Summary

Main use
Prevention of thrombotic cardiovascular events (such as myocardial infarction) and stroke
Who takes it
Adults · Children
Adult dose
Do not use this product in combination with other ADP receptor inhibitors (ticlopidine, clopidogrel, prasugrel). Acute coronary syndrome: Administer an initial dose of 180mg, followed by 90mg twice daily. Treatment for 12 months is recommended unless discontinuation of this product is clinically necessary. Patients taking this product should also receive a maintenance dose of aspirin (75 - 150mg), unless specifically contraindicated. In patients with acute coronary syndrome at increased risk of bleeding who have undergone percutaneous coronary intervention (PCI), monotherapy with this product (discontinuation of aspirin) may be considered after 3 months of concomitant treatment with this product and aspirin. (See 3. Use with caution in the following patients.) Patients with a history of myocardial infarction (occurring at least 1 year previously) who are at high risk of thrombotic cardiovascular events: Administer 60mg twice daily. Patients taking this product should also receive a maintenance dose of aspirin (75 - 150mg), unless specifically contraindicated. - Patients who took this product 90mg or another ADP receptor inhibitor (ticlopidine, clopidogrel, prasugrel) during the first year after myocardial infarction may continue treatment with this product 60mg. - Treatment with this product may also be initiated in patients within 2 years after myocardial infarction or within 1 year after discontinuing a previous ADP receptor inhibitor (ticlopidine, clopidogrel, prasugrel). - The safety and efficacy of this product were evaluated at 3 years of follow-up. Patients with type 2 diabetes who have undergone percutaneous coronary intervention (PCI) for coronary artery disease: Administer 60mg twice daily. Patients taking this product should also receive a maintenance dose of aspirin (75 - 150mg), unless specifically contraindicated. - For patients with acute coronary syndrome, refer to the corresponding dosage and administration. - Data on the safety and efficacy of this product when administered for more than 3 years are limited. Patients with acute ischemic stroke or high-risk transient ischemic attack: Administer an initial dose of 180mg, followed by 90mg twice daily for 30 days. Patients taking this product should also receive a maintenance dose of aspirin (75 - 100mg), unless specifically contraindicated. An initial loading dose of aspirin (300-325mg) is recommended in patients with acute ischemic stroke or high-risk transient ischemic attack. This product is administered orally and may be taken with or without food. For patients who have difficulty swallowing tablets, crush this product into a fine powder, disperse it in half a glass of water, and drink immediately; rinse the cup with another half glass of water and drink it. This mixture may also be administered through a nasogastric tube (CH8 or larger); it is important to flush the nasogastric tube with water after administration. When switching from another antiplatelet agent to this product, start this product 24 hours after the last dose of the previous medication. If a dose is missed, take only 1 tablet (the next scheduled dose) at the next scheduled dosing time. No dose adjustment is necessary in patients with renal impairment. No dose adjustment is necessary in patients with mild hepatic impairment. This product has not been studied in patients with severe hepatic impairment, and experience with its use in patients with moderate hepatic impairment is limited.
Child dose
The safety and efficacy of this product in pediatric patients under 18 years of age have not been established (see “Other” section).

This medication helps to reduce the incidence of thrombotic cardiovascular events when used in combination with aspirin. It is mainly used for patients with acute coronary syndrome or those at high risk of thrombotic cardiovascular events who experienced a myocardial infarction more than one year ago. Additionally, it reduces the incidence of myocardial infarction and stroke in patients with coronary artery disease who have a history of percutaneous coronary intervention and no history of myocardial infarction or stroke, provided they are taking aspirin. It is also used in combination with aspirin to prevent the recurrence of stroke in patients with acute ischemic stroke or high-risk transient ischemic attack (TIA). This medication effectively lowers the risk of cardiovascular and cerebrovascular diseases.

Precautions

This medication prevents the aggregation of platelets in the blood to prevent the formation of clots. However, it may increase the risk of bleeding, so it should not be used in patients with an active bleeding condition or a history of intracranial hemorrhage. If surgery is planned, this medication should be discontinued at least 5 days before to reduce the risk of bleeding. Symptoms of bleeding should be reported to medical personnel immediately, and it is important to treat the bleeding rather than discontinuing the medication if possible. Patients allergic to any components of this medication or those with severe liver impairment should not take this medication. Caution should be exercised in patients with a high risk of bleeding, those with recent trauma or surgery, and those with coagulation disorders, especially when taken in combination with other drugs that may cause bleeding. Particularly, caution is necessary when used in conjunction with warfarin as the risk of bleeding may be increased. This medication may have side effects that slow the heart rate; therefore, caution is necessary for patients who experience bradycardia or are taking medications that cause bradycardia. Patients with asthma or chronic obstructive pulmonary disease should follow their doctor's instructions carefully when taking this medication, as it may cause breathing difficulties. Pregnant or breastfeeding women should consult with their doctor before taking this medication, and women of childbearing age should use appropriate contraception to avoid pregnancy. The safety and efficacy of this medication have not been established for children and adolescents under 18 years of age. In the event of overdose, the risk of bleeding may increase, and immediate medical attention is necessary. This medication may interact with other drugs, so patients should inform their healthcare provider of any other medications they are taking, especially when taken with CYP3A4 inhibitors or inducers and P-gp substrate medications. Symptoms such as dizziness or confusion may occur during treatment, so caution is advised when driving or operating machinery. Store the medication out of reach of children and keep it in its original container for safety.
