obsessive-compulsive disorder, and premenstrual dysphoric disorder. In patients with depression, the medication helps alleviate symptoms within 5 to 6 weeks and improves daily functioning. In bulimia nervosa, it reduces binge eating and unnecessary purging behaviors. For patients with obsessive-compulsive disorder, it reduces compulsive symptoms, alleviating difficulties in social and work life. It also helps relieve symptoms of premenstrual dysphoric disorder, lessening mood swings and physical discomfort. In all cases, it is important to regularly evaluate the individual patient's condition during long-term treatment. This medication is used to effectively treat these conditions.
Adults 1. Depression 1) Initial treatment: The initial dose is 20mg of fluoxetine hydrochloride orally once daily in the morning. In controlled clinical trials, doses of 20~80mg daily were administered. If no improvement is observed, an increase in dose may be considered after several weeks. Doses exceeding 20mg daily should be administered twice daily (morning and noon), and the maximum dose of 80mg daily should not be exceeded. The onset of an adequate antidepressant effect may be delayed until 4 weeks or longer after treatment begins. 2) Maintenance/continuation/extended treatment: There are no definitive data on how long patients should be treated with this drug, but psychopharmacologists agree that acute episodes of depression require several months or more of drug therapy. It is not known whether the amount of antidepressant needed to relieve symptoms is the same as that needed to maintain or sustain remission. 2. Bulimia nervosa Administer 60mg of this drug daily. The daily dose should not exceed 80mg. Measure electrolyte levels before starting treatment. 3. Obsessive-compulsive disorder Administer 20~60mg of this drug daily. The daily dose should not exceed 80mg. 4. Premenstrual dysphoric disorder Administer 20mg of this drug daily. Reassess the need for continued treatment after 6 months of treatment. Patients with renal and/or hepatic impairment, patients receiving multiple medications, older adults, and patients with concomitant diseases: Administer a lower dose or reduce the frequency of administration. Adjust the dose as appropriate according to age and symptoms. Switching from MAO inhibitors used to treat psychiatric disorders Allow an interval of at least 14 days between discontinuing an MAO inhibitor used to treat psychiatric disorders and starting treatment with this drug. Conversely, at least 5 weeks must elapse after discontinuing this drug before starting an MAO inhibitor to treat psychiatric disorders. Other MAO inhibitors, such as linezolid or methylene blue This drug should not be initiated in patients receiving linezolid or intravenous methylene blue because of the increased risk of serotonin syndrome. In patients who require more urgent treatment for a psychiatric condition, including hospitalization or other interventions, consider administering treatment. It may be necessary to urgently administer linezolid or intravenous methylene blue to a patient already receiving this drug. If no alternative to linezolid or intravenous methylene blue is available and, for a particular patient, the benefits of treatment with linezolid or intravenous methylene blue are judged to outweigh the risk of serotonin syndrome, discontinue this drug immediately and administer linezolid or intravenous methylene blue. Monitor the patient for symptoms of serotonin syndrome for 5 weeks after administration of linezolid or intravenous methylene blue, or until 24 hours after the last dose, whichever occurs first. Treatment with this drug may be resumed 24 hours after the last dose of linezolid or intravenous methylene blue. The risk associated with administration of methylene blue by non-intravenous routes (oral tablets or local injection), or intravenous administration of this drug at a dose of 1mg/kg or less, is unclear. Nevertheless, clinicians should be aware of the possibility of emergency symptoms of serotonin syndrome with these uses.
This medication may increase the risk of suicidal thoughts or behaviors in children, adolescents, and young adults with major depression or psychiatric disorders, therefore close monitoring is necessary when administered to this age group. This drug should not be used in certain patients, such as those with allergies to this medication or its additives, those with severe kidney impairment, and those who are taking MAO inhibitors. Patients with a history of seizures should be cautious when using this medication, and treatment should be discontinued if seizures occur or increase in frequency. Patients with a risk of suicide require careful monitoring from the beginning of treatment, and family members or caregivers should closely observe the patient. Patients with a history of manic or hypomanic episodes should be cautious when using this medication and should discontinue use if manic symptoms appear. Patients with cardiovascular diseases or those at risk of QT interval prolongation should consider undergoing an ECG while using this medication, and treatment should be immediately stopped if symptoms of arrhythmia occur. Patients with impaired liver or kidney function should be administered a lower dose with caution, and diabetic patients should monitor their blood sugar levels. This medication may dilate pupils and should be used cautiously in patients at risk for increased intraocular pressure or acute glaucoma. Various side effects such as headaches, nausea, insomnia, fatigue, and diarrhea may occur during treatment, so patients should monitor for these symptoms. Withdrawal symptoms may occur when discontinuing this medication, and the dosage should be gradually reduced under a doctor's guidance. This drug may cause a dangerous condition known as serotonin syndrome when used with other serotonergic agents, hence caution is advised when co-administered. Alcohol use is not recommended, and potential interactions with other medications should be discussed with a doctor if the patient is taking other medications. During pregnancy, the risks and benefits to the fetus must be carefully considered before using this medication, and caution is needed as this medication may be secreted in breast milk during breastfeeding. The safety in children and adolescents has not been established, and use is not recommended. Elderly patients should be cautious as there may be a risk of side effects due to other diseases or medications they may be taking. In cases of overdose, serious symptoms such as nausea, vomiting, seizures, and cardiac abnormalities may occur, necessitating immediate medical attention. This medication should be stored out of reach of children, and transferring it to another container should be avoided.
