Summary

Main use
Treatment of herpes zoster and genital herpes; suppression of recurrent genital herpes
Who takes it
Adults
Adult dose
1. Treatment of herpes zoster Adults: Administer famciclovir 250 mg orally 3 times daily for 7 days. It is preferable to start treatment as soon as symptoms appear after infection. 2. Treatment of an initial episode of genital herpes Adults: Administer this medicine 250 mg orally 3 times daily for 5 days. It is preferable to start treatment as soon as symptoms appear after infection. 3. Treatment of acute recurrent genital herpes Adults: Administer this medicine 125 mg orally twice daily for 5 days. It is preferable to start treatment during the prodromal period or as soon as symptoms appear after infection. 4. Suppression of recurrent genital herpes Adults: Administer this medicine 250 mg orally twice daily. Discontinue treatment periodically every 6~12 months to observe whether the natural course of the disease has changed. 5. Patients with renal impairment: Since the creatinine clearance of this medicine is reduced in patients with renal impairment, dose adjustment should be undertaken carefully. The dosage for these patients is as follows. 1) Treatment of herpes zoster and an initial episode of genital herpes: Creatinine clearance (mL/min/1.73㎡) 30~59 250 mg per dose twice daily 10~29 250 mg per dose once daily 2) Treatment of acute recurrent genital herpes: Creatinine clearance (mL/min/1.73㎡) 30~59 125 mg per dose twice daily 10~29 125 mg per dose once daily 3) Suppression of recurrent genital herpes: Creatinine clearance (mL/min/1.73㎡) 30~59 250 mg per dose twice daily 10~29 125 mg per dose twice daily 4) If only serum creatinine is known, creatinine clearance can be calculated using the following formula. Men: {body weight (kg) × (140-age)}/ {72 × serum creatinine (mg/dL)} Women: 0.85 × the value calculated for men 5) Patients with renal impairment undergoing hemodialysis For patients on hemodialysis, it is preferable to administer this medicine every 48 hours, taking the interval between dialysis sessions into account. Since 4 hours of hemodialysis reduces the plasma concentration of this medicine by approximately 75%, administer it immediately after dialysis. The recommended dose is 250mg for patients with herpes zoster and 125mg for patients with genital herpes. 6. Patients with hepatic impairment: No separate dose adjustment is necessary in patients with chronic compensated liver disease. Since there is insufficient treatment experience in patients with overt chronic decompensated liver disease, no clear recommended dose has been established.

Precautions

This medication should not be administered to certain patients. For instance, patients allergic to this medication or its components, or those with genetic issues digesting lactose, should not take this drug. Additionally, individuals allergic to soybean oil, soy, or peanuts should also avoid this medication. Patients with poor kidney function or problems with fat metabolism should use this medication with extra caution and may need dosage adjustments as necessary. Side effects such as headache, dizziness, nausea, vomiting, and skin rash may occur during the use of this medication, and if symptoms are severe, consultation with a doctor is necessary. Especially in elderly patients or those with impaired kidney function, central nervous system symptoms such as drowsiness or confusion may occur more frequently, requiring caution. This medication does not completely cure genital herpes, and it is advisable to avoid sexual contact during treatment. Maintaining safe sexual relations is key to preventing transmission. If dizziness or drowsiness occurs while driving or operating machinery, caution is needed. Patients with poor liver function generally do not require dosage adjustments, but severe liver impairment may reduce efficacy, so caution is warranted. This medication may interact with other drugs, particularly those affecting renal excretion like probenecid, so it is important to follow the doctor’s instructions when used in combination. Some drug combinations may reduce the effectiveness of this medication or increase its toxicity, so consulting with healthcare professionals is crucial. There is insufficient data proving the safety of this medication for pregnant or breastfeeding women, thus it should only be used cautiously when absolutely necessary. For children under 18 years, its safety and efficacy have not been established, so administration is not recommended. Elderly patients generally do not require dosage adjustments, but adjustments should be made if there are issues with kidney function. In case of overdose, it is crucial to visit a medical institution immediately for proper treatment, and blood dialysis can lower the drug concentration. This medication should be stored away from direct sunlight in a dry place below 30 degrees Celsius, and kept out of reach of children. Additionally, transferring it to another container can pose risks of quality degradation or accidents.
