This medication is used for surface anesthesia prior to injections, dental impressions, X-rays, and tartar removal in dentistry. In ENT, it is applied for procedures like maxillary sinus puncture, nasal, pharyngeal, and oropharyngeal surgical interventions, and punctures. It is also used to reduce pain during the insertion of endotracheal tubes, laryngoscopes, bronchoscopy, and esophagoscopy in respiratory and digestive tracts. In obstetrics, it is utilized as an analgesic adjunct during the final stages of labor and prior to episiotomy and perineal suturing. It effectively alleviates discomfort and pain in surface anesthesia for various regions.

Dosage and Administration

Adult dosage

1. Dental area: Apply lidocaine to the mucosal area at a dose of 10-50mg (1-5 sprays) with anesthetic effect appearing within 1-2 minutes. 2. ENT area: For maxillary sinus puncture, administer 30mg of lidocaine (3 sprays) with anesthetic effect appearing in 2-3 minutes. 3. Precautions

This medication should not be used in patients with hypersensitivity to amide local anesthetics. It should be used cautiously in patients with mucosal damage or infection at the application site, patients with epilepsy, those with heart conduction disorders or impaired liver function, patients in severe shock or with severe renal failure, as well as the elderly and those who are generally weak. When using this medication, patients should be monitored closely for signs of shock such as hypotension, facial pallor, abnormal pulse, and respiratory depression; if unusual symptoms occur, use should be discontinued immediately and appropriate treatment should be administered. Due to its effect on the central nervous system, symptoms such as tremors or seizures may occur; if such symptoms appear, drug administration should be stopped immediately, and necessary treatment should be provided. Symptoms like drowsiness, anxiety, dizziness, nausea, nerve irritability, and loss of consciousness may also occur, thus it’s important to monitor the patient's condition closely to ensure that any adverse reactions do not worsen. Signs of cardiac function suppression such as hypotension, myocardial depression, bradycardia, and cardiac arrest may also occur, so the cardiac condition should be monitored closely. Allergic reactions such as urticaria and edema may occur infrequently, and in severe cases, may progress to anaphylactic shock, necessitating caution. Local irritation may occur at the application site, and applying it to the laryngeal mucosa during endotracheal intubation may result in transient symptoms such as throat pain, hoarseness, or voice loss. This drug is rapidly absorbed, and should systemic concentration become elevated, acute toxicity can occur, hence caution is particularly necessary during excessive dosing or application below the vocal cords. Excessive dosing or administering too frequently can lead to a rapid increase in blood concentration, causing serious adverse reactions; therefore, appropriate dosage and spacing must be maintained. Injured mucosa may absorb more drug, increasing the risk of side effects; therefore, having resuscitation equipment and medications ready in such cases is important. Caution is needed when administering to the oral cavity or pharynx as swallowing may be impaired and aspiration risk may be heightened; also, caution is necessary to avoid biting the tongue or oral mucosa when there is numbness. For patients undergoing general anesthesia, blood concentration may appear higher, and thus dosage adjustment and monitoring are necessary. When used concomitantly with specific cardiac medications, the impact on the heart may be enhanced, requiring close monitoring including ECG. This medication should not be used with plastic material endotracheal tube cuffs as it may cause damage leading to leaks. In patients with acute porphyria, this medication should be used cautiously and only if necessary. Rarely, shock or overdose symptoms may occur with local anesthetic use, making it advisable to use it in an environment where emergency care is possible. Complete prevention of adverse reactions from this drug is not possible, so it is important to monitor the patient's condition closely, use only the minimum amount required, and use only small amounts in areas with rapid absorption. When used with other local anesthetics, there may be an increased risk of toxicity, requiring caution. The safety of this medication during pregnancy has not been established; thus, it should only be administered if the therapeutic benefit outweighs the risks, and while it is known to be secreted in limited amounts during breastfeeding, it generally does not have a significant effect on newborns. In cases of overdose, toxicity may manifest in the central nervous system and cardiovascular system; immediate appropriate emergency treatment is required, and if seizures persist, anticonvulsants should be administered while supporting respiration and circulation.
