Summary

Main use
Prevention of thrombotic cardiovascular events (e.g., myocardial infarction) and stroke
Who takes it
Adults · Children
Adult dose
Do not use this medicine concomitantly with other ADP receptor inhibitors (ticlopidine, clopidogrel, prasugrel). Acute coronary syndrome: Administer an initial dose of 180mg, followed by 90mg twice daily. Treatment for 12 months is recommended unless discontinuation is clinically necessary. Patients taking this medicine should also receive a maintenance dose of aspirin (75 - 150mg), unless specifically contraindicated. Patients with a history of myocardial infarction (occurring at least 1 year previously) who are at high risk of thrombotic cardiovascular events: Administer 60mg twice daily. Patients taking this medicine should also receive a maintenance dose of aspirin (75 - 150mg), unless specifically contraindicated. Patients who took this medicine 90mg or another ADP receptor inhibitor (ticlopidine, clopidogrel, prasugrel) during the first year after myocardial infarction may continue treatment with this medicine 60mg. Treatment with this medicine may also be initiated in patients within 2 years after myocardial infarction or within 1 year after discontinuing a previous ADP receptor inhibitor (ticlopidine, clopidogrel, prasugrel). The safety and efficacy of this medicine were evaluated at 3 years of follow-up. Patients with type 2 diabetes who have undergone percutaneous coronary intervention (PCI) for coronary artery disease: Administer 60mg twice daily. Patients taking this medicine should also receive a maintenance dose of aspirin (75 - 150mg), unless specifically contraindicated. Refer to the corresponding dosage and administration for patients with acute coronary syndrome. Data on the safety and efficacy of treatment with this medicine for more than 3 years are limited. Patients with acute ischemic stroke or high-risk transient ischemic attack: Administer an initial dose of 180mg, followed by 90mg twice daily for 30 days. Patients taking this medicine should also receive a maintenance dose of aspirin (75 - 100mg), unless specifically contraindicated. An initial loading dose of aspirin (300-325mg) is recommended for patients with acute ischemic stroke or high-risk transient ischemic attack. This medicine is administered orally and may be taken with or without food. For patients who have difficulty swallowing the tablet, crush this medicine into a fine powder, disperse it in half a glass of water, and drink it immediately. Rinse the glass used with half a glass of water and drink the rinse. This mixture may also be administered through a nasogastric tube (CH8 or larger); it is important to flush the tube with water after administration. When switching from another antiplatelet agent to this medicine, start this medicine 24 hours after the last dose of the previous medicine. If a dose is missed, take only 1 tablet (the next scheduled dose) at the next scheduled dosing time. No dose adjustment is needed in patients with renal impairment. No dose adjustment is needed in patients with mild hepatic impairment. This medicine has not been studied in patients with severe hepatic impairment, and there is limited experience with its use in patients with moderate hepatic impairment.
Child dose
The safety and efficacy of this medicine have not been established in pediatric patients under 18 years of age (see the Other section).

This medication helps reduce the incidence of thrombotic cardiovascular events, such as death due to cardiovascular anomalies, myocardial infarction, and stroke, in adult patients when used in combination with aspirin. It is particularly effective in patients with acute coronary syndrome or a history of myocardial infarction over one year who are at high risk for thrombotic cardiovascular events. Additionally, in patients with type 2 diabetes undergoing PCI for coronary artery disease while receiving aspirin, it reduces the risk of myocardial infarction or stroke. Using this medication with aspirin can also lower the incidence of stroke in patients who have experienced acute ischemic stroke or high-risk transient ischemic attacks (TIAs). Overall, this medication is used to reduce the risk of thrombotic cardiovascular events.

Precautions

This medication is used to prevent the formation of blood clots by inhibiting coagulation, thereby reducing the risk of cardiovascular diseases such as myocardial infarction and stroke. It may increase the risk of bleeding; therefore, patients with ongoing bleeding or a history of intracranial hemorrhage should not use it. Patients undergoing surgery should discontinue this medication at least 5 days prior to the procedure and report any signs of bleeding to their healthcare provider immediately. It should not be administered to patients with hypersensitivity to this medication, severe hepatic impairment, those taking certain antibiotics or antiviral agents, or those with lactose intolerance. Precautions should be taken when administering this medication to patients at high risk of bleeding or those on other anticoagulants. Special care is required for patients with bradycardia, respiratory distress, central sleep apnea, the elderly, impaired renal function, a history of hyperuricemia or gout. This medication may affect platelet function and impact blood test results, so rapid diagnosis and treatment are necessary in cases of suspected thrombocytopenia. Common side effects include bleeding and respiratory distress, while rare effects may include cardiac arrhythmia, thrombocytopenic purpura, and allergic reactions. Interactions with other medications are possible, especially with CYP3A4 inhibitors or inducers and P-glycoprotein substrate drugs; patients must inform healthcare providers of all medications they are taking. Pregnant or nursing women are advised to avoid this medication, and women of childbearing age should avoid pregnancy during treatment. In case of overdose, bleeding risk increases, and medical assistance should be sought immediately; currently, there is no antidote for this medication. The safety and efficacy of this medication have not been established in children, thus its use is not recommended. If symptoms such as dizziness or confusion occur during treatment, patients are advised to avoid driving or operating machinery. This medication should be stored in its original packaging and kept out of the reach of children.
