Treatment of mental disorders (schizophrenia and mania), behavioral and psychological symptoms of dementia, and behavioral disorders associated with intellectual disability
This medication is used for the treatment of schizophrenia, helping to alleviate agitation, aggression, and psychotic symptoms in moderate to severe Alzheimer's-type dementia patients for a short period (up to 6 weeks). Additionally, it is used as an adjunct therapy for mood stabilizers in the treatment of manic episodes of bipolar disorder and is effective in improving disruptive behavior, impulsivity, and self-harm behaviors in children, adolescents, and adults with behavioral disorders. This drug is used to treat various psychiatric symptoms and behavioral issues.
The therapeutic dose of this medication should be adjusted according to the patient's clinical condition, and the lowest effective dose possible should be administered. 1. Schizophrenia — Adults Initial dose: Administer 1 mg of risperidone orally twice daily and increase the dose gradually. Maintenance dose: Administer 1∼3 mg twice daily. Adjust the dose appropriately according to the patient's age and symptoms. At doses of 10 mg or more per day, no additional effects were observed compared with lower doses, and the risk of extrapyramidal symptoms may increase. Doses of 10 mg or more per day should be administered only when the benefits to the individual patient are considered to outweigh the risks. Do not administer doses of 16 mg or more per day, as safety has not been established. If additional sedation is needed, a benzodiazepine may be co-administered. 1. Schizophrenia — Elderly patients and patients predisposed to hypotension The initial dose is 0.5 mg twice daily. Depending on the patient's condition, increase the dose by 0.5 mg twice daily until a dose of 1~2 mg twice daily is reached. In general, increases to doses of 1.5 mg or more twice daily should be made at intervals of at least 1 week. 2. Agitation, aggression, or psychotic symptoms in patients with dementia of the Alzheimer type The initial dose is 0.25 mg of this medication twice daily. If needed, depending on the patient, the dose may be increased by 0.25 mg twice daily at intervals of at least 2 days. The optimal dose for most patients is 0.5 mg twice daily, but the dose may be increased to 1 mg twice daily depending on the patient. Once the patient has reached the optimal dose, once-daily administration may also be considered. As with symptomatic treatment, the appropriateness of continued use of this medication should be evaluated. 3. Adjunctive treatment of mania in bipolar disorder The recommended initial dose is 2 mg of this medication once daily. Depending on the patient, the dose may be increased by 1 mg per day at intervals of 24 hours or more. For most patients, the optimal dose is within the range of 2∼6 mg per day. As with symptomatic treatment, the appropriateness of continued use of this medication should be evaluated. Renal and hepatic impairment For patients with renal or hepatic impairment, administer half the initial and maintenance doses and adjust the dose more gradually. Use this medication with caution in these patients (see 5. General Precautions).
1. Schizophrenia — Clinical experience in children and adolescents under 15 years of age is limited. 3. Adjunctive treatment of manic episodes in bipolar disorder — Clinical experience in children and adolescents under 15 years of age is limited. 4. Conduct disorder and other disruptive behavior disorders — Body weight ≥ 50 kg: The initial dose is 0.5 mg of this medicine once daily. If necessary, the dose may be increased by 0.5 mg per day at intervals of at least 2 days, depending on the patient. For most patients, the optimal dose is 1 mg once daily. Depending on the patient, 0.5∼1.5 mg may be administered once daily. 4. Conduct disorder and other disruptive behavior disorders — Body weight < 50 kg: The initial dose is 0.25 mg once daily. If necessary, the dose may be increased by 0.25 mg per day at intervals of at least 2 days, depending on the patient. For most patients, the optimal dose is 0.5 mg once daily. Depending on the patient, 0.25∼0.75 mg may be administered once daily. 4. Conduct disorder and other disruptive behavior disorders — Clinical experience in children under 5 years of age is limited.
This drug should be used cautiously as it may increase mortality in elderly dementia patients. Particularly, caution is needed when taken concomitantly with certain diuretics like furosemide, which may increase the risk of death. Due to the risk of venous thromboembolism, it is crucial to assess thrombotic risk factors before and during treatment and take preventive measures. This drug should not be administered to patients allergic to this medication or its components, those in states of central nervous system depression or coma, patients receiving epinephrine, and those with lactose intolerance. It should be administered with caution to patients with cardiovascular diseases or a history of arrhythmia, impaired liver or kidney function, patients at risk of suicide, individuals with Parkinson's disease, diabetic patients, the elderly, and women who are pregnant or breastfeeding. This medication can cause various side effects including drowsiness, dizziness, hypotension, movement disorders, gastrointestinal issues, and skin rashes; if any symptoms occur, immediate consultation with healthcare personnel is necessary. As an antipsychotic, there is a rare risk of serious blood abnormalities, allergic reactions, and neuroleptic malignant syndrome, requiring careful monitoring. This drug can enhance central nervous system depressant effects when combined with other central nervous system agents or alcohol, so caution is warranted in such cases. Additionally, as the drug is metabolized in the liver, dosage adjustments and monitoring are required when taken with medications that affect liver function. During pregnancy, it should only be used if necessary, and breastfeeding should be discontinued if feasible. In cases of overdose, visit a medical facility immediately for appropriate emergency treatment, and since there is no antidote for this drug, supportive treatment according to symptoms is necessary. Store out of reach of children, do not transfer to other containers, and strictly adhere to the prescribed dosage and administration.
