For adults with renal impairment, the dose should be adjusted based on renal function (eGFR). The dosage and frequency vary by classification. Normal kidney function (≥ 90 mL/min): 5mg once daily. Mild renal impairment (60 -< 90 mL/min): 1

Precautions

This drug should not be used in patients who are hypersensitive to hydroxyzine or piperazine derivatives or have a history of such hypersensitivity. Patients with end-stage kidney disease or those receiving hemodialysis should also refrain from taking this medication. It should not be administered to infants under 1 year old, and for children under 6 years old, it should be given in liquid form instead of tablet form. The solution contains maltitol, so patients who are congenitally unable to digest fructose should avoid it. Patients with impaired kidney or liver function, as well as the elderly, should use this drug with caution as the blood concentration of this medication may increase. Side effects affecting the nervous system such as drowsiness, headache, dizziness, and fatigue may occur upon taking this medication. Gastrointestinal side effects, such as nausea, decreased appetite, abdominal pain, and diarrhea, may occur, and vomiting can also happen rarely. Rapid or irregular heartbeats, hypertension, and palpitations may occur as cardiovascular side effects. Congenital blood-related side effects such as leukopenia or thrombocytopenia have been reported infrequently. Allergic reactions such as skin rashes, itching, angioedema, and photosensitivity should be approached with caution. Cloudy vision or conjunctival congestion can occur rarely. Periodic testing is necessary as liver and kidney function tests may show temporary increases. In children, side effects such as fever, diarrhea, vomiting, and otitis media may occur more commonly. Drowsiness or fatigue may occur after taking this medication, so caution is advised when driving or operating machinery. Taking this medication with alcohol or antidepressants that affect the central nervous system may exacerbate these symptoms. Patients at risk of urinary obstruction should use caution when taking this medication. In patients with epilepsy or at risk of seizures, this medication can worsen symptoms, so careful administration is required. The solution contains preservatives and sweeteners which can cause allergic reactions or health impacts, so caution is advised. Avoid taking with alcohol, and consider potential interactions with other medications. During pregnancy, safety data is limited, so it should be used cautiously under a doctor's guidance. If breastfeeding, this medication may be secreted in breast milk, which could cause adverse reactions in the baby, so caution is advised. Dosage adjustment is necessary for children and the elderly, especially for the elderly with impaired kidney function, who should start with low doses. It’s important to discontinue this medication prior to any allergy testing as it may affect test results. Overdose may cause drowsiness or restlessness, and there is no specific antidote, so symptomatic treatment should be applied. It is difficult to remove this medication through hemodialysis, thus appropriate medical management is needed in case of overdose.
