Improvement of blood glucose control and prevention of cardiovascular events in type 2 diabetes
This drug is used to help control blood glucose levels in adults with type 2 diabetes. It can be administered as monotherapy in conjunction with dietary and exercise therapy. If blood glucose control is not sufficient with monotherapy, it can be used in combination with metformin, or together with metformin and other diabetes medications to enhance blood glucose management. Additionally, it can be administered in conjunction with insulin therapy or with DPP-4 inhibitors. Extra caution is required for patients with cardiovascular disease who have difficulty controlling glucose levels. This drug is used to improve blood glucose control in patients with type 2 diabetes.
The recommended dose for this drug in monotherapy and as an additional combination therapy with other glucose-lowering agents such as insulin is 10 mg once daily. If the patient has good tolerance to the 10 mg dose and requires additional blood glucose control, this dose can be increased to 25 mg once daily. This drug can be taken regardless of meals. The tablet should be swallowed whole. If a dose is missed, it should be taken as soon as remembered.
This drug may not be suitable for certain patients. First, it should not be given to patients who are allergic to the active ingredient or any of the components. Also, it is prohibited for use in patients with type 1 diabetes, diabetic ketoacidosis, severely impaired kidney function (glomerular filtration rate below 30 mL/min/1.73m2), end-stage kidney disease, or those undergoing dialysis. This drug contains lactose, so it should not be administered to patients with lactose intolerance, galactose intolerance, or genetic lactase deficiency. For patients with slightly impaired kidney function (glomerular filtration rate below 60 mL/min/1.73m2), it is advisable not to initiate treatment with this drug. If tolerance is good and treatment has started, the dosage should be adjusted or maintained, and treatment should be discontinued if kidney function worsens. In patients with end-stage kidney disease or those undergoing dialysis, the efficacy of this drug is not expected, so it is not used. In patients with heart failure, this drug should be used with caution in mild or moderate cases and is not recommended for severe heart failure patients. When this drug is used in conjunction with insulin or sulfonylureas, the risk of hypoglycemia may increase, so the dosages of these medications may need to be reduced. During the course of treatment with this drug, rare adverse reactions such as ketoacidosis, urinary tract infections, genital infections, fluid volume depletion, and hypoglycemia may occur, so caution is necessary. In particular, ketoacidosis can occur even if blood glucose levels are not high, so if symptoms such as vomiting, abdominal pain, breathing difficulties, or malaise appear, immediate consultation with medical personnel is required. Additionally, this drug can affect the body's fluid status, so elderly patients (especially those over 75 years old) or those with a history of hypotension should be particularly cautious of fluid volume loss and hypotension risks. Regular monitoring of kidney function is recommended during the use of this drug, and assessment should be immediately carried out if kidney function impairment is suspected. Women who are pregnant or planning to become pregnant should avoid this medication, and breastfeeding should be discontinued if this drug is taken during lactation. No specific toxic symptoms have been reported in case of overdose, but appropriate treatment is necessary based on the clinical situation. This drug has not been shown to be safe and effective for children (under 18 years of age), so it should not be used. Finally, caution is necessary regarding the risk of hypoglycemia when driving or operating machinery, especially when taken with insulin or sulfonylureas. Regular health check-ups and attention to unusual symptoms are important during treatment, and this medication should be taken according to a doctor's instructions.
