Adult dose
The therapeutic dose of this medicine should be adjusted according to the patient's clinical condition, and the lowest possible effective dose should be used. 1. Schizophrenia Adults Initial dose: Administer 1 mg of risperidone per dose orally twice daily and increase gradually. Maintenance dose: Administer 1∼3 mg per dose twice daily. Adjust as appropriate according to age and symptoms. No additional benefit was observed with doses above 10 mg/day compared with lower doses, and such doses may increase the risk of extrapyramidal symptoms. Doses above 10 mg/day should be administered only when the benefit to the individual patient is considered to outweigh the risk. Do not administer doses above 16 mg/day, as safety has not been established. A benzodiazepine may be coadministered if additional sedation is needed. Elderly patients and patients predisposed to hypotension: The initial dose is 0.5 mg per dose twice daily. Depending on the patient's condition, increase by 0.5 mg twice daily until a dose of 1~2 mg per dose twice daily is reached. Increases to doses above 1.5 mg per dose twice daily should generally be made at intervals of at least 1 week. 2. Agitation, aggression, or psychotic symptoms in patients with dementia of the Alzheimer's type The initial dose is 0.25 mg per dose twice daily. If needed, depending on the patient, the dose may be increased by 0.25 mg per dose twice daily at intervals of at least 2 days. The optimal dose for most patients is 0.5 mg per dose twice daily, but the dose may be increased to 1 mg per dose twice daily depending on the patient. Once the patient has reached the optimal dose, once-daily administration may also be considered. 3. Adjunctive therapy for manic episodes of bipolar disorder The recommended initial dose is 2 mg once daily. Depending on the patient, the dose may be increased by up to 1 mg/day at intervals of 24 hours or more. The optimal dose for most patients is within the range of 2∼6 mg/day. Switching from existing antipsychotic medication to this medicine When medically appropriate, initiate this medicine while gradually reducing the dose of the existing medication. Also, when switching from an existing depot antipsychotic medication, initiate this medicine in place of the next scheduled depot injection, when medically appropriate.

Precautions

Caution is particularly advised when administering this drug to elderly dementia patients, as there may be an increased mortality risk. Co-administration with certain diuretics, such as furosemide, may further increase the risk of death, hence careful consideration of risks and benefits is necessary before combination therapy. There is a risk of venous thromboembolism, thus patients with risk factors should be assessed and precautions taken. This drug contains the artificial sweetener aspartame, and patients with certain genetic disorders such as phenylketonuria must be informed and be cautious before use. Patients with hypersensitivity to this drug, those in a state of central nervous system depression due to alcohol or central nervous system depressants, patients in coma, those receiving epinephrine, and patients with lactose intolerance should not take this medication. Patients with cardiovascular conditions or impaired liver and kidney function should be administered this drug with gradual dose adjustments. Patients with a history of suicide attempts or risk of suicide, those with Parkinson's disease or Lewy body dementia, those at risk for diabetes, as well as pregnant or breastfeeding women need special attention. This drug can cause various neurological symptoms, gastrointestinal symptoms, skin rashes, weight gain, drowsiness, dizziness, among other side effects, and if any abnormal symptoms occur, immediate consultation with healthcare professionals is necessary. Additionally, caution is required as this drug may amplify the central nervous system depressant effects when taken with other CNS depressants or alcohol. This drug is metabolized through CYP2D6, CYP3A4, and P-gp enzymes, thus drug concentration may vary when used in conjunction with drugs that inhibit or induce these enzymes necessitating dose adjustments. Use during pregnancy can affect the fetus, hence women of childbearing potential should consult their physician before taking it, and breastfeeding should be discontinued if nursing. The elderly should be particularly careful with dose adjustments due to the likelihood of decreased kidney function, and in case of overdose, emergency treatment should be sought immediately. This drug is fragile, so do not crush or open the packaging by pressing, and it should be taken immediately after opening, and should not be mixed with tea or cola. Lastly, it should be stored out of reach of children and precautions should be taken to avoid transferring to another container.
