Treatment of chronic hepatitis B virus infection
This medication is used in adults with chronic hepatitis B who have active viral replication and persistent elevation of serum aminotransferases (AST or ALT) or have histologically confirmed active disease. It helps to suppress viral activity and improve liver function abnormalities, thereby enhancing the patient's health condition.
This medicine should be taken orally on an empty stomach (2 hours after a meal or at least 2 hours before a meal). The recommended dose is 0.5 mg of entecavir orally once daily. Patients with lamivudine resistance, that is, patients aged 16 years or older who have experienced persistent replication of hepatitis B virus despite lamivudine treatment or who have lamivudine-resistant mutations: 1 mg of entecavir once daily on an empty stomach Patients with renal impairment: In patients with renal impairment, the oral clearance of this medicine decreases as creatinine clearance decreases. Adjust the dose as follows in patients with creatinine clearance below 50 mL/min, including those receiving hemodialysis or continuous ambulatory peritoneal dialysis (CAPD). Creatinine clearance (mL/min) ≥ 50: Entecavir dose 0.5 mg once daily / Entecavir dose in patients with lamivudine resistance 1 mg once daily Creatinine clearance (mL/min) 30 - <50: Entecavir dose 0.25 mg a once daily or 0.5 mg every 48 hours / Entecavir dose in patients with lamivudine resistance 0.5 mg once daily or 1 mg every 48 hours Creatinine clearance (mL/min) 10 - <30: Entecavir dose 0.15 mg a once daily or 0.5 mg every 72 hours / Entecavir dose in patients with lamivudine resistance 0.3 mg a once daily or 1 mg every 72 hours Creatinine clearance (mL/min) <10, hemodialysis b or CAPD: Entecavir dose 0.05 mg a once daily or 0.5 mg every 7 days / Entecavir dose in patients with lamivudine resistance 0.1 mg a once daily or 1 mg every 7 days a: For doses below 0.5 mg, the syrup formulation is recommended. b: If administered on a hemodialysis day, this medicine should be administered after the hemodialysis session. (Pharmacokinetic properties in patients with renal impairment: see “8. Administration to patients with renal impairment” under Precautions for Use)
For lamivudine-resistant patients, that is, patients over 16 years old with persistent replication of hepatitis B virus or resistant mutations despite lamivudine therapy, the dosage is the same as adults: 1mg of entecavir once a day on an empty stomach.
This drug is used to treat liver disease and has several important precautions when used. First, rare but serious liver problems or lactic acidosis may develop when using this drug alone or in combination with certain other antivirals, thus patients with poor liver function should exercise extra caution. Second, if this drug is suddenly stopped, hepatitis B may worsen rapidly, so treatment discontinuation should be carefully managed according to the physician's advice. Third, this drug is not recommended for HIV and hepatitis B co-infected patients who are not receiving HIV treatment; an HIV test is necessary prior to treatment. Fourth, patients with an allergy to this drug or its components or with certain genetic metabolic disorders should not use it. Fifth, patients with impaired renal function or those receiving dialysis need dosage adjustment; liver transplant patients should also carefully monitor kidney function. Sixth, during the use of this drug, side effects such as headache, fatigue, dizziness, nausea may occur, and regular check-ups like liver function tests are recommended. Seventh, this drug may have reduced absorption if taken with food; it is better to take it on an empty stomach. Eighth, during pregnancy, it should be used only when necessary, considering the risks to the fetus, and during breastfeeding, consideration should be given to stopping breastfeeding or discontinuing the medication. Ninth, special caution is needed for certain patient groups such as children, the elderly, and pregnant women, especially for children under 2 years of age, where use is not recommended. Finally, since this drug is primarily excreted through the kidneys, caution is necessary when used in conjunction with other medications that may affect renal function, and medical supervision is required in case of overdose. To use this drug safely, it is essential to follow the doctor's instructions thoroughly and consult immediately if any abnormal symptoms arise during treatment.
Not everyone experiences these, and they can vary from person to person.
