Treatment of hypercholesterolemia (primary, familial) and reduction of cardiovascular disease risk
This medication is primarily used for patients with hypercholesterolemia and hyperlipidemia. It is taken as an adjunct when dietary measures and exercise are insufficient to control cholesterol levels. It helps to reduce serum cholesterol and LDL-cholesterol levels, thus delaying the progression of atherosclerosis. It is also effective as an adjunct therapy for patients with familial hypercholesterolemia or dysbetalipoproteinemia. It may be used in pediatric patients exceeding certain criteria to help control blood lipid levels. Furthermore, it contributes to reducing the risk of stroke, myocardial infarction, and revascularization procedures in men over 50 years and women over 60 years who have specific cardiovascular risk factors. Therefore, this drug is used to reduce the risk of cardiovascular diseases through the management of blood lipids.
For patients with primary hypercholesterolemia, combined hyperlipidemia, dysbetalipoproteinemia, and homozygous familial hypercholesterolemia, the initial dosage is 5 mg taken once daily. The maintenance dosage is 10 mg taken once daily, which can be adjusted based on LDL-cholesterol levels and patient response at intervals of 4 weeks or more. The maximum dosage is 20 mg taken once daily. No dose adjustment is required for elderly patients and those with mild to moderate renal impairment.
For pediatric patients with heterozygous familial hypercholesterolemia (ages 10 to 17), the recommended dosage is 5 mg to 20 mg once daily. The dosage may be adjusted individually based on treatment goals at intervals of 4 weeks or more.
This drug should not be used in certain patients. For instance, patients with allergies to any component of this drug, severe liver impairment, muscle diseases, and critically decreased kidney function should not take it. Pregnant or nursing women, as well as women who may become pregnant, must consult a physician before taking this drug and should use appropriate contraceptive methods. Caution is particularly needed with high doses, as they may increase the risk of muscle-related side effects. This drug may affect liver function; therefore, patients with liver disease or those consuming alcohol excessively should have regular liver function tests both before and during treatment. Conditions such as hypothyroidism or nephrotic syndrome must be managed prior to treatment. If symptoms such as muscle pain, weakness, or cramps occur, medical staff should be notified immediately, and blood tests may be necessary to check for muscle damage. If muscle enzyme levels rise significantly above normal, the medication should be discontinued. This drug may interact with certain medications; thus, it is essential to inform the physician or pharmacist if other medications are being taken. In particular, the risk of muscle damage may increase when used with certain antibiotics, antifungals, immunosuppressants, and fibrate class drugs. Safety has not been established for pregnant and nursing women; therefore, during treatment with this drug, it is advisable to avoid planning a pregnancy or taking it if pregnant, and nursing mothers should consult experts. In the event of an overdose, there is no specific antidote, so appropriate treatment and monitoring of liver function and muscle enzyme levels based on symptoms are necessary. Caution is needed when driving or operating machinery, as dizziness may occur. If severe skin reactions or allergy symptoms develop during treatment with this drug, it should be discontinued immediately, and medical attention sought. Lastly, this drug contains lactose, so patients with lactose intolerance should not take it.
Allergic reactions including hives and angioedema, Stevens-Johnson syndrome, drug reaction with eosinophilia and systemic symptoms (DRESS), rhabdomyolysis, immune-mediated necrotizing myopathy, severe muscle weakness, ocular myasthenia, cough, dyspnea, jaundice, hepatitis, fatal and non-fatal liver failure, acute renal failure, oliguria, thrombocytopenia, severe skin reactions, severe swelling or urination abnormalities.
Headache, dizziness, constipation, nausea, abdominal pain, itching, rash, myalgia, weakness, hematuria, cough, diarrhea, chest pain, joint pain, fatigue, numbness, paresthesia, nausea, abdominal pain, diarrhea, loss of appetite, abdominal bloating, abnormal liver function tests, syncope, generalized pain, muscle cramps, gout, erectile dysfunction, depression, sleep disorders (including insomnia and nightmares), sexual dysfunction, gynecomastia, memory impairment, amnesia, and memory loss.
Not everyone experiences these, and they can vary from person to person.
This drug is sensitive to air and moisture, so it should be stored in a tightly sealed container. Additionally, it is best stored at room temperature between 1°C and 30°C, away from extremes of temperature.
