Additionally, it is not indicated for patients with type 1 diabetes or diabetic ketoacidosis. Patients with severely impaired kidney function or end-stage renal disease, as well as those currently undergoing dialysis, should also avoid this medication. If kidney function drops below a certain level, the dose may need to be reduced or the medication discontinued. Moreover, this medication is not effective in patients with end-stage renal disease. Caution is also required in patients with mild to moderate heart failure, and it is not recommended for patients with severe heart failure. When used with insulin or insulin secretagogues, the risk of hypoglycemia may increase, necessitating adjustment of the doses of these medications. While taking this medication, watch for symptoms of hypoglycemia, infections, particularly genital and urinary infections, and ketoacidosis. Low blood pressure or dehydration symptoms due to decreased fluid volume may also occur, especially in elderly or hypotensive patients. Ketoacidosis is a rare yet serious side effect that can occur even if blood sugar is not high; if severe nausea, vomiting, abdominal pain, or difficulty breathing arise during treatment, immediate consultation with medical staff is necessary. There may also be a risk of liver damage, so monitor for symptoms of abnormal liver function. Caution is required when driving or operating machinery, especially for those at risk of hypoglycemia.
