Treatment of psychiatric disorders (depression, obsessive-compulsive disorder), bulimia nervosa, and premenstrual dysphoric disorder
This medication is primarily used for the treatment of patients with depression, showing improvement of diagnosed depressive symptoms within 5-6 weeks. It also helps relieve symptoms by reducing binge eating and purging behaviors in patients with bulimia nervosa. In obsessive-compulsive disorders, it significantly reduces obsessive thoughts and behaviors, minimizing the negative impacts on the patient's daily life. It is also effective for premenstrual dysphoric disorder symptoms, used to alleviate mood swings and physical discomfort. However, long-term administration requires regular assessment of each patient's condition, and this medication is used reliably and effectively for various mental health issues.
1. Depression 1) Initial treatment: The initial dose is 20mg of fluoxetine hydrochloride orally once daily in the morning. In controlled clinical trials, doses of 20~80mg per day were administered. If no improvement is observed, an increase in the dose may be considered after several weeks. Doses exceeding 20mg per day should be administered twice daily (morning and noon), and the maximum dose of 80mg per day should not be exceeded. The onset of an adequate antidepressant effect may be delayed until 4 weeks or longer after starting treatment. 2) Maintenance/continuation/extended treatment: There are no definitive data on how long patients should be treated with this drug, but psychopharmacologists recognize that acute episodes of depression require drug treatment for several months or longer. It is not known whether the amount of antidepressant needed to relieve symptoms is the same as that needed to maintain or sustain stability. 2. Bulimia nervosa Administer 60mg of this drug per day. The daily dose should not exceed 80mg. Measure electrolyte concentrations before starting treatment. 3. Obsessive-compulsive disorder Administer 20~60mg of this drug per day. The daily dose should not exceed 80mg. 4. Premenstrual dysphoric disorder Administer 20mg of this drug per day. After 6 months of treatment, reassess whether continued treatment is necessary. Patients with renal and/or hepatic impairment, patients receiving multiple medications, elderly patients, and patients with comorbid conditions: Administer a lower dose or reduce the frequency of administration. Adjust the dose as appropriate according to age and symptoms. Switching from MAO inhibitors used to treat psychiatric disorders When discontinuing an MAO inhibitor used to treat a psychiatric disorder and starting treatment with this product, allow an interval of at least 14 days. Conversely, when starting an MAO inhibitor to treat a psychiatric disorder, at least 5 weeks must have elapsed after discontinuing this product. Other MAO inhibitors, such as linezolid or methylene blue Patients receiving linezolid or intravenous methylene blue should not start treatment with this product because of the increased risk of serotonin syndrome. For patients who require more urgent treatment of a psychiatric condition, including hospitalization or other interventional procedures, treatment should be considered. It may be necessary to urgently administer linezolid or intravenous methylene blue to a patient already receiving this product. If no alternative to linezolid or intravenous methylene blue is available and the benefits of treatment with linezolid or intravenous methylene blue are judged to outweigh the risk of serotonin syndrome for a particular patient, discontinue this product immediately and administer linezolid or intravenous methylene blue. Monitor the patient for symptoms of serotonin syndrome until 5 weeks after administration of linezolid or intravenous methylene blue, or 24 hours after the last dose, whichever occurs first. Treatment with this product may be restarted 24 hours after the last dose of linezolid or intravenous methylene blue. The risk associated with administration of methylene blue by non-intravenous routes (oral tablets or local injection) or intravenous administration of this product at doses of 1mg/kg or less is unclear. Nevertheless, clinicians should be aware of the possibility of emergency symptoms of serotonin syndrome with these uses.
This medication is used for treating psychiatric disorders such as major depression, and caution is needed as there may be an increased risk of suicidal thoughts and behaviors, especially in children, adolescents, and young adults. This medication should not be administered to patients with hypersensitivity, severe renal dysfunction, or those taking specific drugs like MAO inhibitors. Patients with a history of seizures or suicidal tendencies, those with manic symptoms, heart disease, liver dysfunction, renal dysfunction, or diabetes should use this medication carefully. Common side effects may include headache, nausea, insomnia, fatigue, and diarrhea, while rare but serious side effects may involve severe allergic reactions, arrhythmias, bleeding, or liver function abnormalities. During the initial treatment phase or following dosage adjustments, changes in suicidal thoughts or behaviors may occur; therefore, patients and caregivers should closely monitor such symptoms and consult a doctor promptly if any abnormalities arise. This medication can interact with other drugs, so it is crucial to inform their doctor or pharmacist about any other medications being taken. During pregnancy, there may be risks to the fetus, so careful use should be based on medical advice, and while breastfeeding, consideration should be given to stopping nursing due to the potential for the drug to be excreted in breast milk. Safety and efficacy in children and adolescents have not been sufficiently established, so it is generally recommended not to administer it in these age groups. No specific adverse effects in the elderly have been reported, but caution is necessary depending on other conditions or medications. In case of overdose, severe symptoms like nausea, vomiting, seizures, and heart issues may occur, thus immediate medical attention is required. Gradual dosage reduction is necessary to minimize withdrawal symptoms when discontinuing treatment. Caution should be exercised while driving or operating hazardous machinery; alcohol consumption is not recommended. Keep out of reach of children and avoid transferring to other containers to maintain quality and prevent accidents.
