Prevention of thrombotic cardiovascular events (myocardial infarction, stroke, etc.)
This drug helps reduce the incidence of thrombotic cardiovascular events, such as death due to cardiovascular disorders, myocardial infarction, and stroke when used in conjunction with aspirin in adult patients. It is particularly effective in patients with acute coronary syndrome or those with a history of myocardial infarction who are at high risk for thrombotic cardiovascular events. Additionally, it reduces the incidence of myocardial infarction or stroke in patients with type 2 diabetes who have no history of myocardial infarction or stroke but have undergone percutaneous coronary intervention (PCI). It also helps prevent recurrent stroke in patients with acute ischemic stroke or high-risk transient ischemic attack (TIA). Overall, this drug effectively improves the risk of cardiovascular and cerebrovascular events when used in combination with aspirin.
This medicine should not be used concomitantly with other ADP receptor inhibitors (ticlopidine, clopidogrel, prasugrel). Acute coronary syndrome: Administer an initial dose of 180mg, followed by 90mg twice daily. Treatment is recommended for 12 months unless discontinuation is clinically necessary. Patients taking this medicine should also receive a maintenance dose of aspirin (75 - 150mg), unless specifically contraindicated. Patients with a history of myocardial infarction (occurring at least 1 year previously) who are at high risk of atherothrombotic cardiovascular events: Administer 60mg twice daily. Patients taking this medicine should also receive a maintenance dose of aspirin (75 - 150mg), unless specifically contraindicated. Patients who took 90mg of this medicine or another ADP receptor inhibitor (ticlopidine, clopidogrel, prasugrel) during the first year after myocardial infarction may continue treatment with 60mg of this medicine. Treatment with this medicine may also be initiated in patients within 2 years after myocardial infarction or within 1 year after discontinuing a prior ADP receptor inhibitor (ticlopidine, clopidogrel, prasugrel). Patients with type 2 diabetes who have undergone percutaneous coronary intervention (PCI) for coronary artery disease: Administer 60mg twice daily. Patients taking this medicine should also receive a maintenance dose of aspirin (75 - 150mg), unless specifically contraindicated. For patients with acute coronary syndrome, refer to the corresponding dosage and administration instructions. Patients with acute ischemic stroke or high-risk transient ischemic attack: Administer an initial dose of 180mg, followed by 90mg twice daily for 30 days. Patients taking this medicine should also receive a maintenance dose of aspirin (75 - 100mg), unless specifically contraindicated. An initial loading dose of aspirin (300-325mg) is recommended for patients with acute ischemic stroke or high-risk transient ischemic attack. This medicine is administered orally and may be taken with or without food. For patients who have difficulty swallowing tablets, crush this medicine into a fine powder, disperse it in half a glass of water, and drink immediately. Rinse the glass with another half glass of water and drink it. This mixture may also be administered through a nasogastric tube (CH8 or larger); it is important to flush the tube with water after administration. When switching from another antiplatelet medicine to this medicine, start this medicine 24 hours after the last dose of the previous medicine. If a dose is missed, take only 1 tablet (the next scheduled dose) at the next scheduled dosing time. No dose adjustment is necessary in patients with renal impairment. No dose adjustment is necessary in patients with mild hepatic impairment. This medicine has not been studied in patients with severe hepatic impairment, and experience in patients with moderate hepatic impairment is limited.
The safety and efficacy in pediatric patients younger than 18 years have not been established (see “Other” section).
This drug prevents blood clotting to avoid thrombus formation. However, due to the risk of bleeding, it should not be used in patients with active bleeding or a history of intracranial bleeding. If surgery is planned, the drug should be discontinued at least 5 days prior to minimize bleeding risks. If symptoms of low blood pressure occur, the possibility of bleeding should be suspected, and it is preferable to treat the bleeding without discontinuing the medication. Patients with allergies to this drug or severe liver impairment, as well as those taking certain antibiotics or antiviral medications, should not use it. It should be used with caution in patients at high risk of bleeding, those who have recently undergone surgery or trauma, or those taking other medications that may increase bleeding. In particular, caution is needed when taken with warfarin, as this may increase bleeding risk. Patients with symptoms of dyspnea should be cautious when taking this drug, especially those with asthma or chronic obstructive pulmonary disease (COPD). Patients at risk of bradycardia (slow heart rate) should be careful, and extra caution is warranted when this drug is used with other agents that may cause bradycardia. Pregnant or breastfeeding women are advised to avoid pregnancy or discontinue breastfeeding while taking this drug. Safety and efficacy in children under the age of 18 have not been established and it is not recommended. Overdosage may increase the risk of bleeding, and there is currently no antidote for this drug, making adequate treatment necessary if bleeding occurs. This drug may interact with other medications, so anyone taking other drugs must inform their healthcare provider. Dizziness or confusion may occur while driving or operating machinery, so caution is advised. Discontinuation of this drug may increase the risk of cardiovascular events, hence it should be continued according to the physician's instructions.
