Treatment of major depressive disorder, anxiety disorders (e.g., panic disorder and social anxiety disorder), and obsessive-compulsive disorder
This medication is used for major depressive disorder and panic disorder, whether or not agoraphobia is present. It is also effective in treating various mental health issues such as social anxiety disorder (social phobia), generalized anxiety disorder, and obsessive-compulsive disorder. It helps improve anxiety and depression symptoms, reducing discomfort in patients' daily lives. Overall, it is used to enhance mental health by improving emotional regulation and alleviating anxiety.
Safety has not been established for doses exceeding 20mg per day. This medication is administered once daily in the morning or evening, regardless of meals. Major depressive disorder: The usual dose is 10mg once daily. Depending on the patient’s response, the dose may be increased to a maximum of 20mg per day. If the dose is to be increased to 20mg, it should be increased at intervals of at least 1 week. In general, antidepressant effects take 2-4 weeks to develop. Even after symptoms have resolved, medication treatment for at least 6 months is required to ensure a therapeutic effect. Panic disorder with or without agoraphobia: It is recommended to start with 5mg per day for 1 week and then increase to 10mg per day. Depending on the patient’s response, the dose may be increased to a maximum of 20mg per day. The maximum effect appears after approximately 3 months. The safety and efficacy of long-term administration of this medication for more than 10 weeks in the treatment of panic disorder have not been systematically studied. The physician should periodically reassess each patient to determine the need for continued treatment. Social anxiety disorder: The usual dose is 10mg once daily. In general, 2-4 weeks are needed for symptoms to improve. Thereafter, depending on the patient’s response, the dose may be reduced to 5mg per day or increased to a maximum of 20mg per day. Social anxiety disorder is a chronic condition, and continued treatment for 12 weeks is recommended to ensure a therapeutic effect. Long-term treatment of responding patients has been studied for 6 months and should be considered individually for each patient to prevent relapse. The usefulness of long-term treatment should be reassessed regularly. Generalized anxiety disorder: The initial dose is 10mg once daily. Depending on the patient’s response, the dose may be increased to a maximum of 20mg per day after at least 1 week of treatment. A long-term study was conducted in patients who responded to 12 weeks of open-label treatment, with 20mg per day administered for at least 6 months. The physician should regularly reassess the usefulness of long-term treatment with this medication for each patient. Obsessive-compulsive disorder: The usual dose is 10mg once daily. Depending on the patient’s response, the dose may be increased to a maximum of 20mg per day. A long-term study was conducted in patients who responded to 16 weeks of open-label treatment, with 10mg or 20mg per day administered for at least 24 weeks. Obsessive-compulsive disorder is a chronic condition, and treatment should be continued for a sufficient period to ensure symptom relief. If this medication is used long term, the benefits and dose should be reassessed regularly for each patient. Patients with renal impairment: No dose adjustment is necessary for patients with mild or moderate renal impairment. Caution is advised in patients with severe renal impairment (CLCR < 30㎖/min). Patients with hepatic impairment: An initial dose of 5mg per day for the first 2 weeks is recommended. Depending on the patient’s response, the dose may be increased to 10mg per day. Discontinuation: If treatment with this medication is discontinued, the dose should be reduced gradually over at least 1-2 weeks to avoid the possibility of withdrawal symptoms. Switching from MAO inhibitors used to treat psychiatric disorders: When discontinuing an MAO inhibitor used to treat psychiatric disorders and starting treatment with this medication, an interval of at least 14 days should be allowed. Conversely, when starting an MAO inhibitor to treat psychiatric disorders, at least 14 days must elapse after discontinuing this medication. Other MAO inhibitors, such as linezolid or methylene blue: Patients receiving linezolid or intravenous methylene blue should not start this medication because of the increased risk of serotonin syndrome. It may be necessary to urgently administer linezolid or intravenous methylene blue to a patient already receiving this medication. If no alternative to linezolid or intravenous methylene blue is available and the benefits of treatment with linezolid or intravenous methylene blue are judged to outweigh the risk of serotonin syndrome in a particular patient, this medication may be discontinued immediately and linezolid or intravenous methylene blue administered. Patients should be monitored for symptoms of serotonin syndrome for 2 weeks after receiving linezolid or intravenous methylene blue, or until 24 hours after the last dose, whichever comes first. Treatment with this medication may be restarted 24 hours after the last dose of linezolid or intravenous methylene blue. The risk associated with administration of methylene blue by non-intravenous routes (oral tablets or local injection) or with intravenous doses of this medication of 1mg/kg or less is unclear.
This medication is primarily used as an antidepressant to treat psychiatric disorders such as depression. It may increase the risk of suicidal thoughts or behaviors in children, adolescents, and young adults (ages 18-24), so care should be taken when prescribing to this age group. Serious side effects may occur if this medication is taken with certain allergens or in conjunction with MAO inhibitors, so these should never be taken together. Patients with glaucoma or a history of closed-angle glaucoma should use this medication cautiously as it can dilate the pupils and increase intraocular pressure. During treatment with this medication, side effects such as anxiety, restlessness, insomnia, appetite changes, decreased libido, digestive issues, headaches, dizziness, rashes, and increased sweating may occur, particularly in the first few weeks after starting treatment. Especially during treatment, patients and caregivers should closely monitor for suicidal thoughts or behaviors, aggression, and unstable emotional changes and consult a doctor immediately if such symptoms arise. Abrupt discontinuation can lead to withdrawal symptoms such as dizziness, anxiety, and sleep disturbances, so the dosage should be reduced gradually under a doctor's guidance. This medication can affect the heart, posing a risk of arrhythmias such as QT prolongation, so patients with heart disease or those taking related medications should consult a doctor before using it. This medication may affect the fetus and newborn during pregnancy, especially in late pregnancy, so caution is required during use in pregnancy, and breastfeeding should be avoided or stopped as the medication can pass into breast milk. Potential interactions with other medications can occur, so be sure to inform your doctor or pharmacist of any other medications you are taking. Care should be taken when driving or operating machinery, and it is important to check your condition after taking this medication. This medication is not recommended for children and adolescents as safety and efficacy have not been established. In the case of overdose, symptoms such as central nervous system symptoms, gastrointestinal disturbances, and heart issues may occur, necessitating immediate medical attention. Finally, this medication should be stored out of reach of children and should not be transferred to other containers.
