Treatment for type 2 diabetes blood glucose control and reduction of cardiovascular risks in chronic heart failure and kidney disease
This medication is used to help control blood sugar levels in patients with type 2 diabetes. It is administered alone or in combination with diet and exercise therapy to improve blood sugar control. It also helps reduce the risk of death and hospitalization due to cardiovascular disease in patients with chronic heart failure. This medication is used alongside other standard heart failure treatments to prevent the worsening of heart failure. In patients with chronic kidney disease, it is effective in reducing the deterioration of kidney function and the risk of kidney-related death. Overall, this medication is used for the treatment of type 2 diabetes, chronic heart failure, and chronic kidney disease.
The recommended dose for this medication is 10 mg once daily for monotherapy and additional combination therapy. When used as initial combination therapy with metformin, it may be given at 5 mg or 10 mg once daily. In cases of chronic heart failure and chronic kidney disease, the dose is 10 mg once daily. There are special dosing guidelines for patients with renal impairment, hepatic impairment, and elderly patients, and the medication can be taken once daily at any time regardless of food intake.
Efficacy and safety in children under 18 years of age have not been established, so it should not be administered.
This medication should not be used by certain patients. For example, individuals with allergies to the active ingredient or excipients should avoid it, as well as patients with type 1 diabetes or diabetic ketoacidosis, those with problems digesting lactose, and patients undergoing dialysis. Careful use should be exercised in patients with reduced fluid volume or kidney function issues. Since this medication can lower blood pressure or affect kidney function, fluid status and kidney function should be thoroughly checked before and after administration. In particular, patients with moderate to severe reductions in kidney function may experience diminished blood glucose control, and it is not recommended for patients with very low eGFR. While taking this medication, it is necessary to be cautious as various side effects such as genital infections, urinary tract infections, hypoglycemia, reduced fluid volume, and diabetic ketoacidosis may occur. Genital infections are more common in women, and immediate treatment should be sought if symptoms of infection appear. The risk of hypoglycemia may increase when taken with other hypoglycemic agents such as insulin or sulfonylureas, thus symptoms of hypoglycemia should be monitored, and if necessary, the dosages of these medications should be adjusted. Also, since this medication may cause a reduction in fluid volume, it is advisable to temporarily stop taking it if symptoms of dehydration or low blood pressure occur. Diabetic ketoacidosis can occur regardless of blood sugar levels, so if symptoms such as vomiting, abdominal pain, or breathing difficulties appear, immediate medical attention should be sought. This medication should also be used with caution in older adults with impaired liver or kidney function, and glucose may be detected in urine tests while taking this medication. It is safer for pregnant or breastfeeding women not to use this medication. Finally, this medication may interact with other medications, so it is important to consult with a physician or pharmacist about appropriate dose adjustments and management, especially when taking diuretics, insulin, or sulfonylureas.
