Treatment of herpes zoster; treatment of genital herpes and suppression of recurrence
This medication is used to treat herpes zoster virus infections and genital herpes infections. It is also effective in suppressing the recurrence of recurrent genital herpes. This efficacy helps alleviate symptoms for patients and prevent the spread of infection.
1. Treatment of herpes zoster Adults: Administer 250 mg of famciclovir orally per dose, 3 times daily for 7 days. It is preferable to start treatment as soon as symptoms appear after infection. 2. Treatment of initial genital herpes infection Adults: Administer 250 mg of this medicine orally per dose, 3 times daily for 5 days. It is preferable to start treatment as soon as symptoms appear after infection. 3. Treatment of acute recurrent genital herpes infection Adults: Administer 125 mg of this medicine orally per dose, twice daily for 5 days. It is preferable to start treatment during the prodromal period or as soon as symptoms appear after infection. 4. Suppression of recurrent genital herpes Adults: Administer 250 mg of this medicine orally per dose, twice daily. Discontinue treatment periodically every 6~12 months to assess any change in the natural course of the disease. 5. Patients with renal impairment: Because creatinine clearance of this medicine is reduced in patients with renal impairment, dosage adjustment should be made with caution. The dosage and administration for these patients are as follows. 1) Treatment of herpes zoster and initial genital herpes infection: Creatinine clearance (mL/min/1.73㎡) Dose 30~59 250 mg per dose twice daily 10~29 250 mg per dose once daily 2) Treatment of acute recurrent genital herpes infection: Creatinine clearance (mL/min/1.73㎡) Dose 30~59 125 mg per dose twice daily 10~29 125 mg per dose once daily 3) Suppression of recurrent genital herpes infection: Creatinine clearance (mL/min/1.73㎡) Dose 30~59 250 mg per dose twice daily 10~29 125 mg per dose twice daily 4) If only serum creatinine is known, creatinine clearance can be calculated using the following formula: Men: {body weight (kg) × (140-age)}/ {72 × serum creatinine (mg/dL)} Women: 0.85 × the value calculated for men 5) Patients with renal impairment undergoing hemodialysis For patients on hemodialysis, administration every 48 hours is preferable, taking the dialysis interval into consideration. A 4-hour hemodialysis session reduces the plasma concentration of this medicine by approximately 75%; therefore, administer this medicine immediately after dialysis. The recommended dose is 250mg for patients with herpes zoster and 125mg for patients with genital herpes. 6. Patients with hepatic impairment: No dosage adjustment is required for patients with chronic compensated liver disease. Sufficient treatment experience is not available for patients with overt chronic decompensated liver disease; therefore, no definite recommended dose has been established.
This medication should not be administered to certain patients. Patients who have had an allergic reaction to this medication or its components, particularly penciclovir, should not take this medication. Additionally, due to the presence of lactose, patients with lactose intolerance or genetic lactose metabolism issues should also avoid taking it. Patients with allergies to soybean oil, soy, or peanuts should also stay away from this medication. Patients with poor kidney function or those with lipid metabolism issues, such as hyperlipidemia or diabetic dyslipidemia, and pancreatitis should use this medication cautiously. Common side effects of this medication may include headaches, dizziness, drowsiness, nausea, vomiting, abdominal pain, and diarrhea. Rarely reported side effects include thrombocytopenia, leukopenia, hallucinations, palpitations, liver function abnormalities, skin rash, pruritus, angioedema, and acute renal failure. If any of these symptoms occur, consult with a healthcare professional immediately. This medication does not completely cure genital herpes, and sexual contact should be avoided even if symptoms are present during treatment. It is important to maintain safe sexual practices and start treatment immediately if recurrence symptoms appear. While this medication typically does not significantly impact the ability to drive or operate machinery, caution should be exercised if symptoms such as dizziness, drowsiness, or confusion are present. Patients with mild to moderate liver function impairment do not require special dosage adjustments, but those with severe liver dysfunction may experience reduced efficacy of this medication. This medication may interact with certain drugs, so those taking other medications should inform their healthcare provider. In particular, when used with probenecid, increased concentrations of this medication may require dosage adjustments. Women who are pregnant or may become pregnant should use this medication only when the benefits of treatment outweigh the risks. Breastfeeding women should also use it cautiously; there is limited information regarding its excretion in breast milk. Children under 18 years of age should not use this medication as its safety and efficacy have not been established. Generally, elderly patients do not need dosage adjustments, but those with reduced kidney function should have their dosages adjusted accordingly. In cases of overdose, appropriate supportive and symptomatic treatment is necessary, and hemodialysis can be used to reduce drug concentration. This medication should be stored in a dry place below 30 degrees Celsius and out of reach of children. Additionally, transferring it to other containers can compromise its quality, so caution is advised.
Not everyone experiences these, and they can vary from person to person.
This medication may be sensitive to air or moisture, so it must be stored in a tightly sealed container. Also, it should be kept in a dry place below 30 degrees Celsius to ensure its effectiveness.
