Effect of fluid control and waste removal in patients with chronic kidney failure
Peritoneal dialysis solution is used by patients with chronic kidney failure to remove waste from the blood. This solution facilitates the expulsion of unnecessary substances and excess fluid from the body via the peritoneum. Thus, it helps maintain homeostasis and correct electrolyte imbalances even when kidney function is impaired. Peritoneal dialysis solution is primarily used for the treatment of chronic kidney failure.
This medication must be administered intra-abdominally, typically 2000 mL is infused into the abdomen four times a day and drained 2-10 hours later. The dosage should be adjusted individually according to the patient. If pain due to abdominal distension occurs during peritoneal dialysis, the infusion amount should be initially reduced to 500-1500 mL per session. For patients with no remaining kidney function and who have a larger physique, the dosage may be increased, and for such patients or patients who can tolerate larger infusion amounts, the amount may be increased to up to 2500 mL per session.
For children, the dosage should be adjusted according to age, physique, and weight (30-40 mL/kg).
This medication is a dialysis solution used to remove waste and excess fluid from the body through peritoneal dialysis. Stopping peritoneal dialysis prematurely may pose a risk to life if kidney replacement therapy is not conducted, hence caution is advised. This medication should not be used by patients with severe hypokalemia or hypercalcemia, hypotension, recent abdominal surgery or injury, severe abdominal inflammation, inflammatory bowel disease, peritonitis, abdominal fistula, intra-abdominal tumors, hernias, intestinal obstruction, lung disease, sepsis, hyperlipidemia, or severe weight loss. Additionally, this medication should be avoided in patients who cannot physically or mentally perform peritoneal dialysis properly. Caution should be exercised when using this medication in patients experiencing electrolyte loss due to vomiting or diarrhea, patients on calcium-containing medications, patients using digitalis preparations, patients taking diuretics, and diabetic patients, and if necessary, the electrolyte concentration of the dialysis solution or the dosage of blood glucose control medications should be adjusted. This medication plays a role in maintaining electrolyte balance but may cause adverse reactions such as hypokalemia, hypercalcemia, dehydration, fluid overload, blood glucose elevation, hyperlipidemia, and weight gain. Complications related to peritoneal dialysis may also occur, such as peritonitis, catheter infections, diarrhea, constipation, dyspnea, hernias, abdominal distension, shoulder pain, and encapsulating peritoneal sclerosis; if any unusual symptoms arise, consult a physician immediately. Prolonged peritoneal dialysis may impair peritoneal function and cause loss of protein and vitamins; therefore, nutritional status must be well managed. Regular monitoring of weight, blood electrolyte levels, protein levels, kidney function, etc., is important. This medication may affect the efficacy of other drugs, so dosage adjustments of concomitant medications may be necessary. Caution is required when using this medication during pregnancy or lactation, and in children, the dosage should be adjusted based on weight and age. The elderly are at higher risk for hernias and should be carefully evaluated before peritoneal dialysis. In the case of overdose, the dialysis solution should be drained to correct excess fluid and electrolyte imbalances, and the solution should be used within 24 hours after mixing. The solution should be stored in a tightly closed container to prevent exposure to air or moisture. Avoid using if the container is damaged or if the solution is cloudy, and handle the dialysis solution in a sterile manner to reduce infection risk. While this medication has a safety level due to its electrolyte composition similar to that of blood, careful management and regular monitoring are necessary during peritoneal dialysis.
