Treatment of mental disorders (depression and obsessive-compulsive disorder), binge-eating disorder, and premenstrual dysphoric disorder
This medication is effective in improving symptoms in patients with depression within 5 to 6 weeks. Depression is accompanied by various symptoms, including persistent sadness that disrupts daily life, changes in appetite and sleep, fatigue, guilt, and decreased concentration. In addition, it is effective in reducing binge eating and purging behavior in patients with bulimia nervosa. It also helps reduce obsessive symptoms in patients with obsessive-compulsive disorder, facilitating social and work life. It alleviates symptoms of premenstrual dysphoric disorder, thereby easing difficulties in daily life. However, regular assessment of efficacy is necessary for each patient during long-term administration. Overall, this drug is effectively used in the treatment of depression, bulimia nervosa, obsessive-compulsive disorder, and premenstrual dysphoric disorder.
1. Depression 1) Initial treatment: The initial dose is fluoxetine hydrochloride 20 mg orally once daily in the morning. In controlled clinical trials, doses of 20~80 mg per day were administered. If no improvement is observed, an increase in dose may be considered after several weeks. Doses exceeding 20 mg per day should be administered twice daily (morning and noon), and the maximum dose of 80 mg per day should not be exceeded. Onset of an adequate antidepressant effect may be delayed until 4 weeks or longer after the start of treatment. 2) Maintenance/continuation/extended treatment: There are no definitive data on how long patients should be treated with this medicine, but psychopharmacologists recognize that acute episodes of depression require several months or more of drug treatment. It is not known whether the amount of antidepressant needed to relieve symptoms is the same as that needed to maintain or sustain remission. 2. Bulimia nervosa Administer 60 mg of this medicine per day. The daily dose should not exceed 80 mg. Measure electrolyte levels before starting treatment. 3. Obsessive-compulsive disorder Administer 20~60 mg of this medicine per day. The daily dose should not exceed 80 mg. 4. Premenstrual dysphoric disorder Administer 20 mg of this medicine per day. After 6 months of treatment, reassess whether continued treatment is necessary. Patients with renal and/or hepatic impairment, patients receiving multiple medications, older adults, and patients with comorbid conditions: Administer a lower dose or reduce the frequency of administration. Adjust the dose appropriately according to age and symptoms. When discontinuing an MAO inhibitor used to treat a psychiatric disorder and starting treatment with this medicine, allow an interval of at least 14 days. Conversely, at least 5 weeks must elapse after discontinuing this medicine before starting an MAO inhibitor for the treatment of a psychiatric disorder. It may be necessary to urgently administer linezolid or intravenous methylene blue to a patient already receiving this medicine. If there is no alternative to linezolid or intravenous methylene blue and the benefits of treatment with linezolid or intravenous methylene blue are judged to outweigh the risk of serotonin syndrome in a particular patient, discontinue this medicine immediately and administer linezolid or intravenous methylene blue. Monitor the patient for symptoms of serotonin syndrome until 5 weeks after administration of linezolid or intravenous methylene blue, or 24 hours after the last dose, whichever occurs first. Treatment with this medicine may be restarted 24 hours after the last dose of linezolid or intravenous methylene blue. The risks associated with methylene blue administered by a non-intravenous route (oral tablets or local injection) or with an intravenous dose of 1 mg/kg or less of this medicine are unclear.
This medication is an antidepressant used in the treatment of major depression and other psychiatric disorders. In children, adolescents, and young adults aged 18-24, the risk of suicidal thoughts and behaviors may increase, so it is important for a physician to carefully weigh the risks and benefits when using this drug. This drug should not be administered to certain patients, such as those with hypersensitivity, those with severe renal impairment, or those currently taking or having recently taken MAO inhibitors. Patients with a history of seizures need to be cautious when using this medication, and treatment should be stopped immediately if seizures occur or their frequency increases. Patients at risk of suicide should be closely monitored, especially during the initial treatment and when changing dosages, and both the patient and guardians should be aware of changes in suicidal thoughts or behaviors and consult medical staff right away. Patients with a history of mania or hypomania should also be cautious when using this medication, and treatment should be discontinued if manic symptoms appear. Patients with heart disease or a risk of QT interval prolongation should undergo ECG monitoring, and treatment should be stopped immediately if there are any abnormal signs. Patients with liver or kidney impairment may need to start at lower doses or may require dosage adjustments. Diabetic patients need to monitor blood glucose levels regularly as there may be changes in glucose control. Patients at risk for increased intraocular pressure or acute glaucoma should also use this medication with caution. Common side effects during treatment may include headache, nausea, insomnia, fatigue, and diarrhea. Withdrawal symptoms may occur when stopping the medication, so abrupt discontinuation should be avoided, and dosage should be reduced gradually per medical advice. If allergic reactions or skin rashes occur, treatment should be discontinued immediately and medical advice sought. This medication may affect blood coagulation, so caution is needed in patients at risk of bleeding. It is advisable to avoid alcohol consumption during treatment, and any interactions with other medications should be discussed with a doctor or pharmacist. During pregnancy, careful consideration of risks and benefits to the fetus is required, and in breastfeeding, consideration may be given to discontinuing breastfeeding as the medication may pass into breast milk. It is recommended to avoid use in children and adolescents as safety and effectiveness have not been established. Elderly patients may have an increased risk of side effects due to decreased body function or other diseases, so caution is required. In cases of overdose, symptoms may include nausea, vomiting, seizures, arrhythmias, and coma, and immediate medical attention is necessary. This medication should be kept out of the reach of children and should not be transferred to other containers.
